Therapeutic potential of traditional herbal medicine against multidrug-resistant pulmonary infections: Based on clinical and pharmacodynamic evidence.

ETHNOPHARMACOLOGICAL RELEVANCE: Traditional herbal medicine has been used for centuries to treat respiratory infections, with documented historical applications for conditions characterized by persistent fever, productive cough, and lung inflammation. These traditional preparations represent indigenous medical knowledge that may offer alternative approaches to modern antibiotic-resistant pulmonary infections, particularly given the global crisis of antimicrobial resistance and diminishing treatment options. AIM OF THE STUDY: To systematically evaluate the clinical efficacy and pharmacodynamic

Clinical Outcomes with and without Adherence to Evidence-Based Medicine Guidelines for Lumbar Degenerative Spondylolisthesis Fusion Patients.

INTRODUCTION: Degenerative lumbar spondylolisthesis (DS) patients are treated with instrumented fusion, following EBM guidelines, and typically have excellent clinical outcomes. However, not all lumbar fusion procedures adhere to EBM guidelines, typically due to a lack of prospective data. OBJECTIVE: This retrospective study compared outcomes of DS lumbar fusion patients treated according to EBM guidelines (EBM concordant) to lumbar fused patients with procedures that did not have clear EBM literature that supported this treatment, the goal being to examine the value of present EBM to guide c

Evidence-Based Medicine of Traumatic Thoracolumbar Burst Fractures: A Systematic Review of Operative Management across 20 Years.

Study Design Systematic literature review. Objective The management of traumatic thoracolumbar burst fractures (TLBF) remains challenging, and analyzing the levels of evidence (LOEs) for treatment practices can reform the decision-making process. However, no review has yet evaluated the operative management of traumatic thoracolumbar burst fractures with particular attention placed on LOE from an established methodology. The objective of the present study was to characterize the literature evidence for TLBF, specifically for operative management. Methods A comprehensive search of the English l

Evidence-based medicine (EBM) in practice: agreement between observers rating esophageal varices: how to cope with chance?

In clinical medicine, several tests that are commonly performed (i.e., physical exams, x-rays, or endoscopy) rely on a degree of subjective interpretation by observers. A limitation for clinicians is the concept of agreement between two observers that is present and beyond chance. Clinical decisions based on the presence or absence of a finding in a diagnostic test (i.e., size of esophageal varices on endoscopy) are commonly reached depending on the agreement between observers. Observations that measure the agreement between two or more observers should include a formula that takes into accoun

[Lung maturation therapy with glucocorticoids in threatened premature labor. Considerations of risk-benefit in evidence-based medicine].

Prematurity is a major cause of perinatal morbidity and mortality. Antenatal administration of glucocorticoids improves the neonatal outcome of preterm born infants. 1994 the NIH published recommendations for the use of glucocorticoids for women at risk of preterm delivery. A recent evaluation by the Cochrane Collaboration in 1999 showed that antenatal administration of glucocorticoids significantly reduced the rate of RDS and IVH in the gestational age between 24 and 34 weeks. Consequences of repeated courses of antenatal glucocorticoids are not sufficiently studied. The effectivity and safet

A Comparative Study of Artificial Intelligence and Faculty Rubric-Based Grading of Pharmacy Student Writing Assignments in an Evidence-Based Medicine Course.

BACKGROUND: Generative artificial intelligence (AI) tools are increasingly used in health professional education, including for automated writing evaluation (AWE). While AI-based grading may reduce faculty workload and variability, its alignment with human grading remains unclear. This study compared rubric-based grading of pharmacy student drug information papers by faculty and ChatGPT (Open AI, San Francisco, CA). METHODS: We conducted a retrospective comparative analysis of 159 de-identified assignments from a required evidence-based medicine course. Faculty-assigned grades were paired wit…

Update of the European Society of Anaesthesiology and Intensive Care Medicine evidence-based and consensus-based guideline on postoperative delirium in adult patients.

Postoperative delirium (POD) remains a common, dangerous and resource-consuming adverse event but is often preventable. The whole peri-operative team can play a key role in its management. This update to the 2017 ESAIC Guideline on the prevention of POD is evidence-based and consensus-based and considers the literature between 01 April 2015, and 28 February 2022. The search terms of the broad literature search were identical to those used in the first version of the guideline published in 2017. POD was defined in accordance with the DSM-5 criteria. POD had to be measured with a validated POD s

[Selection of timing for endoscopic treatment of gastroesophageal reflux disease: a discussion from an evidence-based medicine perspective].

Gastroesophageal reflux disease (GERD) is a common gastrointestinal disorder, with increasing prevalence due to obesity and lifestyle changes. Although proton pump inhibitors (PPIs) remain the first-line therapy, a proportion of patients have an unsatisfactory response, require long-term medication, or experience symptom relapse after discontinuation. Positioned between pharmacotherapy and surgery as a third therapeutic option, endoscopic therapy offers an additional choice for patients with refractory GERD. Based on current evidence, this article examines the optimal timing of endoscopic inte

Integrated evidence chain-based effectiveness evaluation of traditional Chinese medicines (Eff-iEC): A demonstration study.

Addressing the enduring challenge of evaluating traditional Chinese medicines (TCMs), the integrated evidence chain-based effectiveness evaluation of TCMs (Eff-iEC) has emerged. This paper explored its capacity through a demonstration study that evaluated the effectiveness evidence of six commonly used anti-hepatic fibrosis Chinese patent medicines (CPMs), including Biejiajian Pill (BP), Dahuang Zhechong Pill (DZP), Biejia Ruangan Compound (BRC), Fuzheng Huayu Capsule (FHC), Anluo Huaxian Pill (AHP), and Heluo Shugan Capsule (HSC), using both Eff-iEC and the Grading of Recommendations, Assessm

Lessons learnt in evidence-based perioperative pain medicine: changing the focus from the medication and procedure to the patient.

Over time, the focus of evidence-based acute pain medicine has shifted, from a focus on drugs and interventions (characterized by numbers needed to treat), to an appreciation of procedure-specific factors (characterized by guidelines and meta-analyses), and now anesthesiologists face the challenge to integrate our current approach with the concept of precision medicine. Psychometric and biopsychosocial markers can potentially guide clinicians on who may need more aggressive perioperative pain management, or who would respond particularly well to a given analgesic intervention. The challenge wi
