Otoplasty for prominent ears.

A technique for correction of prominent ears is described. It combines the advantages of simplicity and accuracy and provides satisfactory results. Essential points of the technique are perichondrial cartilage scratching at the superior crus and antihelical fold according to Stenström, mattress sutures applied to the perichondrium and soft tissues at the medial surface, and to these we add the trimming of the tail of the helix, thinning of the antitragus, and a double-spindle skin excision at the medial auricular surface. In patients with insufficient curling of the helical rim ("shell-ear" de…

Encephalocele of the middle ear.

A case of congenital encephalocele of the middle ear presenting as a conductive hearing loss is reported. In retrospect it is possible that CT scanning with metrizimide contrast studies may have been helpful in establishing the diagnosis (Cooper and Kassel, 1982). However, it is stressed that even in the absence of radiological abnormality of the temporal bone, surgical exploration of the middle cranial fossa should be undertaken.

Immunobiology of the inner ear.

Immunologic mechanisms may play an etiologic role in ear disease, and many disorders long considered idiopathic are now being examined for such an immunologic basis. While it is possible that many of these idiopathic diseases will ultimately prove not to be immunologically mediated, the outcome of these research endeavors will certainly clarify some of the basic mechanisms of host immunity involved in ear disease. The immunobiology of diseases that affect the inner ear is discussed from both experimental and clinical viewpoints.

Congenital cholesteatoma of the ear.

Congenital cholesteatoma of the ear is a capricious and challenging problem for the otologist. We present 21 cases managed at the Mayo Clinic from 1961 to 1983. In all but one unusual case, the cholesteatoma developed behind an intact tympanic membrane and there was no history of aural infections. Preoperative symptoms and signs were predominantly conductive deafness with either a normal tympanic membrane or an opaque, whitish appearance to the drum. Disease was located in the middle ear or in the middle ear and mastoid, except in one patient who had cholesteatoma extending into the petrous ap…

The unsafeness of 'safe' ears.

Active chronic otitis media affects 0.5 per cent of adults and has two main variants. The first is when there is a squamous epithelial retraction pocket or cholesteatoma. The second variant is when the disease is primarily of the mucosa of the middle ear and mastoid air-cell system. Classically a cholesteatoma is considered 'unsafe' because of the risk of complications, particularly intracranial infection. Mucosal disease, on the other hand, is considered 'safe' because complications are thought to be rare. Surgery in the form of a modified radical mastoidectomy is considered to make active ch…

[Actinomycosis of the middle ear].

A recent patient with actinomycosis of the middle ear has stimulated us to call attention to this very rare disease. The differential diagnosis, pathophysiology, the pathways of spread, diagnosis and therapy of this disease are discussed. Combined therapy by antibiotics such as ampicillin with clindamycin is the best treatment currently available. Surgery is confined to taking a biopsy for histology and to the draining of inflammatory foci.

[Metastasis of a hypernephrome to the ear].

Renal cell carcinoma (Grawitz tumor, hypernephroma), may metastasize to the head and neck region at different stages of its evolution. The authors present a case of a 79 year old woman who consulted for otalgia, deafness, and tinnitus. A polyp in the external auditory canal proved to be a metastasis of a renal carcinoma. The patient underwent a nephrectomy eight years previously and had no problems since. The data on otolaryngological metastasis of this tumor as well as its pathogenesis are reviewed.
