Solitary ulcer of the rectum is an unusual condition occurring more often in young adults. Unfortunately the term is rather confusing, since the lesion may not necessarily be solitary, nor be confined to the rectum; moreover, it may be polypoid rather than ulcerating. The etiology is uncertain, but chronic constipation and fecal impaction could play a role. Today most authors believe solitary ulcer syndrome is a distinct clinical inflammatory manifestation associated with rectal prolapse. Surgical treatment of the prolapse is usually followed by a recovery of the solitary rectal ulcer.

Clostridium perfringens infections are often associated with high mortality and typically result from soft tissue infections. Stercoral colitis, a complication of fecal infection, is more frequently linked to gram-negative bacteremia. We present a rare case of C. perfringens bacteremia caused by stercoral colitis. Seed bezoars are a subcategory of phytobezoars caused by undigested vegetable seeds or fruit pits. We report the case of a 48-year-old woman who was admitted due to severe constipation and rectal pain. Rectal examination was notable for numerous sunflower seeds at the anal verge. Initial CT imaging revealed fecal impaction with a rectal bezoar which was managed with manual evacuation. A 72-year-old women was referred to our hospital because of lower left abdominal pain. Computed tomography showed prominent sigmoid colon dilation and double tumors on both the oral and anal sides. Surgical resection revealed an expanded sigmoid colon involved in double cancer that showed strong adhesion to the surrounding tissues. The pathological findings revealed obstructive colitis and minor perforation in the dilated colon. The minor perforation was considered to have been caused by fecal impaction in the closed cavity between the two tumors, resulting in an increase in colon pressure.

Surgery for complete rectal prolapse was performed in 17 women and 3 men between November 1986 and April 1991. An abdominal approach with posterior rectopexy was used in 17. The Thiersch procedure of anal narrowing was performed in 3 high risk patients. All recovered without major complications. Urinary tract infection developed in 15% and postoperative fecal impaction in 10%. Anal continence improved in 70% of those previously incontinent. There was no recurrence of complete rectal prolapse during follow-up. All patients rated the operative results as either good or very good.

A number of changes in thyroid physiology occur with normal aging, including a modest fall in T3 levels and a reduction in the rate of turnover of thyroid hormones. Additionally, the diagnosis of either hyperthyroidism or hypothyroidism can be more difficult in the elderly because the symptoms may be more subtle. Hyperthyroidism may be disguised by a very apathetic presentation, without many of the typical adrenergic symptoms such as tachycardia and nervousness. Some of the many ways in which hypothyroidism may present include occult congestive heart failure or a severe fecal impaction.

Esophagitis is the first cause of dysphagia and food impaction in a young male adult. Eosinophilic esophagitis is a clinical and pathological entity with involvement limited to the oesophagus. Eosinophilic esophagitis is associated with allergy and atopy. Most common in Caucasians, with 3:1 male predominance. Endoscopic findings are unspecific. Stercoraceous perforation of the sigmoid colon has rarely been reported in the literature. This lesion is assumed to be produced by the pressure from a hard scybalum resulting in a perforated ulcer with necrotic edges. We treated a patient with a complete invagination of the appendix which contained one large laminated calcified fecalithy. Colonofiberscopy showed a dimpling submucosal tumor, which was palpated as a bony hard tumor at laparotomy. This finding suggested that the fecalith caused an intussusception of the appendix and that such an intussusception should be suspected when there is a bony hard dimpling submucosal tumor in the cecum.
