INTRODUCTION: Surgical treatment of lateral epicondylitits (LE) has not been investigated sufficiently to allow general guidelines, nor has non-invasive treatment produced sufficiently good results. We present the results of 31 mini-tenotomies performed to treat lateral epicondylitis.
MATERIALS AND METHODS: We performed a retrospective follow-up study of 31 elbows treated with mini-tenotomy for LE. The average period with symptoms was 125 weeks, the average follow-up 73 weeks. The mini-tenotomies were performed under local anaesthesia and the extensor aponeurosis was cut close to the epicondy

Elbow arthroscopy has become to be the most useful tool for the treatment of many intra-articular affections of the elbow. Radiological statement is necessary including plain radiographs and CT or MR arthrography before performing arthroscopy. Loose bodies are the more frequent indication, they are often related with an other intra-articular pathology. The others indications for an elbow arthroscopy can be, osteochondritis dissecans, synovial fringe, synovitis especially rheumatoid arthritis and arthritic elbow. The treatment of this different pathologies is discribe keeping in mind the potent

OBJECTIVE: To review the literature on nonsurgical treatment of tennis elbow.
METHODS: We searched Medline for all randomized controlled trials (RCTs), controlled clinical trials (CCTs) and literature reviews published from 1966 to December 2003 on nonsurgical treatment of tennis elbow. We used the keys words controlled clinical trial, tennis elbow on lateral epicondylitis, and treatment. We found 46 reports of RCTs and CCTs on 14 nonsurgical treatments and 11 literature reviews.
RESULTS: Corticosteroid injection is the best treatment option for the short term. However, beneficial effects pe

Skeletally immature throwing athletes are injured when they throw too frequently or use throwing styles and pitches that overstress their elbows. Despite safety guidelines for throwing that recommend throwing more than 300, but less than 600, pitches per season, compliance is almost impossible to monitor, given multiple opportunities for throwing abuse away from organized, supervised league play. All throwers should avoid the side-arm throwing style. Pitchers should not play in multiple leagues, should not play hard-throwing positions when not pitching, and should not pitch when having elbow o

BACKGROUND: This review is one in a series of reviews of interventions for lateral elbow pain. Lateral elbow pain, or tennis elbow, is a common condition causing pain in the lateral elbow and forearm and lack of strength and function of the elbow and wrist. Surgery is sometimes recommended in treating chronic cases of lateral elbow pain where other less invasive interventions have failed. Various operations have been described based upon the surgeon's concept of the pathological entity. The most described surgical procedures involve release of the extensor carpi radialis brevis (ECRB) from the

BACKGROUND: To ascertain whether there were any differences in the outcome of release of the common extensor origin and release of the common extensor origin and drilling of the lateral epicondyle in the management of recalcitrant tennis elbow.
METHODS: One hundred and eleven patients (125 elbows; 40 males, median age: 47 years; 71 females, median age: 45 years) entered the study. The minimum follow-up period was 2 years (average follow-up: 52.8 months). Patients were reviewed at 2 and 6 or 8 weeks, and 3 and 6 months after surgery. If a complication ensued, or if a patient did not report imp

We have reviewed 66 consecutive Souter-Strathclyde arthroplasties of the elbow implanted in 59 patients between 1982 and 1993. Thirteen patients (15 elbows) (19.6%) died. Sixteen elbows (24.2%) were revised, six for aseptic loosening (9%), four (6%) because of fracture or loosening after a fracture, three (4.5%) for infection and three (4.5%) for dislocation. Four patients refused to attend for review. In 33 elbows with a follow-up of 93 months (60 to 167) complete relief of pain was achieved in 22 (67%) when seen at one year. After ten years or more 36% of the elbows were painfree and 64% had

Successful reconstruction of posttraumatic elbow instability depends on restoration of the anatomic contributors to stability. The osseous and articular structures are paramount. The radial head and coronoid should be repaired or reconstructed and the olecranon (proximal ulna) should be repaired in anatomic alignment so that the contour and dimensions of the trochlear notch are restored and the radiocapitellar joint is aligned appropriately. The lateral collateral ligament complex is commonly disrupted and usually can be reattached to its origin from the lateral epicondyle. Patients with longs

The purpose of this study was to review our experience with the treatment of twenty-five infections (in twenty-five patients) after total elbow arthroplasty and to examine indications for salvage of the prosthesis compared with those for resection arthroplasty. The patients were divided into three groups on the basis of treatment. Group I comprised fourteen patients who were managed with multiple, extensive irrigation and debridement procedures with retention of the original components. The primary indication for retention of the prosthesis was evidence that it was well fixed as determined bot

Athletes of all ages and skill levels are increasingly participating in sports involving overhead arm motions, making elbow injuries more common. Among these injuries is lateral epicondylitis, which occurs in over 50% of athletes using overhead arm motions. Lateral epicondylitis is characterised by pain in the area where the common extensor muscles meet the lateral humeral epicondyle. The onset of this pathological condition begins with the excessive use of the wrist extensor musculature. Repetitive microtraumatic injury can lead to mucinoid degeneration of the extensor origin and subsequent f

Approximately 75 percent of all fractures sustained by children occur in the upper extremities and frequently occur during a fall onto an outstretched hand. The majority of these injuries involve the wrist and forearm, but the elbow alone accounts for approximately 10 percent of all fractures in children. Elbow fractures in children are challenging because of the abundance of unossified cartilage and the high potential for limb-threatening damage to neurovascular structures. Common types of elbow fractures include supracondylar, lateral condylar, medial epicondylar, radial neck and transphysea

Significant functional impairment, increasing pain or joint infection are the indications for arthrodesis of the elbow joint. Six patients underwent a compression arthrodesis with internal and external fixation and an additional bone graft in our institution between 1987 and 1993. Out of this series only one patient developed a non-union due to a fall on the extremity. In a series of nine patients treated with external fixation alone between 1978 and 1987, four patients developed a non-union. Unlike other joints, there is no optimal position for arthrodesis of the elbow and the functional defi

Magnetic resonance imaging (MRI) was performed in eight patients under the age of 8 years who suffered elbow fractures, to assess possible fracture extension into the distal nonossified epiphysis of the humerus in seven cases and to determine the displacement and location of the radial head in one case. MRI allowed accurate depiction of the fracture line when it extended into the cartilaginous epiphysis. In four cases, MRI findings were confirmed at surgery. In five cases, surgery was obviated because no articular extension of the fracture was seen on MRI (4 cases) or because no displacement w

Arthroscopy is highly useful for the diagnosis and treatment of loose bodies of the elbow. Not all loose bodies are detected radiographically, and often there are multiple loose bodies, sometimes both anteriorly and posteriorly. When not complicated by an underlying disorder such as arthritis, loose bodies can be removed from any part of the elbow to provide excellent results in a predictable manner. This article describes techniques developed for the safe and effective application of this procedure in the elbow.

Post-traumatic infection of the elbow joint is a serious complication, because the bone structure and the thin soft tissue cover make all actions directed at reconstruction and at calming the infection difficult. A good overall prognosis for later function depends decisively on early diagnosis, stability of the fragments and the earliest possible initiation of functional therapy. If this cannot be achieved, the infection must first be treated by transfixation techniques and the joint function restored subsequently. In late cases with chronic inflammation and destruction, nothing but resection
