Low-grade adenocarcinoma of endolymphatic sac mimicking jugular paraganglioma at clinical and neuroradiological examination.

We report a case of low-grade adenocarcinoma of endolymphatic sac origin mimicking jugular paraganglioma at clinical and neuroradiological examination. The lesion occurred in a 72-year-old male who presented with a long-standing history of right-sided hearing loss and a few-week history of progressive facial nerve palsy and right aural pain. At histology, the tumor was composed of pseudoglandular spaces with papillary infoldings. Lumina contained colloid-like material. The lesion was surgically removed with suboccipital approach following endoarterial embolization. This study emphasizes that l…

Surgical opening of the endolymphatic sac in Ménière's disease--our experience from 1962-80.

Ten per cent of patients referred to the authors' clinic for vertigo prove to have Ménière's disease, and 93% of these are managed conservatively, leaving less than 1% of the entire population of vertigo patients to undergo an operation for Ménière's disease. Incision of the endolymphatic sac without any shunt is the operation performed by the authors. Results are given for a total of 157 patients and also in a smaller group of 101, all having a follow-up of greater than three years. Vertigo was eliminated or diminished in 78% during the first 12 months after surgery in the former group, and i…

Simultaneous Cochlear Implantation and Labyrinthine Surgery for Meniere Disease: Endolymphatic Sac Decompression Versus Labyrinthectomy.

INTRODUCTION: Patients with medically refractory Meniére disease (MD) may present with concomitant vertigo attacks and severe-profound hearing loss (SNHL). In such cases, simultaneous labyrinthectomy/cochlear implantation (CI) is a viable treatment option. Simultaneous endolymphatic sac decompression (ESD) and CI is less well described but represents a promising alternative. This study aims to analyze outcomes after simultaneous or staged ESD/CI as compared with simultaneous labyrinthectomy/CI. MATERIALS AND METHODS: MD patients undergoing simultaneous or staged ESD/CI and simultaneous labyri…

Imaging Analysis of Patients With Meniere's Disease Treated With Endolymphatic Sac-Mastoid Shunt Surgery.

Objective: Endolymphatic sac surgery is effective in treating intractable Meniere's disease (MD), but the underlying mechanism is still unknown. Our study investigated the mechanism by which endolymphatic sac-mastoid shunt (EMS) surgery is effective in treating MD by means of imaging. Methods: The experiment included 19 patients with intractable MD who underwent 3D-fluid-attenuated inversion recovery (FLAIR) MRI with a 3-Tesla unit 6 h after intravenous administration of gadolinium, before EMS, and 2 years after the surgery. The enhanced perilymphatic space in the bilateral cochlea, vestibule,…

Meniere's Disease Patients with Distinct Sac Pathoanatomic Findings React Differently to Endolymphatic Duct Blockage.

OBJECTIVES: To explore the possible difference in response to endolymphatic duct blockage (EDB) treatment in patients with Meniere's disease (MD) with distinct pathoanatomic characteristics of the sac. METHODS: In a total of 24 patients with MD receiving EDB treatment, the dynamics of the vertigo attack, hearing, vestibular function, and endolymph hydrops (EH) before surgery and 40 months following surgery in patients with normoplastic extraosseous portion of endolymphatic sac (eES) were compared with that in patients with atrophic eES. RESULTS: A higher prevalence of complete vertigo contro…

Endolymphatic sac tumor: single-institution series of seven cases with updated review of literature.

BACKGROUND: Endolymphatic sac tumour (ELST) is a rare low-grade locally aggressive neoplasm arising from the endolymphatic duct or sac. It presents mostly with vestibulo-cochlear symptoms either sporadically or as part of von Hippel-Lindau (VHL) syndrome. Micro-neurosurgical excision remains the cornerstone of therapy with the role of radiotherapy (RT) being controversial. This is a clinico-pathological analysis of consecutive ELST patients presenting to a single-institution in India. METHODS: Neuropathology database of a tertiary-care comprehensive cancer centre was searched electronically t…

An endolymphatic sac tumor with imaging features of aneurysmal bone cysts: differential diagnostic considerations.

BACKGROUND: Endolymphatic sac tumors (ELSTs) are rare, slow-growing tumors of the petrous bone. Despite the typical localisation, their radiological diagnosis can be challenging due to the variety of other tumors potentially showing similar features. CASE REPORT: We present a 16-year-old child with progressive hearing loss, vertigo, and tinnitus who had a large petrous bone lesion showing imaging features of both ELSTs and aneurysmal bone cysts (ABCs). The patient underwent preoperative embolization of the tumor-supplying vessels and subsequently a subtotal resection. Histological examination…

Long-term results of endolymphatic sac drainage with local steroids for intractable Meniere's disease.

