A case of Eagle syndrome with a marked elongation of the styloid process; investigating its relation to ankylotic spondylitis.

Eagle syndrome is characterized by the presence of an abnormally elongated styloid process and has been related to different etiologies, including enthesitis, a known feature of spondyloarthropathies. We are describing a case of marked elongation of the styloid process in a patient with pre-existing ankylosing spondylitis and associated enthesopathy.

An unusual presentation of Lyme arthritis.

A patient eventually found to have Lyme arthritis presented with a symmetrical polyarthritis clinically resembling both rheumatoid arthritis and an enthesopathy but with a negative RA latex test. A history of tick bite was obtained only on followup questioning. Her Lyme serology was positive and her response to antibiotic treatment was complete.

GRAPPA 2023 Debate: Is Psoriatic Disease Really a Primary Enthesitis That Drives Joint Synovitis? The Enthesitis Hypothesis 25 Years On.

The enthesitis hypothesis posits that enthesitis is a primary lesion and that inflammation at the enthesis initiates the musculoskeletal symptoms of psoriatic arthritis (PsA) and spondyloarthropathies (SpA). The hypothesis suggested that inflamed entheseal tissue near the synovium could trigger cytokine-mediated synovitis, that enthesis bone anchorage could explain osteitis, and that the location of entheses at the soft tissue interface could explain dactylitis. Advances in imaging techniques that allow better visualization of enthesitis lesions and the development of animal models have allowe

An evaluation of the relationship between the degree of entheseal changes and the severity of osteodegenerative processes at fibrocartilaginous entheses.

An enthesis refers to the interface at which a tendon or a ligament integrates into the periosteum. Its morphology can be influenced by intrinsic factors such as sex, age, and extrinsic factors such as levels of activity, which will in turn impact on bone remodeling and lead to morphological changes. In bioarcheology, entheseal changes have had a long tradition of being used for the reconstruction of past activities. The literature has shown that in some cases of osteoarthritis, entheseal changes are associated with osteoarthritic manifestations. This work aims to evaluate the relationship bet

Utility of Magnetic Resonance Imaging in Diagnosis and Monitoring Enthesitis in Patients with Spondyloarthritis: An OMERACT Systematic Literature Review.

OBJECTIVE: A systematic literature review was performed to document published magnetic resonance imaging (MRI) lesion definitions and scoring systems for enthesitis in spondyloarthritis (SpA). METHODS: PubMed, Embase, and Cochrane Library databases were searched for original publications involving adult patients with SpA undergoing MRI of axial/peripheral joints. Selected articles were assessed for quality using a standardized assessment tool and metric indices. RESULTS: Considering the heterogeneous design, quality, and outcome measures of studies, statistical data pooling was considered in

Rapidly progressive sacroiliitis in a patient with lymphocytic lymphoma.

Rheumatological manifestations may develop as paraneoplastic syndromes in patients with malignancy. Sacroiliitis and spondyloarthropathy have not, however, been previously associated with cancer. The case is described of a patient with a stage IV diffuse well differentiated lymphocytic lymphoma who developed concomitant sacroiliitis and enthesopathies with rapid progression following the diagnosis of malignancy.

[Radiographic aspects of the hand in diffuse idiopathic skeletal hyperostosis].

Radiographs of the hand of 53 patients with diffuse idiopathic skeletal hyperostosis (DISH) were compared with 20 acromegalic subjects and 100 matched controls. Bone qualitative changes, characteristic of DISH, include: (1) arrow-heading of the distal phalangeal tufts; (2) enthesopathy of the proximal phalanges; (3) enlarged sesamoid bones; (4) hook-like exostoses at the metacarpophalangeal heads; (5) cortical thickness of diaphyseal tubular bones. Therefore, the hand in this disease appears as a "robust" form, particularly in males, resembling acromegalic pattern, but in this disease, the han…

Is Lateral Elbow Tendinopathy an Appropriate Clinical Diagnostic Term When the Condition Is Persistent?

