The concept of dual-graft liver transplantation was introduced to overcome the discrepancy between liver transplantation demand and liver donation. Dual-graft transplantation also mitigates cumulative family risk by decreasing individual donor risk through minimization of the resected liver volume from each donor. Here, we describe the first two cases performed in Saudi Arabia wherein a dual-graft living donor liver transplantation was facilitated by the use of one left lobe graft and one left lateral segment in both cases. These are the first two cases of dual-graft liver transplantation repo

Fifty years since the first successful human heart transplant from a non-heart beating donor, this concept of heart transplantation from donation after circulatory determined death (DCD) promises to be one of the most exciting developments in heart transplantation. Heart transplantation has established itself as the best therapeutic option for patients with end-stage heart failure, with the opportunity to provide these patients with a near-normal quality of life. However, this treatment is severely limited by the availability of suitable donor hearts. In recent times, heart transplantation has

BACKGROUND: Parenting and children's adjustment were examined in 30 surrogacy families, 31 egg donation families, 35 donor insemination families, and 53 natural conception families.
METHODS: Parenting was assessed at age 3 by a standardized interview designed to assess quality of parenting and by questionnaire measures of anxiety, depression, and marital quality. Children's adjustment was assessed at ages 3, 7, and 10 using the Strengths and Difficulties Questionnaire (SDQ).
RESULTS: Although children born through reproductive donation obtained SDQ scores within the normal range, surrogacy c

Access to lung transplantation is severely limited by a scarcity of suitable donors, resulting in increasing numbers of deaths on the heart and lung transplant waiting lists, and strict selection criteria for recipients. Unlike some other solid organs, the lung may be ideally suited to retrieval for transplant following substantial intervals after circulatory arrest. This may be because lung parenchymal cells do not rely on perfusion for cellular respiration. This review outlines the relevant published experimental data that addresses the concept that lungs might be suitable for transplant eve

BACKGROUND: Over the past 2 decades the characteristics of women giving birth in the United States and the nature of the births themselves have changed dramatically, with increases in older maternal age, plural births, cesarean deliveries, and conception from infertility treatment.
OBJECTIVE: We sought to evaluate the risk of severe maternal morbidity by maternal fertility status, and for in vitro fertilization pregnancies, by oocyte source and embryo state combinations.
STUDY DESIGN: Women in 8 states who underwent in vitro fertilization cycles resulting in a live birth during 2004 through

Bone marrow-derived cell (BMDC) therapy has numerous applications as potential biological cells for use in regenerative medicine. Here, we present an original case of endometrial atrophy associated with genital tuberculosis in a woman who achieved a live birth with BMDC. This 27-year-old woman came to our center with endometrial atrophy and primary infertility. She had a past history of genital tuberculosis and amenorrhea. Her husband's semen quality was normal. The patient was counseled for hysteroscopy due to thin endometrium and advised in vitro fertilization (IVF) with donor eggs in lieu o

OBJECTIVE: To discuss the technical and anatomical analysis and design of an osteocutaneous allograft transplant incorporating the donor maxilla and the execution of the operative protocol during the transplant.
METHODS: The Cleveland Clinic reported the world's first successful combined face and maxilla transplant in December 2008. Unlike the 3 prior face transplants, this surgical procedure was done as a salvage operation in a patient who had undergone 23 major reconstructive procedures. The additional complexity due to significant postoperative scarring and recipient vessel depletion prese

Both chemotherapy and radiotherapy can cause infertility in a patient undergoing cancer therapy. As both the ovaries and uterus are involved in female patients, the effects are profound. Where cryopreservation is not used or is unavailable, reproductive options include spontaneous conception if fertility returns, donor gametes, adoption, and surrogacy. Semen, oocyte, embryo, and ovarian tissue cryopreservation all offer an opportunity for a pregnancy after cancer cure if natural fertility is not possible. The risks of fertility preservation are significant for women due to the risks of surgica

Recently, concepts for the transplantation of hematopoietic stem cells have changed dramatically. High-dose myeloablative conditioning regimens are in the process of being replaced by immuno-suppressive regimens, ablating host myelopoiesis, and neoplastic cells by the co-transplanted donor lymphocytes. Furthermore, the presence of stem cells in the bone marrow, capable of differentiating into a variety of nonhematopoietic tissues as well as the presence of cells in other organs, capable of differentiating into hematopoietic cells, has led to the novel concept of the plasticity of stem cells de

We describe a case of living unrelated renal transplantation from a non-malignant hyperthermia (MH) susceptible donor to an MH-susceptible recipient, along with its intraoperative and perioperative considerations. The renal transplant recipient reported a personal history of MH requiring intensive care unit admission. A nontriggering anesthetic was therefore chosen for the unrelated donor to avoid possible triggering via the transplanted kidney to the MH-susceptible recipient. This case provides a unique opportunity to review the concepts of antigen transfer related to transplant anesthesia, a

BACKGROUND: Artificial insemination with sperm is used to improve the chances of conception for various causes of infertility. Traditionally, sperm is deposited in or around the endocervical canal (cervical insemination - CI). Some studies reported higher pregnancy rates if sperm was deposited in the uterine cavity itself (intrauterine insemination - IUI), but most were uncontrolled. However the cost and the risks (infection and anaphylaxis) of IUI may also be higher.
OBJECTIVES: The objective of this review was to assess the effects of depositing donor sperm in the uterine cavity (intrauteri

BACKGROUND/OBJECTIVES: Early detection of cardiometabolic irregularities is crucial for averting cardiovascular illness; however, demographic cohorts that consistently engage with healthcare systems like habitual blood donors are inadequately leveraged for metabolic monitoring.
METHODS: This study performed lipid profiling and cardiovascular risk assessment among blood donors identified with visually lipemic plasma during routine screening, in order to explore metabolic variability within this selected donor subgroup. Of 13,818 screened donors, 160 with lipemic plasma were included, and multi

BACKGROUND: To understand repeated donation, it is crucial to know its meaning. This issue was explored by asking a sample of regular donors to explain why they maintain this behavior.
STUDY DESIGN AND METHODS: Discourse analysis was conducted in five groups of regular donors, selected according to the setting and number of their donations (from 1990 to 2001), on their motivation, attitudes, commitment to donation, personality, and self-concept.
RESULTS: Three discursive positions were distinguished in relation to the different cultural meanings attributed to blood donation. Blood donation w

Since the first kidney paired donation (KPD) transplant in the United States in 1999, the volume and scope of KPD has expanded substantially, accounting for nearly 20% of living donor kidney transplants in 2021-2022. This review article discusses the practical and ethical issues specific to paired donor exchange that patients, transplant centers, and exchange programs commonly encounter. Access to paired donor exchange and education of candidates regarding the potential benefits, risks, and logistics of KPD are important considerations. Transplant centers and patients must consider practical i

A retrospective analysis was carried out after ovulation induction in donors, and treatment outcome in donors and recipients, relating to variables such as: age, protocol of ovarian stimulation, transfer of fresh or cryopreserved embryos, number of embryos transferred including quality and etiology of donor and recipient infertility. The recipients underwent 214 cycles of embryo transfer from 82 different donors. Forty-five (21%) pregnancies were recorded. The age of the recipients significantly affected the conception rate (pregnancy rate of 30% in those aged < 30 years, compared to 9.7% in t

Modern concepts of cardiac transplantation are reviewed. Selection of potential recipients and donors is discussed with particular attention to those aspects of procurement that involve coordination with other transplant teams. Technical aspects of the operation, postoperative management, immunologic regimens, and late results, including physiologic testing and complications, are also reviewed.
