Skin manifestations of food allergy.

The pediatrician is faced with evaluating a panoply of skin rashes, a subset of which may be induced by food allergy. Acute urticaria is a c … with atopic dermatitis is more difficult and may require additional diagnostic maneuvers, including elimination diets and oral food challenges. The immunopathologic basis of food-allergic disorders that affect the skin and a rational approach to diagnosis and treatment are discussed. Diet and arthritis.

Choice of diet is one way in which an individual can influence his/her own health, and it is to be expected that patients will seek their ph … arthritis (n-3 fatty acids). Food hypersensitivity appears to be a rare cause of polyarthritis, and elimination diets and fasting have little or no place in routine practice. Strategies under investigation include oral tolerization, the utility of which remains to be established.

[Vegetarian diets in the nutrition of pregnant and breastfeeding women].

Pregnant and breastfeeding women who eat vegetarian are a source of much controversy. This is the result of concern that eliminating some or all animal produce may lead to nutritional deficiencies and thus adversely affect the mother's and child's health. The American Dietetic Association's position is that appropriately planned vegan, lacto-vegetarian and lacto-ovo-vegetarian diets ensure a normal course of pregnancy and lactation. However, in practice the balancing of such a diet can pose certain difficulties, especially for individuals without the necessary experience or knowledge about nut

Toxic effects, metabolism, and carry-over of ergot alkaloids in laying hens, with a special focus on changes of the alkaloid isomeric ratio in feed caused by hydrothermal treatment.

Ergot alkaloids (EA) are mycotoxins formed by Claviceps purpurea. Due to the large variation in EA content, the mass proportion of ergot (hardened sclerotia) in animal diets is not suited to establish safe levels of EA. Therefore, the aim of the present study was to examine the dose-dependent effects of dietary EA on laying hens. Ergoty rye or ergot-free rye (control diet) was included in the diets either untreated or after hydrothermal treatment ("expansion"). The total EA levels in five different diets containing 0-3% of untreated or expanded rye were 0.1-14.56 mg/kg (untreated rye) and 0.08

Pediatric eosinophilic esophagitis: a review for the clinician.

Eosinophilic esophagitis (EoE) is a chronic clinical-pathologic disease characterized by eosinophilic infiltration of the esophageal epithelium with esophageal dysfunction symptoms.EoE can occur at any age and has different clinical manifestations depending on the age onset. To date, esophago-gastroduodenal endoscopy (EGD) with biopsy is the gold-standard for EoE diagnosis.According to the recent consensus guidelines, proton pump inhibitors, corticosteroids and elimination diets could be a first-line therapy option. The aim of the treatment is clinical and histological remission for preventing …

Seafood allergy: lessons from clinical symptoms, immunological mechanisms and molecular biology.

Food allergy consists of a wide range of disorders that result from adverse immune responses to dietary antigens. Manifestations of allergic response includes acute, potentially fatal anaphylactic reactions and a variety of chronic diseases that mainly affect the gastrointestinal tract, skin, and respiratory tract. Tools for clinical diagnosis and management, which have not changed much in the past two decades, include the clinical history, tests for specific IgE antibody to suspected foods, elimination diets, oral food challenges, and provision of medications such as epinephrine for emergency…

Coeliac disease: histopathological findings in the small intestinal mucosa studies by a peroral biopsy technique.

Villous atrophy, changes in the surface epithelium, mucosal thickening, and glandular hypertrophy were a feature of all the mucosal biopsies from the small intestine obtained from eight coeliac children. No histological differences were observed between the children previously treated with intermittent gluten-free diets and the untreated children. Serial biopsy studies were carried out on one coeliac child before and after treatment with a gluten-free diet over a period of two years. On the whole they confirmed the irreversible nature of the observed histopathological changes but minor improve

Net K+ secretion in the thick ascending limb of mice on a low-Na, high-K diet.

Because of its cardio-protective effects, a low-Na, high-K diet (LNaHK) is often warranted in conjunction with diuretics to treat hypertensive patients. However, it is necessary to understand the renal handling of such diets in order to choose the best diuretic. Wild-type (WT) or Renal Outer Medullary K channel (ROMK) knockout mice (KO) were given a regular (CTRL), LNaHK, or high-K diet (HK) for 4-7 days. On LNaHK, mice treated with either IP furosemide for 12 hrs, or given furosemide in drinking water for 7 days, exhibited decreased K clearance. We used free-flow micropuncture to measure the

Single-center experience of the Crohn's disease exclusion diet in the United States: A retrospective study.

BACKGROUND: The Crohn's Disease Exclusion Diet (CDED) offers a dietary means of inducing and maintaining remission for patients with Crohn's disease (CD). Descriptions of efficacy have emerged primarily from Israel, Europe, and Canada. Here, we offer real-world experience, from a tertiary care center in the United States. METHODS: We conducted a retrospective chart review of patients receiving education on the CDED and those adopting the diet. Demographics, medication exposure, disease-related complications, and reported side effects were collected. Corticosteroid-free clinical remission (SF-

Nutrition care and dietetic resources in pediatric inflammatory bowel disease: An initiative of the European Society for Paediatric Gastroenterology, Hepatology and Nutrition Porto Group and Special Interest Group.

