Effects of phototherapy on electrooculographic ratio in winter seasonal affective disorder.

Low electrooculographic (EOG) ratios have been reported in patients with seasonal affective disorder (SAD). This study was undertaken to replicate these results and to consider the effects of light therapy on the EOG in SAD patients. Sixteen outpatients with SAD and 16 age-, sex-, and medication-matched control subjects had EOG testing before and after 1 week of light therapy during the winter. The EOG ratios in the SAD patients were only marginally lower than those in the normal control subjects. These differences persisted after light therapy. Although the slightly decreased EOG ratios in SA

Electrophysiologic detection of choroidal detachment in eyes with rhegmatogenous retinal detachment.

The base values of electrooculograms (EOGs) of 39 patients with new, unilateral rhegmatogenous retinal detachment and clear media were studied. None of the patients had a history of ocular surgery. Ophthalmoscopic examinations revealed choroidal detachment in 11 eyes. The mean (+/- SD) amplitude, expressed as a percentage of the amplitude recorded for unaffected fellow eyes, was 116 +/- 31% in the 28 eyes without choroidal detachment, and 39 +/- 10% in the 11 eyes with choroidal detachment. This difference was statistically significant (P < 0.0001). There was no significant correlation in eith

Circadian rhythm of regular spike-wave discharges in childhood absence epilepsy.

Four girls with childhood absence epilepsy with several seizures every day were investigated using an ambulatory cassette EEG. Recordings were started at about 6 pm, and were run continuously for about 22 hours. We studied only the regular and symmetrical 3 Hz spike-wave discharges of at least 5 seconds duration, which are quite similar to or identical with those found in the clinical seizures. Regular spike-wave discharges occurred frequently during wakefulness in 2 cases and during sleep in the other 2 cases: in the latter, they occurred rarely during wakefulness. During wakefulness, we did

Congenital adduction palsy and synergistic divergence: a clinical and electro-oculographic study.

We studied two patients with a peculiar congenital disturbance of ocular motility in which the horizontal movements of the left eye were always opposite the normal expected direction. The common features were: (1) congenital monocular adduction palsy and exotropia of the left eye; (2) simultaneous abduction of both eyes (divergence) on attempted dextroversion; (3) ocular torticollis, head turned to the right; and (4) inverse nystagmus of the left eye, occurring spontaneously as well as during optokinetic and vestibular testing. Clinical and electrooculographic findings suggested a close relati

Eye tracking, schizophrenic symptoms, and schizotypal personality disorder.

Schizophrenic patients and patients with schizotypal personality disorder were significantly more likely than normal controls to demonstrate impaired eye tracking performance. Fifteen of 27 schizophrenics and 15 of 27 schizotypals had impaired eye tracking, compared with 11 of 39 normal controls. In the schizophrenic group, including 10 out-patients in a stable state of relative remission, impaired eye tracking was associated with more severe formal thought disorder and more time spent in psychiatric hospitals. Among stable schizophrenic out-patients, poor eye tracking was related to more seve

Asymmetry in congenital ocular motor apraxia.

A 10-month-old girl presented with apparently unilateral ocular motor apraxia (OMA). Electro-oculography confirmed an inability to generate a saccade for fixation or in response to an optokinetic or vestibular evoked stimulus to the right. Fixation and optokinetic saccades and vestibular evoked nystagmus were present to the left. Magnetic resonance imaging revealed a small cerebellar vermis but no localizing unilateral abnormality or tumour. Subsequent review of video recordings of 16 children with congenital OMA revealed that 5 demonstrated asymmetry in the generation of fixation saccades. Tw

Heart rate and blood pressure changes associated with cataplexy in canine narcolepsy.

Blood pressure and heart rate were monitored in narcoleptic dogs by means of a chronically implanted catheter placed in the descending aorta. Changes in these variables were recorded during spontaneously occurring cataplectic episodes. We found no reliable change in blood pressure associated with cataplexy onset. However, heart rate showed a marked increase prior to the onset of cataplexy, with peak heart rates being reached at or shortly after the disappearance of muscle tone. Autonomic events correlated with increased heart rate may contribute to the triggering of cataplexy in narcoleptics.

[Paralysis of downward gaze and chronic global amnesia as a result of a bilateral thalamo-subthalamic lesion].

A 47 year-old male was found in coma and upon awakening 36 hours later, he showed a selective paralysis of downward gaze and convergence associated to an intellectual impairment. He was euphoric, unconcerned, poorly attentive, and had a severe amnesia for both anterograde and retrograde material. CT scan and NMR demonstrated a bilateral lacunar lesion of thalamic and subthalamic paramedian region. Over the 5 years follow-up, behavioral, personality and amnestic changes remained. On the other hand, supranuclear gaze paralysis improved as documented by EOG recordings with persisting limitations

Acute rubella retinal pigment epitheliitis in an adult.

A 50-year-old man who had been taking betamethasone for 20 days experienced a slight bilateral decrease in visual acuity. There were localized dark-gray atrophic lesions at the posterior pole, accompanied by a diffuse detachment of the sensory retina. The retinal vessels appeared to be normal and only minimal anterior uveitis was present in the more severely affected eye. Fluorescein angiograms showed no masking of choroidal fluorescence in the early phase and late-phase fluorescein angiograms showed multifocal leakage of the dye into the subretinal space. Electro-oculographic findings were ab

[Systematic deviations of saccadic eye movements in Wallenberg syndrome].

A patient suffering from lateral medullary infarction (Wallenberg's syndrome) is presented, exhibiting a striking bias of all saccadic eye movements toward the side of the lesion. Oculographic tracings demonstrate this oculomotor disorder. Other disturbances of ocular motility which resemble this one superficially are discussed. Interruption of fixation leads to a gliding movement veeringly to the side of the infarction. Since all saccades generated under different circumstances are affected, it is contemplated where the pathological signal is intruded into the prenuclear level, to adulterate

[X-linked night-blindness: clinical and electrophysiological investigations including de-ERG in two families (author's transl)].

Patients from two families with X-linked recessive nyctalopia and myopia were investigated. The functional tests included clinical examinations (visual acuity, color sense, visual fields, dark adaptation) and electrophysiological examinations (ac coupled ERG, dc-coupled ERG, EOG). The dc-coupled ERG registrations were done without general anesthesia. In one case, a comparison with results obtained in 1967 was possible. There was no evidence of increase in functional impairment. On the basis of the electrophysiological results, the nature and localization of the defect causing nyctalopia are di

Abnormal eye movements in rubella syndrome.

A patient with a characteristic picture of rubella eye disease is presented. Its interest lies in the associated finding of asymmetric nystagmus and ocular flutter. The presence of flutter would indicate cerebellar or cerebellar pathway disease, a previously unassociated finding.

Galvanic vesitbular test.

Galvanic stimulation of the vestibular system provokes a nystagmus as well as a tendency to fall. The direction of these phenomena depends on the localization of the anode. In this study the value of the minimal current strength (threshold), which is able to provoke nystagmus with a constant frequency during at least 30 sec was determined. Also the effects of the lesions in the vestibular system on these threshold values were measured. The method of bipolar-biaruricular stimulation showed to be the most sensitive one. A striking difference was found in the thresholds in rabbits with a vestibul
