Artificial Intelligence Models for Dysphonia Patient Education.

OBJECTIVE: Evaluate and compare artificial intelligence (AI) chatbot responses to queries extrapolated from statements by Dysphonia International (DI). STUDY DESIGN: Cross-sectional analysis. SETTING: Online using ChatGPT, Google AI Overview (GAO), and DI. METHODS: Two board-certified otolaryngologists and an otolaryngology resident each blindly rated ChatGPT, GAO, and DI responses to spasmodic dysphonia, muscle tension dysphonia, and vocal tremor questions. Response comprehensiveness, accuracy, appropriateness to patients, and patient safety were graded on a 5-point Likert scale. Flesch-Ki…

Laryngeal injury and dysphonia after endotracheal intubation.

We carried out a prospective case series study in order to evaluate the laryngeal complications of the endotracheal tube. Two hundred patients aged 15 years and above who were subjected to endotracheal intubation for less than 5 hours were enrolled in the study. The data were collected from the Al-Salam Teaching Hospital in Mosul, Iraq. A preoperative assessment was accomplished clinically using 70º and/or 90º Hopkins rods or fiber optic laryngoscopy. As part of the assessment, the patients' voices were recorded as well. Five to seven days after the procedure, the same assessment was repeated

Persistent Dysphonia in Hospitalized COVID-19 Patients.

INTRODUCTION: The main objective of this study is to estimate the prevalence of persistent dysphonia in hospitalised COVID-19 patients. METHODS: Data were collected from those COVID-19 patients who, during the months of March to April 2020, were hospitalised in ward or intensive care unit at the University Hospital of Fuenlabrada. Patients with dysphonia prior to SARS-CoV-2 were excluded. Informed consent was obtained orally by a telephone call, as well as clinical and epidemiological data. Patients who reported persistent dysphonia were assessed using the Voice Handicap Index 10, the maximum…

Dysphonia after Bevacizumab Rechallenge: A Case Report.

Inhibition of vascular endothelial growth factor (VEGF) signaling, an initiator of tumor angiogenesis, inhibits tumor growth and invasion. Bevacizumab, a monoclonal antibody to VEGF, in common use as an adjunct to standard chemotherapy like irinotecan in advanced colorectal cancer, also affects the normal (nontumor) vasculature. Dysphonia or voice changes have been anecdotally reported in patients that have been exposed to antiangiogenics. In this case report, we present an occurrence of severe dysphonia in a 60-year-old male with metastatic colorectal cancer after reintroduction of irinotecan…

Functional dysphonia: strategies to improve patient outcomes.

Functional dysphonia (FD) refers to a voice problem in the absence of a physical condition. It is a multifaceted voice disorder. There is no consensus with regard to its definition and inclusion criteria for diagnosis. FD has many predisposing and precipitating factors, which may include genetic susceptibility, psychological traits, and the vocal behavior itself. The assessment of voice disorders should be multidimensional. In addition to the clinical examination, auditory-perceptual, acoustic, and self-assessment analyses are very important. Self-assessment was introduced in the field of voic…

Relationship between fatigue, perfectionism, and functional dysphonia.

OBJECTIVES: Increased levels of fatigue and perfectionism were noted during evaluation of cognitive behavioral therapy for the treatment of functional dysphonia. The investigators thus aimed to explore levels of general fatigue and perfectionism in patients with functional dysphonia and controls. STUDY DESIGN: Case-control study. SETTING: Teaching hospital, United Kingdom. SUBJECTS AND METHODS: Patients recruited through speech therapy were asked to recruit a friend as a control, of the same sex and within 5 years of their age. An 11-point fatigue questionnaire, previously validated on a no…

Muscle tension dysphonia in Vietnamese female teachers.

There has been no published research on muscle tension dysphonia (MTD) in speakers who use a tonal language. Using a sample of 47 Northern Vietnamese female primary school teachers with MTD, we aimed to discover whether professional voice users of tonal languages presented with the same symptoms of MTD as speakers of nontonal languages and whether they presented with additional symptoms as a result of speaking a tonal language. The vocal characteristics were assessed by use of a questionnaire and expert perceptual evaluation. Laryngeal features were assessed by photolaryngoscopy. The results s…

Describing pediatric dysphonia with nonlinear dynamic parameters.

OBJECTIVE: Nonlinear dynamic analysis has emerged as a reliable and objective tool for assessing voice disorders. However, it has only been tested on adult populations. In the present study, nonlinear dynamic analysis was applied to normal and dysphonic pediatric populations with the goal of collecting normative data. Jitter analysis was also applied in order to compare nonlinear dynamic and perturbation measures. This study's findings will be useful in creating standards for the use of nonlinear dynamic analysis as a tool to describe dysphonia in the pediatric population. METHODS: The study …

Thyroplasty for adductor spasmodic dysphonia: further experiences.

OBJECTIVES: Two different surgical techniques for midline lateralization thyroplasty (type 2 thyroplasty) for adductor spasmodic dysphonia (SD) have been described, one using a composite graft and the other without. Indications and results for each method among different types of SD were compared, together with the theoretical background for the surgery. STUDY DESIGN: Retrospective. METHODS: The medical records of six patients with SD together with the fiberscopic video recording of laryngeal findings before, during, and after surgery were reviewed. The intraoperative video recordings of the…

Variations in adductor spasmodic dysphonia: acoustic evidence.

Acoustic analysis was used to gain information about the normal, as well as the abnormal acoustic events associated with adductor spasmodic dysphonia (ADSD). This analysis was completed to determine whether specific acoustic events could be used to differentiate the voice of individuals with ADSD from those with normal voice. A group comparison between 14 women diagnosed with ADSD and 14 women (age-matched) with no evidence of vocal pathology or vocal dysfunction was completed. Phonatory breaks, aperiodicity, and frequency shifts, acoustic parameters previously identified in ADSD, were found t…

Spasmodic dysphonia: new diagnosis and treatment opportunities.

The ability to communicate is a basic need. Spasmodic dysphonia, a rare dystonia focal to the larynx, affects the vocal muscles resulting in abnormal sound and speech production. The voice may become hoarse, strained, strangled, tremulous, whispered, or aphonic. Lack of appropriate diagnosis along with the continued use of an abnormal voice often results in frustration, anxiety, and depression. Improved diagnostic ability and unique treatment with botulinum toxin injection (Botox) are now available to patients in several large medical centers across the United States and Canada. A knowledgeabl…

EMG biofeedback treatment of pediatric hyperfunctional dysphonia.

A 9-year-old boy with a long history of impaired communication resulting from hyperfunctional dysphonia and vocal nodules had proven to be unresponsive to traditional voice therapy. In this study, he engaged in biweekly visual EMG biofeedback training to reduce laryngeal muscle tension. In a multiple baseline design across two responses with changing criteria, results demonstrated sequential stepwise reductions in muscle tension with each new criterion. Concomitant improvements were found in laryngeal aerodynamic functioning and in voice quality. The treatment also resulted in the elimination …

Emotional considerations in spasmodic dysphonia: psychometric quantification.

Whereas recent research has focused upon neuromotor aspects of spasmodic dysphonia, the disorder has historically been regarded as having a psychogenic basis. Relatively little empirical evidence, however, has been offered either to support or refute that claim. The present study examines emotional characteristics of 18 female spasmodic dysphonic subjects in comparison to matched normal controls across psychometric measures of depression, anxiety, and somatic complaints. Statistically significant differences were noted between groups for all measures and over half of the dysphonic subjects exh…
