From Preservation to Precision in Pediatric Dentistry: Evidence-Calibrated Viewpoint and Heuristic Framework for Silver Diamine Fluoride Guidance.

Silver diamine fluoride (SDF) is a key preservation-based intervention in pediatric dentistry. It can arrest many cavitated lesions, reduce treatment burden, and expand access for children who cannot receive conventional restorative care. This viewpoint article offers a reasoned, heuristic framework for calibrating SDF guidance to the strength of the underlying evidence. It does not present a systematic review or formal policy standards. Foundational trials support the clinical usefulness of 38% SDF. The 2017 AAPD guidelines provided conditional recommendations based on low-quality evidence. T

Substance-use screening and interventions in dental practices: survey of practice-based research network dentists regarding current practices, policies and barriers.

BACKGROUND: Dental visits represent an opportunity to identify and assist patients with unhealthy substance use, but little is known about how dentists are addressing patients' use of tobacco, alcohol and illicit drugs. The authors surveyed dentists to learn about the role their practices might play in providing substance-use screening and interventions. METHODS: The authors distributed a 41-item Web-based survey to all 210 dentists active in the Practitioners Engaged in Applied Research and Learning Network, a practice-based research network. The questionnaire assessed dental practices' poli

An evidenced-based scoring index to determine the periodontal prognosis on molars.

BACKGROUND: This retrospective study evaluates and assigns scores to six prognostic factors and derives a quantitative scoring index used to determine the periodontal prognosis on molar teeth. METHODS: Data were gathered on 816 molars in 102 patients with moderate-to-severe periodontitis. The six factors evaluated (age, probing depth, mobility, furcation involvement, smoking, and molar type) were assigned a numeric score based on statistical analysis. The sum of the scores for all factors was used to determine the prognosis score for each molar. Only patients with all first and second molars

Non-Operative, Micro- and Minimally Invasive Methods for Caries Treatment-A Narrative Review.

The management of dental caries has evolved from the traditional mechanical approach of "extension for prevention" to a biologically oriented philosophy centered on preserving natural tooth structures. Minimally invasive dentistry (MID) emphasizes early detection, risk assessment, prevention, and conservative intervention based on the lesion's activity and depth. This review outlines current evidence on non-operative, micro-invasive, and minimally invasive strategies, including fluoride therapy, remineralizing agents such as casein phosphopeptide-amorphous calcium phosphate (CPP-ACP), self-ass

Multimodal physiotherapeutic management of postoperative complications in orthognathic surgery: A literature review and institutional protocol proposal.

BACKGROUND: Orthognathic surgery (OS) corrects skeletal and dental discrepancies in the maxillofacial region, improving occlusion, masticatory function, respiration, and facial aesthetics. However, the immediate and intermediate postoperative periods are frequently associated with acute pain, facial edema, neurosensory disturbances (e.g., paresthesia of the inferior alveolar and/or infraorbital nerves), and restricted mandibular mobility (trismus), which may prolong recovery and impair quality of life. Physiotherapeutic rehabilitation has emerged as a key component of postoperative care, yet s

Serious mental illness and tobacco addiction: a model program to address this common but neglected issue.

Tobacco addiction among persons with serious mental illness (SMI) has been largely ignored. About 75 to 85% of persons with schizophrenia, bipolar disorder, and other SMI use tobacco; most will either die and/or have reduced quality of life because of tobacco-caused medical diseases. Tobacco addiction is the most common co-occurring disorder for the SMI population. A dramatic reduction in tobacco use in the general population has occurred during the past 40 years; however, there has been almost no reduction for smokers with SMI. The University of Medicine and Dentistry of New Jersey program ta

Traumatic Dentoalveolar and Maxillofacial Injuries in Cats: Overview of diagnosis and management.

PRACTICAL RELEVANCE: Maxillofacial and traumatic dentoalveolar injuries can cause pain and inflammation, and reduce function of the mouth, impacting a cat's quality of life. Many traumatically induced feline fractures have been reported to involve the mandible or skull and, in cats with maxillofacial trauma, traumatic dentoalveolar injuries are particularly prevalent. Traumatic dentoalveolar injuries can also often be found in otherwise healthy cats. CLINICAL CHALLENGES: Some traumatic dentoalveolar injuries require emergency treatment; timely recognition and managment is therefore important

Oleogel-based drug delivery for the treatment of periodontitis: current strategies and future perspectives.

Periodontitis is the chronic inflammation of tooth-supporting tissues that leads to loss of tooth support if untreated. Conventional therapy for periodontitis (mechanical removal of microbial biofilm and oral hygiene enforcement) is augmented by anti-microbial and anti-inflammatory drugs. These drugs are frequently delivered locally into the periodontal pocket for maximum efficiency and minimum adverse effects. The potential of oleogels for periodontal drug delivery has been discussed and further, the future scope of oleogel-based drug delivery systems in dentistry. An oleogel-based local drug
