Efficacy of Vagal Nerve Stimulation for Drug-Resistant Epilepsy: Is it the Stimulation or Medication?

BACKGROUND: Vagus nerve stimulation (VNS) therapy has been widely recognized as an alternative for the treatment of drug-resistant epilepsy, although modification of antiepileptic drugs (AEDs) during VNS treatment could explain the improvement in patients. METHODS: We retrospectively assessed the efficacy of VNS in 30 adult patients with epilepsy treated with >6 months of follow-up. The criteria for implantation were the following: (1) not a candidate for resective epilepsy surgery, (2) drug-resistant epilepsy, (3) impairment of quality of life, (4) no other option of treatment, and (5) patie

[Drug resistance in partial epilepsy: epidemiology, mechanisms, pharmacogenetics and therapeutical aspects].

It has been established that 20-30% of epilepsies are not controlled by antiepileptic drugs. Drug resistance is associated with several major problems, including prognosis, cognitive function, behavior, mortality, cost and quality of life. Apart from classic risk factors for drug resistance, such as neurological, psychiatric, imaging, EEG abnormalities, a high frequency of seizures before medical therapy and complex febrile convulsions, the potential role of multidrug transporters as well as their genetic control and the altered sensitivity of neuronal drug receptors has gained growing attenti

Major vault protein, a marker of drug resistance, is upregulated in refractory epilepsy.

PURPOSE: The molecular basis of drug resistance in epilepsy is being explored. Two proteins associated with drug resistance in cancer, P-glycoprotein and multidrug resistance-associated protein 1, are upregulated in human epileptogenic pathologies. Other proteins associated with resistance in cancer include major vault protein (MVP) and breast cancer resistance protein (BCRP). We hypothesized that these proteins would also be upregulated in human epileptogenic pathologies. METHODS: Hippocampal sclerosis (HS), focal cortical dysplasia (FCD), and dysembryoplastic neuroepithelial tumor (DNT) wer

[The comparison of ketamine with methohexital and thiopental in the intraoperative EEG in drug-resistant epilepsy].

UNLABELLED: The acute (ECoG) was examined in 291 patients with intractable epilepsy, without structural brain lesion--from 1971 to 1997. Temporal lobectomy was performed in 198 cases and extratemporal (frontal, parietal or occipital) in remaining 93 cases to achieve seizure control. Epileptic foci was activated during acute ECoG by i.v. administration of ketamine (154 cases) or short-acting barbiturates--methohexital (110 cases) and thiopental (27 cases). RESULTS: Ketamine significantly more often caused ECoG identified electrographic seizures than methohexital: p = 0, 00001 or thiopental, wh

Chemical kindling: implications for antiepileptic drugs - sensitive and resistant epilepsy models.

The efficacy of phenobarbital (PB) and phenytoin (PHT) was evaluated against the convulsions in chemically (picrotoxin, PTX) kindled rats. Two protocols were used: assessment of seizures immediately after the completion of the kindling procedure and after the 2-week postkindling PTX-free period, as compared with acute PTX seizures. Kindled convulsions were more sensitive than acute PTX seizures to the antiepileptic action of PB and PHT. On the other hand, the "postkindling state" was characterized by the enhancement of the severity of the convulsions in comparison with both kindled and acute P

Real-world use of Cenobamate in pediatric drug-resistant epilepsy: A European multicenter retrospective study.

Pediatric drug-resistant epilepsy (DRE) remains a significant clinical challenge, with few effective pharmacological options. This European multicenter retrospective study assessed the real-world efficacy, tolerability, and retention of off-label cenobamate (CNB) in 108 children and adolescents with DRE (median age: 13.83 years, range 3.9-19.8). Overall, 57/108 patients (52.8%) achieved a ≥50% seizure reduction, including 20/108 (18.5%) who were seizure-free during the documented interval between visits. The median monthly seizure frequency declined from 30 to 9. Median follow-up was 581.5 day

[18F]DPA-714 PET in presurgical evaluation of drug-resistant epilepsy: from clinical to pathology.

INTRODUCTION: Fluorine 18 ([18F])–labeled DPA-714 binds to the translocator protein (TSPO), which can be used for monitoring activated glial cells during inflammatory process, and be potentially helpful for localizing the epileptogenic zone. In this study, we aimed to analyze the application of [18F] DPA-714 PET in preoperative evaluation of epilepsy, and to validate the results through surgical outcome and pathological results. METHODS: Patients who underwent epilepsy surgery with both preoperative [18F]DPA-714 PET and 18F-fluorodeoxyglucose ([18F]FDG) PET were retrospectively reviewed. We vi

Delayed Surgical Evaluation After Drug-Resistant Epilepsy Diagnosis Worsens Outcomes in Children: A Multicenter Study.

BACKGROUND AND OBJECTIVES: Despite its proven effectiveness, epilepsy surgery for drug-resistant epilepsy (DRE) remains underutilized and frequently delayed. Previous studies of epilepsy duration before surgery-using variable delay thresholds (2-20 years)-were small, single-center cohorts focused mainly on temporal/frontal lobe epilepsy, showed better seizure freedom with earlier surgery, but did not distinguish total epilepsy duration from DRE duration. As contemporary epilepsy surgery now includes broader indications and emphasizes faster evaluation, the timing and impact of evaluation acros

Proteome profiling of cerebrospinal fluid-derived extracellular vesicles reveals potential biomarkers for drug-resistant epilepsy.

BACKGROUND: Epilepsy is one of the most common neurological disorders in humans and in dogs. Treatment currently focuses on alleviating symptoms, and a wide range of anti-seizure medications (ASMs) is available. Still, over one-third of patients have an inadequate response to ASM. The proteome of cerebrospinal fluid (CSF)-derived extracellular vesicles (EVs) offers a potential source of biomarkers for drug-resistant epilepsy (DRE). METHODS: We utilised a spontaneous canine epilepsy model to study the proteomic content of CSF-derived EVs as a source of biomarkers for DRE. We included 37 drug-n

Optimizing outcomes in drug-resistant mesial temporal lobe epilepsy patients undergoing stereoelectroencephalography-guided radiofrequency thermocoagulation.

BACKGROUND: Mesial temporal lobe epilepsy (MTLE) epileptiform discharges have been reported to arise from the hippocampus or the extrahippocampal medial temporal cortex, such as the amygdala, and then propagate to the temporal lobe cortex. The surgical ablation of which of these structures would result in a better postoperative outcome is debatable. OBJECTIVE: To assess the possible factors that might have influenced the postoperative outcome of a group of drug-resistant mesial MTLE patients who underwent stereoelectroencephalography (SEEG)-guided radiofrequency thermocoagulation (RFTC). Psychiatric comorbidities before and after cannabidiol treatment in adult patients with drug resistant focal epilepsy.

Cannabidiol oil (CBD) has been approved as an antiseizure medication for the treatment of drug -resistant epilepsy in pediatric patients in 2018 for some special types of epilepsy. Since this time its use was extended to other forms of epilepsy. However, to date, there are few publications on the use of CBD in adult patients with drug-resistant focal epilepsy and psychiatric comorbidities. We conducted a prospective, observational, open cohort study, with a before-after design, in adult patients, we assessed the effectiveness, dosage, and tolerance of adjunctive CBD treatment. Our study conclu
