Demographic and clinical data of patients with spinal epidural angiolipomas.

Spinal epidural angiolipomas are rare, benign, mesenchymal tumors. It remains unclear whether spinal epidural angiolipomas are genuinely rare or merely underreported. Herein, we assessed the demographic and clinical characteristics of patients with spinal epidural angiolipoma. We collected data from patients with spinal epidural angiolipoma from three sources. First, we retrospectively analyzed data from patients diagnosed with spinal epidural angiolipoma in our hospital between January 1, 2014, and December 31, 2023. Second, we performed a literature review of studies retrieved from PubMed. T

Chronic spontaneous epidural hematoma in the lumbar spine with cauda equina syndrome and severe vertebral scalloping mimicking a spinal tumor: a case report.

BACKGROUND: Chronic spinal epidural hematomas (SEHs) are rare clinical entities. SEH with vertebral scalloping is extremely rare, with only a few cases having been reported to date. We report a unique case of spontaneous chronic SEH in the lumbar spine with severe vertebral scalloping mimicking an epidural tumor. CASE PRESENTATION: A 71-year-old man presented with a 2-month history of lumbar pain and a 3-week history of paresthesia and pain in the right lower extremity, hypesthesia in the perineal and perianal regions, and bladder dysfunction. Computed tomography following myelography reveale

Acute pain management of rib fractures: a narrative review.

BACKGROUND: Rib fractures are common conditions often associated with significant complications, including respiratory failure, pneumonia, prolonged hospitalisation, and chronic pain. Adequate pain management is crucial to improve outcomes and reduce morbidity in these patients. Despite the wide adoption of multimodal analgesia, the optimal combination of analgesic strategies remains uncertain. OBJECTIVE: To review and synthesize current evidence on analgesic strategies for acute pain management in patients with rib fractures. METHODS: We conducted a narrative review by searching scientific

Neuroimaging of Spinal Cord and Cauda Equina Disorders.

PURPOSE OF REVIEW: This article reviews the neuroimaging of disorders of the spinal cord and cauda equina, with a focus on MRI. An anatomic approach is used; diseases of the extradural, intradural-extramedullary, and intramedullary (parenchymal) compartments are considered, and both neoplastic and non-neoplastic conditions are covered. Differentiating imaging features are highlighted. RECENT FINDINGS: Although T2-hyperintense signal abnormality of the spinal cord can have myriad etiologies, neuroimaging can provide specific diagnoses or considerably narrow the differential diagnosis in many c

Pediatric neurosonography.

Neurosonography is an excellent modality for imaging a wide array of intracranial pathology in the infant and newborn. Neurosonography is also finding increasing use in other accessible regions of the central nervous system, including the adult brain during craniotomy and the spine during laminectomy. Sonography represents the primary modality for the evaluation of the preterm brain. Because of the high incidence of pathology in gestationally immature neonates, screening sonography is required in every infant. The sonographic features of intracranial hemorrhage include areas of increased echog

Spinal epidural capillary hemangioma: A systematic literature review and an illustrative case.

BACKGROUND: Spinal hemangiomas are rare vascular malformations resulting from proliferation of vascular endothelial cells. The cavernous form is the most common and represents 5-12% of spinal vascular malformations, while the capillary form is rare. CASE DESCRIPTION: A 56-year-old patient with no past medical history presented with progressive spinal cord compression symptoms localizing to the T10 level with MRC grade 4 proximal paraparesis. Preoperative MRI demonstrated a well-delineated, dumbbell-shaped, epidural lesion, without bony involvement, resulting in spinal cord compression at the

Esthesioneuroblastoma metastatic to the thoracic intradural and extradural space.

BACKGROUND CONTEXT: Ethesioneuroblastoma (ENB) is a rare tumor of the olfactory epithelium that has been shown to metastasize mostly to the cervical lymphatics, with only infrequent spread to other locations. We report a rare case of ENB characterized by recurrence and distant metastasis to the T7-T8 intradural and extradural space. PURPOSE: To report a rare case of recurrent ENB metastatic to the thoracic intradural and extradural space. STUDY DESIGN/SETTING: Case report with a review of the literature. METHODS: A 64-year-old man with recurrent ENB presented with chronic pain in the neck,

[Study of combination therapy for thymoma: a case of stage IV which presented as total spinal block caused by epidural metastasis and which preoperative combination therapy was effective for minimizing the tumor].

