[Impact of the Lubrizol Rouen plant fire of September 26, 2019 on ophthalmic emergency room visits].

INTRODUCTION: A fire at the Lubrizol chemical factory in Rouen on September 26, 2019 generated a huge column of smoke directed northeast toward the city. As the eye might be particularly affected by the smoke and other toxic emissions from the fire, we assessed the impact of this industrial and ecological disaster on irritative eye surface disease in the week following the accident. MATERIALS AND METHODS: We retrospectively collected the medical data of the patients who presented to the Ophthalmology Emergency Department (OED) of Rouen University Hospital (the only OED open during the days fo

[Urologic Emergencies: Paraphimosis].

Urologic Emergencies: Paraphimosis Abstract. Paraphimosis presents a rare but acute urological emergency whereby the foreskin becomes entrapped behind the coronary sulcus of the penis. Therapy is quick and feasible, even in an outpatient setting. In most cases compression of the preputial edema and subsequent reposition of the prepuce is sufficient. Rarely, surgical intervention in form of a dorsal incision of the constriction is required. With partial or full phimosis being the underlying condition, paraphimosis occurs predominantly in infants and toddlers. However, persistent or secondary ph

Hypocalcaemic laryngospasm in the emergency department.

A 51-year-old African male presented to the emergency department with a sore throat, a productive cough and a striking pinched voice. Investigation revealed severe hypocalcaemia (0.97 mmol/l) with a prolonged QT interval on ECG. The authors administered intravenous calcium in the emergency department, which instantly improved the quality of the patient's voice. He was eventually diagnosed with primary hypoparathyroidism and an upper airway infection. Severe hypocalcaemia is a life-threatening condition. Identifying laryngospasm as an atypical presentation in the presence of a concomitant airwa

Sustaining Emergency Medicine: Workforce Wellbeing, Moral Integrity and the Power of Action.

Sustainability in emergency medicine is often framed in environmental terms, yet workforce wellbeing is equally central to the specialty's long-term viability. Across Australasia, EDs are experiencing sustained access block, rising demand and staffing pressures, contributing to high rates of burnout, moral injury and workforce attrition. These phenomena are not individual failings but signals of systemic strain. This article reframes ED workforce wellbeing and retention as core sustainability issues. It argues that clinician involvement in organisational redesign, high-value care initiatives a

Perspectives on the Phoenix Sepsis Criteria for the Emergency Medicine Practitioner.

BACKGROUND: Sepsis is one of the leading causes of death among children worldwide. The Society of Critical Care Medicine Pediatric Sepsis Definition Task Force developed and validated the Phoenix sepsis criteria, new clinical criteria for pediatric sepsis and septic shock based on organ dysfunction. Although this represents a significant advancement in standardizing sepsis and septic shock definitions, its applicability in emergency department (ED) settings remains unexplored. IMPORTANCE: Given that EDs are often the first point of contact for critically ill children, we must examine the use

Heatstroke: The Nurse's Pivotal Role in the Emergency Department.

With record-setting temperature increases, emergency nurses must rapidly recognize and manage life-threatening heat illnesses such as heatstroke. Reaching the core temperature cooling target of 39 °C (102.2 °F) within 30 minutes of heatstroke recognition is critical and directly affects morbidity and mortality. Sequelae of heatstroke may include neurologic and vital organ damage, particularly in cases of delayed cooling. This article summarizes evidence-based strategies and nursing considerations for heatstroke, including recommendations for total body immersion without the need for specialize

Emergency physician-performed ultrasound-guided nerve blocks: A scoping review of the published literature.

OBJECTIVE: Ultrasound-guided nerve blocks (UGNBs) continue to grow within the practice of emergency medicine. The purpose of this scoping review was to summarize the existing literature reporting UGNBs performed by emergency physicians (EPs) to identify trends in the literature and directions for future research. METHODS: Utilizing PRISMA-ScR guidelines, we electronically searched the PubMed, Scopus, Embase, and Cochrane libraries from database inception to May 12, 2025, to identify clinical studies reporting on outcomes of EP-performed UGNBs. Records were screened in duplicate, and data was

Mimics of Monkeypox: Considerations for the emergency medicine clinician.

INTRODUCTION: Mpox, formerly known as monkeypox, is a public health emergency most commonly presenting with a painful rash and several systemic findings. However, there are several conditions that may mimic its presentation. OBJECTIVE: This narrative review provides a focused overview of mpox mimics for emergency clinicians. DISCUSSION: Mpox is a global health emergency. The disease is primarily spread through contact, followed by the development of a centrifugally-spread rash that evolves from macules to papules to vesicles to pustules. This is often associated with lymphadenopathy and feve

Medical Screening of Mental Health Patients in the Emergency Department: A Systematic Review.

BACKGROUND: Patients presenting to the emergency department (ED) with psychiatric complaints often require medical screening to evaluate for a medical cause of their symptoms. OBJECTIVE: We sought to evaluate the existing literature on the medical screening of psychiatric patients and establish recommendations for ideal screening practices in Western-style EDs. METHODS: PubMed, PsycINFO, and ClinicalTrials.gov were searched for clinical studies examining the medical screening of adult psychiatric patients in the ED or inappropriate referrals to psychiatry. Articles were graded using the Effe

[A prompt diagnosis of Steinert's dystrophy in emergency unit].

Atrial fibrillation is frequently observed in Fist Aid. A rare cause is myotonic dystrophy There are two forms: Steinert's dystrophy caused by a defect of a gene myotoninaprotein kinase and Promm secondary to a defect of a Zinc Finger Protein Gene Clinical manifestations are localized in skeletal and face muscles, vitreous body, sexual glands, endocrine system, smooth muscle, central nervous system and myocardium. Sometimes, in mild and unrecognized forms of this rare disease there are arrhythmias as atrial fibrillation. We report the clinical case of a 52 year-old man, with a suspect diagnosi

Pleural disease in the emergency department.

Emergency department presentations of pleural-based diseases are common, with severity ranging from mild to life threatening. The acute assessment, diagnosis, and treatment of pleural disease are critical as urgent invasive maneuvers such as thoracocentesis and thoracostomy may be indicated. The emergency physician must have a systematic approach to these conditions that allows for rapid recognition, diagnosis, and definitive management. This article focuses on nontraumatic pleural disease, including diagnostic and treatment considerations of pleural effusion, empyema, primary spontaneous pneu

Management of atrial fibrillation in the emergency department and following acute myocardial infarction.

Atrial fibrillation (AF) is the most common arrhythmia managed by emergency physicians and there is increasing evidence that selected patients with acute AF can be safely managed in the emergency department without the need for hospital admission. The principles of management are identification and treatment of precipitating or underlying causes, hemodynamic stabilization/rate control, reduction of thromboembolism risk and the conversion/maintenance of sinus rhythm. A strategy of rate or rhythm control should be chosen based on the patient's clinical status, the duration of AF, the experience

Hyperacute vestibular syndrome: the role of an acute vertigo service.

Differentiating between peripheral and central causes of acute vertigo remains a clinical obstacle in the acute setting. Despite the availability of several validated diagnostic algorithms adoption and implementation of these tools is low because most emergency physicians are unfamiliar with them. Embedding an acute vertigo service within the emergency setting may help improve the diagnostic workup of patients presenting with this specific symptomatology and may have significant economic benefits, such as the avoidance of hospital admissions, reduction in unnecessary investigations, and decrea
