Phantom menace: novel psychoactive substances and the UK Armed Forces.

Novel psychoactive substances (NPS) encompass a large group of synthesised compounds specifically designed to mimic traditional recreational drugs. Current UK Armed Forces compulsory drug testing does not screen for these substances, making them tempting to the small proportion of UK Armed Forces personnel who indulge in recreational drug use. The acute and chronic sequelae of NPS misuse are widely variable and associated with high morbidity. In this paper, we discuss NPS pharmacology and clinical presentation. We describe toxidromes and management of patients who have misused NPS.Finally, we

The drugs most diverted for toxicomanic use from their therapeutic indication are principally the opioids or related substances, with codein … rphine, can be implicated in severe adverse effects. Twenty-one lethal intoxications linked to such drug misuse have already been reported. However, the addictive drug potential is not the only explanation for drugs diverted for toxicomanic use. It is also associated with a polytoxicomania, a symptom of pre-existing difficulties, particularly familial, social …

Relations between medical and nonmedical prescription stimulant misuse, cannabis use, alcohol use, and related consequences among college students.

OBJECTIVE: Prescription stimulant misuse (PSM) is prevalent among young adults, particularly college students. This study utilized structural equation modeling to examine differences in alcohol use, cannabis use, and alcohol consequences across PSM groups, (no use, appropriate use, medical misuse [i.e., not using one's own prescription as directed], and nonmedical misuse [i.e., using prescription stimulants not prescribed to oneself]) while controlling for psychological factors and other prescription drug use. METHOD: Participants (N = 1,692, Mage = 19.1, 52.5% male, 70.5% White, 19.5% Hispan

Overdose mortality rates for opioids and stimulant drugs are substantially higher in men than in women: state-level analysis.

Drug overdoses from opioids and stimulants are a major cause of mortality in the United States. It is unclear if there are stable sex differences in overdose mortality for these drugs across states, whether these differ across the lifespan, and if so, whether they can be accounted for by different levels of drug misuse. This was a state-level analysis of epidemiological data on overdose mortality, across 10-year age bins (age range: 15-74), using the CDC WONDER platform for decedents in the United States in 2020-1. The outcome measure was rate of overdose death (per 100,000) for: synthetic opi…

An ethnographic decision model of the initiation of gabapentin misuse among prescription and/or illicit opioid (mis)user.

AIMS: Gabapentin is used in the treatment of seizures and neuralgia, and it is prescribed off-label to treat substance use disorders and withdrawal symptoms. Recent research documents misuse of gabapentin, especially among prescription opioid misusers. The present study contributes to this literature by examining the initiation of gabapentin misuse. METHODS: Qualitative interviews were conducted with prescription and/or illicit opioid (mis)users who reported a history of gabapentin misuse (N = 62) and who did not (N = 29). During semi-structured interviews, respondents provided descriptions o

A Mixed-Methods Study of Motives for Benzodiazepine Misuse Among Adults Receiving Substance Use Disorder Treatment.

BACKGROUND: Benzodiazepine misuse is common among individuals with substance use disorders (SUDs) and is associated with poorer clinical outcomes. As benzodiazepines have heterogeneous effects, standard measures of substance use motives may overlook key reasons for misuse. Understanding motives better can inform intervention development. OBJECTIVES: Our overarching objective was to characterize motives for benzodiazepine misuse among individuals with SUDs and develop a preliminary measure of these motives. METHODS: We used a two-study, mixed methods design. Participants were recruited from a

A single-center, retrospective analysis evaluating the utilization of the opioid risk tool in opioid-treated cancer patients.

The Opioid Risk Tool (ORT) is a screening tool used to assess risk of opioid misuse by stratifying aberrant drug-seeking behaviors and/or identifying known risk factors for drug abuse. The objectives of this study were to risk stratify opioid misuse in a cancer pain population and determine the most common patient risk factors associated with misuse utilizing the ORT. This was a retrospective analysis conducted at an academic comprehensive cancer center. Patients were referred by an oncologist or hematologist to an outpatient palliative care clinic. One-hundred and fourteen patients with cance

Illicit Drug Use in Older Adults: An Invisible Epidemic?

Illicit drug use/misuse among older adults is understudied, although current trends point to older adults being the fastest-growing segment in the United States and other developed countries. There is a need for further insight into drug use patterns in older adults, who face their own set of socioeconomic, medical, and psychiatric problems. We reviewed the literature for data related to use/misuse of heroin and stimulants (cocaine, amphetamines, and methamphetamines) among people over the age of 40 years. We focused on prevalence rates of use/misuse of these substances, comorbidities, diagnos

Best practice in substance misuse.

While substance misuse by adolescents in the UK has declined over the last decade, the UK continues to have some of the highest rates of alcohol and drug use in Europe. Many young people will try smoking and drinking alcohol during their adolescence and a significant minority will misuse alcohol and illicit drugs. This behaviour remains a significant cause for concern owing to its associated risks to the health and wellbeing of adolescents. Guidance is emerging regarding good practice in the assessment and management of adolescent substance misuse. Paediatricians may encounter substance-misusi