We experienced 24 cases of thymoma in the past 17 years. Associated syndromes as myasthenia gravis and pure red cell anemia (PRCA) were found in 7 patients. Preoperative therapies were done for the cases of stage IV. Total resection of the tumor was performed except 3 cases which ended in exploratory thoracotomy. Postoperative therapies were done except the cases of stage I. Each one case of stage III and IV a whose operation ended in exploratory thoracotomy died of tumors, but latter case had some period of PR after chemotherapy. A case of stage I with PRCA died from pneumonia but other cases

Pediatric intracranial high-grade solitary fibrous tumor/hemangiopericytoma with drop metastasis to cervicodorsal spine: a rare case report.

BACKGROUND: Intracranial solitary fibrous tumor/hemangiopericytoma (SFT/HPC) is extremely rare in children, accounting for fewer than 10% of all SFT/HPCs and significantly fewer primary central nervous system tumors. Pediatric SFT/HPCs of high-grade are particularly aggressive, having a high frequency of recurrence and late metastasis, though spinal "drop" metastases remain, nevertheless, very rare. CASE PRESENTATION: We discuss the case of an 11-year-old girl patient with a 2-year history of recurrent generalized tonic-clonic seizures, worsening headache, vomiting, and acute-onset bilateral

[Pain in one leg in patients with cancer].

Two patients, a woman aged 65 years and a man aged 56 years, with cancer, presented with pain in one leg as the first manifestation of metastases. The woman had tumour plexopathy of the lumbosacral plexus caused by an os sacrum metastasis of a thyroid carcinoma; she received radiotherapy but died a short time later. The man had lumbosacral epidural metastases of a colon carcinoma, compressing lumbosacral roots; with radiotherapy he survived the first year. Back pain with radiating pain is a frequent symptom in patients with cancer. Spinal epidural metastases, spinal and paraspinal metastases w

Fourth ventricular neurocytoma: case report and review of the literature.

OBJECTIVES: Central neurocytoma is a tumour that typically occurs in young adults in close association with the lateral and third ventricles of the cerebrum. METHODS: We report the unusual case of a central neurocytoma that developed in the fourth ventricle of a 59-year-old woman and metastasized to the upper cervical canal. Subtotal excision and adjuvant radiotherapy were used to treat the lesion. Microscopic evaluation, discussion of the pathologic differential diagnosis and theories of the histogenesis of the tumour are presented. RESULTS AND CONCLUSIONS: Fourth ventricular neurocytoma is

Extramedullary masses as presenting features of acute monoblastic leukemia.

Acute monoblastic leukemia is an uncommon type of non-lymphocytic leukemia that generally first manifests with signs and symptoms of bone marrow failure. This study describes four patients with acute monoblastic leukemia in whom the initial manifestations of the disorder were related to extramedullary masses. Three of the four patients were children. All patients had bone marrow involvement. The extramedullary mass lesions of acute monoblastic leukemia may be difficult to distinguish from other hematopoietic neoplasms, such as granulocytic sarcoma, malignant lymphoma and malignant histiocytosi

Metastatic thymoma presenting as spontaneous epidural lumbar haematoma.

We report the case of a 44-year-old man who was found to have metastatic thymoma to his lumbar spine presenting as a spontaneous epidural haematoma. The man presented with back pain and cauda equina like symptoms in the absence of trauma, antiplatelet or anticoagulant agents. Following a laminectomy and excision of the epidural collection he made a full neurological recovery. Histopathology of the haematoma demonstrated metastatic thymoma. To the best of our knowledge, this is the first such case of metastatic thymoma to the lumbar spine presenting as a spontaneous epidural collection. We beli

[Combined chronic subdural and acute epidural hematoma secondary to metastatic hepatocellular cancer: case report].

We report a case of combined chronic subdural and acute epidural hematoma caused by metastatic hepatocellular cancer of the dura mater. A 44-year-old woman, who had been under treatment for hepatic cancer for one year, presented with right hemiparesis. MRI revealed a chronic subdural hematoma on the left fronto-parietal region with subgaleal and epidural metastasis. The patient was found comatose 7 days later. A huge epidural hematoma in the left parietal region was disclosed with CT scan. She died of hepatic failure despite the evacuation of the hematoma. This is the first reported case with

Pure epidural extraosseous cavernous hemangioma with thoracic myelopathy: case report and review of literature.

INTRODUCTION: Pure epidural spinal cavernous hemangiomas are rare, benign vascular tumors that account for approximately 4% of all spinal epidural tumors. Due to their dumbbell shape and propensity for foraminal invasion, they are often misdiagnosed and inadequately treated. We present a case of a 58-year-old male with extra-osseous cavernous hemangioma to better aid in diagnosis and management of these lesions. CASE PRESENTATION: A 58-year-old male presented with chronic lower back pain, progressive lower extremity weakness, T10 sensory level, absent lower extremity proprioception, hyperrefl