Lateral elbow tendinopathy (LET) seems to be the most appropriate term to use in clinical practice because previous terms such as lateral epicondylitis, tennis elbow, lateral epicondylalgia, epicondylosis, enthesopathy, Father of the Bride's Elbow, lateral elbow or extensor tendonitis, lateral elbow or extensor tendinosis, and extensor tendinopathy make reference to inappropriate aetiological, anatomical, and pathophysiological terms [...].

Post-traumatic knee pain leading to the diagnosis of spondyloarthropathy.

The authors report a patient with persistent left knee pain after traumatic injury. The initial bone scan seemed to confirm a valgus stress knee injury. The final diagnosis, however, was focal inflammation of the knee ligaments (enthesopathy) related to ankylosing spondylitis. The diagnostic procedure and the results of other imaging modalities are presented. The bone scan findings are discussed.

[Present status of Poncet's tuberculous rheumatism. A new case].

A case of noninfectious polyarthritis of over one year's duration with calcaneal enthesopathy in a patient with visceral tuberculosis is reported. This pattern, termed Poncet's disease, shares pathophysiologic mechanisms with Freund's complete adjuvant-induced arthritis. Future studies should include polymerase chain reaction studies to look for the tubercle bacillus in joint fluid or synovial biopsy specimens.

Intratendinous gouty tophus mimics patellar tendonitis in an athlete.

We describe the imaging and pathologic features of a case of intratendinous patellar gouty tophus incidentally discovered in a patient with knee pain. The possibility of intratendinous gouty tophus must be kept in mind by sports physicians, especially in the management of patellar tendinopathy in athletes. It may be associated with other injuries, such as enthesopathies or partial tendon tears.

Coexistence of ankylosing spondylitis and diffuse idiopathic skeletal hyperostosis: another report.

A middle-aged man presented with a 10-year history of intermittent low back pain. Radiological examination of the spine disclosed findings of both ankylosing spondylitis and diffuse idiopathic skeletal hyperostosis (DISH). Radiography of the hands showed the typical enthesopathy of DISH. To our knowledge this is the 2nd published case on the coexistence of the two diseases.

Ankylosing spondylitis, Reiter's syndrome, psoriatic arthritis, and arthritis of inflammatory bowel disease.

These diseases are classified together as the seronegative spondyloarthritides. They are characterized by an association with the cell surface antigen HLA-B27, sacroilitis and spondylitis, inflammatory peripheral arthritis, enthesopathy (abnormalities in ligamentous attachments), and the absence of rheumatoid factor. Extra-articular complications are common. The diseases are usually treated with nonsteroidal anti-inflammatory drugs and physical therapy.

HLA B27 and clinical features in Reiter's syndrome.

HLA B27 and other clinical findings were investigated in 18 Turkish patients with Reiter's syndrome (mean age 35.8 +/- 8.09). Male/female ratio was 2/1. All 18 patients were seronegative, 12 (66.6%) presenting with an asymmetrical oligoarticular arthritis. Radiological sacroiliitis and enthesopathy was found in 9 (50%) and 7 (45.6%) patients respectively. HLA B27 was present in 11 (61.1%) patients.

Spondyloarthropathies.

The spondyloarthropathies are disorders characterised mainly by sacroiliitis (with or without spondylitis), peripheral oligoarthritis, enthesopathy, absence of rheumatoid factor, certain extra-articular features such as iritis, and increased prevalence of HLA-B27. The diagnosis should be considered in patients, especially young people, with low back pain. A history of psoriasis, bowel dysfunction, eye problems and peripheral joint inflammation should point to the diagnosis.

Psoriatic arthritis.

Psoriatic arthritis and its associated enthesopathy appear to be another immune-mediated manifestation of the genetic predisposition to develop psoriasis. The occurrence of psoriatic arthritis in the setting of the selective immune deficiency of advanced HIV infection, especially in the association with HLA-B27, points to a role of the CD8 T cell in disease pathogenesis. The relationship of Reiter's syndrome to psoriatic arthritis is emphasized.