OBJECTIVES: Meniere's disease is a common inner ear disease characterized by vertigo, hearing loss and tinnitus. Since Meniere's disease is thought to be triggered by an immune insult to inner ear hydrops, we examined endolymphatic sac drainage with intra-endolymphatic sac application of large doses of steroids for intractable Meniere's patients and observed long-term results from 2 years to over a decade until 13 years. METHODS: Between 1998 and 2009, we enrolled and assigned 286 intractable Meniere's patients to two groups: group-I (G-I) included patients who underwent endolymphatic sac dra…

Endolymphatic sac tumor in a patient with von Hippel-Lindau disease: MR imaging findings.

A 29-year-old male patient with von Hippel-Lindau disease who had an operation for cerebellar hemangioblastoma six years ago is presented. The patient had a four-year history of a slow-growing, locally invasive vascularized lesion of the temporal bone involving the cerebellopontine angle. The mass, studied by MR imaging, was almost completely cystic in appearance. Pathological examination after removal of the mass confirmed a papillary endolymphatic sac tumor.

Endolymphatic sac tumor associated with a von Hippel-Lindau disease patient: an immunohistochemical study.

The authors report a case of endolymphatic sac tumor (ELST) associated with Von Hippel-Lindau disease (VHL). A 20-year-old female VHL patient received a resection of a cerebellar hemangioblastoma 3 years ago and she had a co-existing of left petrous tumor. The petrous tumor showed a remarkable progression in 3 years and was resected subtotally. Histologically, the resected petrous tumor showed a papillary structure containing cuboidal or columnar cells with fibrous stroma and numerous microvessels and destructed temporal bone, all of which are consistent with ELST. We studied the expression of…

Endolymphatic sac tumors. A source of morbid hearing loss in von Hippel-Lindau disease.

OBJECTIVES: Isolated reports suggest a possible association of endolymphatic sac tumors (ELSTs), which are extremely rare in the general population, with von Hippel-Lindau disease (VHL). To determine if hearing loss and ELSTs are a component of VHL, we examined prevalence, clinical presentation, and natural history of hearing loss and ELSTs in VHL. DESIGN: Brain magnetic resonance images (MRIs) from 374 patients screened for VHL were reviewed for evidence of ELSTs. The VHL patients with MRI evidence suggestive of ELSTs or a history of hearing loss, tinnitus, or vertigo underwent additional ra…

AAV8BP2 and AAV8 transduce the mammalian cochlear lateral wall and endolymphatic sac with high efficiency.

Inner ear gene therapy using adeno-associated viruses (AAVs) has been successfully applied to several mouse models of hereditary hearing loss to improve their auditory function. While most inner ear gene therapy studies have focused on the mechanosensory hair cells and supporting cells in the organ of Corti, the cochlear lateral wall and the endolymphatic sac have not garnered much attention. The cochlear lateral wall and the endolymphatic sac play critical roles in inner ear ionic and fluid homeostasis. Mutations in genes expressed in the cochlear lateral wall and the endolymphatic sac are pr…

Responses of the endolymphatic sac to perilymphatic injections and withdrawals: evidence for the presence of a one-way valve.

Although the endolymphatic sac (ES) is thought to be a primary site for endolymph volume regulation, we have limited knowledge of how it responds to volume and pressure changes. In a prior publication, we demonstrated changes of K(+), Na(+) and endolymphatic sac potential (ESP) resulting from volume injections into, and withdrawals from, scala media of the cochlea. In the present study, we compared the influence of injections into and withdrawals from scala tympani of the cochlea on the endolymphatic sac. It is assumed that similar pressure changes are induced in endolymph and perilymph of bot…

Scanning and transmission electron microscopy of the rugose portion of the human endolymphatic sac--a preliminary report.

The rugose portion of two human endolymphatic sacs--one from a Meniere's disease patient, the other from a patient who underwent removal of an eighth nerve tumor and had no other evidence of inner ear disease--has been examined by scanning and transmission electron microscopy. The only morphological finding that distinguished the normal from the Meniere's disease specimen was more folds in the epithelium of the normal sac. Two types of cells have been distinguished in the two sacs through both scanning and transmission electron microscopy: cuboidal-to-columnar cells with a convex luminal surfa…

Comparative Proteomic Analysis of Endolymphatic Sac Luminal Fluid in Patients with Meniere's Disease and Controls.

INTRODUCTION: Meniere's disease (MD) is known to be caused by the dysfunction of the endolymphatic sac (ES), but its molecular mechanism is unknown. METHODS: We performed a comparative proteomic analysis of ES luminal fluids (ELFs) from patients with MD and controls. RESULTS: We found 6 differentially expressed proteins, including 2 significantly increased proteins and 4 significantly decreased proteins, 8 proteins identified exclusively in at least 7 of the 8 ELF samples from MD patients and 3 proteins detected solely in at least 4 of the 5 ELF samples from controls. DISCUSSION: The increa…