OBJECTIVES: Dietary therapy is often used in the management of children with inflammatory bowel diseases (IBD). Nevertheless, appropriate dietetic support and resources are not universally available, potentially leading to variable practices. This survey aimed to describe contemporary dietary practices and resourcing across IBD member centers of European Society of Paediatric Gastroenterology Hepatology & Nutrition (ESPGHAN). METHODS: An anonymous 33-item questionnaire survey on nutritional care and dietetic resources was circulated to the Porto Group and IBD Interest Group of ESPGHAN in Nove

Treatment patterns of eosinophilic esophagitis in the biologic era-a real-world analysis.

Eosinophilic esophagitis is a chronic immune-mediated disease requiring a long-term maintenance treatment. The data on therapy patterns and treatment durability in the biologic era are scarce in a real-world setting. The aim of this project was to provide information on the treatment courses in a real-world setting over past 10 years. Data on demographics, concomitant atopic diseases and treatments were collected retrospectively at seven Austrian centres. The eligible subjects had to start their first therapy between January 2014 and February 2025. In total, 336 patients (30.1% female) were an

Automated image analysis in the study of lymphocyte subpopulation in eosinophilic oesophagitis.

BACKGROUND: Eosinophilic oesophagitis (EoE) is characterized by the presence of eosinophils in oesophageal mucosa. Other inflammatory cells, mainly lymphocytes, dendritic cells, and mast cells may also play an important role in this disease. The aim of this study is to compare the inflammatory pattern of the mucosa between EoE and gastro-oesophageal reflux disease (GERD), using automatic image analysis in digital slides, and to assess treatment response after elimination diet and food challenge test. METHODS: From 2010 to 2013, 35 oesophageal biopsies from EoE and GERD patients were randomly

Dietary therapies for adult and pediatric inflammatory bowel disease.

Diet is an environmental exposure implicated in the development of inflammatory bowel disease (IBD), including Crohn's disease (CD) and ulcerative colitis (UC). Dietary therapy is also a tool for management of these conditions. Nutrition therapy for IBD has been shown to reduce intestinal inflammation, promote healing, and alleviate symptoms, as well as improve patients' nutrition status. Although the mechanisms of action of most nutrition therapies for IBD are not well understood, the diets are theorized to eliminate triggers for gut dysbiosis and mucosal immune dysfunction associated with th

Fermentable oligosaccharides, disaccharides, monosaccharides and polyols (FODMAPs) and nonallergic food intolerance: FODMAPs or food chemicals?

Food intolerance in irritable bowel syndrome (IBS) is increasingly being recognized, with patients convinced that diet plays a role in symptom induction. Evidence is building to implicate fermentable oligosaccharides, disaccharides, monosaccharides and polyols (FODMAPs) in the onset of abdominal pain, bloating, wind and altered bowel habit through their fermentation and osmotic effects. Hypersensitivity to normal levels of luminal distension is known to occur in patients with IBS, with consideration of food chemical intolerance likely to answer many questions about this physiological process.

[Treatment with neurostimulation and ketogenic diet for people with medically refractory epilepsy].

For some people with medically refractory epilepsy adjunct treatment with neurostimulation and ketogenic diet can be an option. Vagal nerve stimulation, VNS, consists of chronic intermittent electrical stimulation of the vagus nerve. VNS is effective and well tolerated in adolescents and adults. There is insufficient evidence on the efficacy of deep brain stimulation, DBS, and brain-responsive neurostimulation, RNS. Ketogenic diet is an established treatment in epilepsy with an increased number of indications, especially in neurometabolic diseases, where dietary therapy can completely eliminat

Serum ferritin levels in celiac disease.

Of the various common assayed parameters of iron metabolism, serum ferritin levels are the most discriminatory in distinguishing between non-treated celiac disease and other gastrointestinal disorders in the pediatric age group. Patients on normal diets usually have very low ferritin levels that increase at an average rate of 1 microgram/1/month when placed on a gluten-free diet. When the patient returns to a normal diet, however, ferritin levels decrease rapidly at an average rate of about 4 microgram/1/month. There is a relationship between abnormal intestinal changes and low ferritin levels

Dietary Management of Eosinophilic Esophagitis in the Era of Molecular Diagnostics: The Role and Limitations of Component-Resolved Diagnostics-A Narrative Review.

Eosinophilic esophagitis (EoE) is a chronic, immune-mediated disorder characterized by eosinophilic infiltration of the esophageal epithelium, primarily driven by exposure to food and aeroallergens. Although dietary elimination remains the cornerstone of therapy, the identification of specific food triggers still largely relies on empiric methods. This narrative review explores the diagnostic and therapeutic role of component-resolved diagnostics (CRD) and other molecular tools in the personalized management of EoE. Across observational and cohort studies, CRD has shown improved sensitivity in
