A double-blind clinical trial in weight control. Use of fenfluramine and phentermine alone and in combination.

We performed a double-blind, controlled clinical trial comparing phentermine resin (30 mg in the morning), fenfluramine hydrochloride (20 mg three times a day), and a combination of phentermine resin (15 mg in the morning) and fenfluramine hydrochloride (30 mg before the evening meal), and placebo. We combined low doses of the two drugs to maintain efficacy while diminishing adverse effects. Eighty-one people with simple obesity (130% to 180% of ideal body weight) participated. Individualized diets were prescribed and discussed again during the 24-week study period. Weight loss in those receiv…

Fluphenazine, ICS 205-930 and dl-fenfluramine differentially antagonise drug-induced emesis in the ferret.

The intravenous injection of cisplatin (10 mg/kg), the subcutaneous injection of apomorphine (0.125-1 mg/kg) and lisuride (0.001-0.1 mg/kg), the oral administration of ipecacuanha (0.3-2.4 mg/kg) and the intragastric administration of copper sulphate (25-100 mg/kg), induced a vomiting and retching response in the ferret. Pretreatment with dl-fenfluramine (5 mg/kg i.p.) prevented or reduced the emesis induced by cisplatin, apomorphine, ipecacuanha and lisuride but failed to significantly antagonise copper sulphate-induced emesis. The 5-HT3 receptor antagonist ICS 205-930 (0.1 mg/kg i.p.) preven…

Fenfluramine hydrochloride for the treatment of seizures in Dravet syndrome: a randomised, double-blind, placebo-controlled trial.

BACKGROUND: Dravet syndrome is a rare, treatment-resistant developmental epileptic encephalopathy characterised by multiple types of frequent, disabling seizures. Fenfluramine has been reported to have antiseizure activity in observational studies of photosensitive epilepsy and Dravet syndrome. The aim of the present study was to assess the efficacy and safety of fenfluramine in patients with Dravet syndrome. METHODS: In this randomised, double-blind, placebo-controlled clinical trial, we enrolled children and young adults with Dravet syndrome. After a 6-week observation period to establish b…

Interaction between the serotonergic, dopaminergic, and glutamatergic systems in fenfluramine-induced Fos expression in striatal neurons.

Fenfluramine (FE) is a halogenated amphetamine derivative used in the treatment of obesity and thought to induce serotonin (5-HT) release from nerve terminals and to reduce re-uptake. However, other pathways may also be involved. In this work, the effects of FE on the major striatal afferent systems, and the possible interactions of these systems in FE-induced striatal expression of Fos, were studied by lesion of the serotonergic and/or dopaminergic system and administration of NMDA glutamate (MK-801) or D1 dopamine (SCH-23390) receptor antagonists. Both the D1 and NMDA receptor antagonists su…

Use of fenfluramine in MECP2-related Rett syndrome: Findings from a retrospective multicenter pediatric case series.

INTRODUCTION: Rett syndrome (RTT) is a severe neurodevelopmental disorder mainly affecting females. The patients could experience many comorbidities, including gastrointestinal, breathing, cardiovascular disorders, and seizures, which are often drug-resistant. Many antiseizure medications (ASMs) can be utilized as monotherapy or in combination. Fenfluramine (FFA), has a unique mechanism of action that targets the serotonergic system and sigma-1 receptors, has shown benefit in other epileptic encephalopathies, but data on RTT are lacking. METHODS: We retrospectively reviewed the charts of four…

Does central serotonergic function correlate inversely with aggression? A study using D-fenfluramine in healthy subjects.

Much research has investigated possible links between serotonergic function and aggression in violent or personality disordered populations, but few studies have looked at healthy subjects. In this study we administered 30 mg of the specific 5-HT releasing agent D-fenfluramine to 35 healthy subjects, along with questionnaire measures of hostility and aggression. Prolactin and cortisol responses were used as indices of central 5-HT function. In males, there were significant inverse correlations between 5-HT mediated cortisol responses and both the Buss-Durkee Hostility Inventory total score and…

Real-World safety profile of Fenfluramine in refractory epilepsy: a pharmacovigilance study based on FAERS database.

OBJECTIVE: The safety profile of Fenfluramine, a drug for refractory epilepsy, remains insufficiently characterized in real-world settings. This study aimed to evaluate post-marketing adverse events (AEs) associated with Fenfluramine using the FDA Adverse Event Reporting System (FAERS) database. METHODS: Our investigation collected all AE reports related to Fenfluramine from the third quarter of 2020 (Q3 2020) to the fourth quarter of 2024 (Q4 2024). We employed four established methodologies including Reporting Odds Ratio (ROR), Proportional Reporting Ratio (PRR), Bayesian Confidence Propaga…

Pharmacovigilance study on old drugs repurposed for rare diseases across different indications: the case of fenfluramine.

OBJECTIVE: As an old drug with a new application in rare diseases with epileptic symptoms, fenfluramine may have potential unrecognized adverse events. Because limited real-world data exist on Lennox-Gastaut syndrome (LGS) and Dravet syndrome (DS) populations, some rare adverse events (AEs) are easily overlooked. The purpose of this study was to comprehensively evaluate the characteristics of adverse events of fenfluramine. METHODS: The data were extracted from the Food and Drug Administration (FDA) Adverse Event Reporting System (FAERS) database from the third quarter of 2020 to the fourth q…

Effect of single doses of pindolol and d-fenfluramine on flumazenil-induced anxiety in panic disorder patients.

The effects of the 5-HT1A receptor blocker pindolol and the 5-HT releasing and uptake blocking agent d-fenfluramine, both used as indirect serotonin agonists, on flumazenil-induced acute anxiety reactions were studied in panic disorder patients to test the hypothesis that serotonin (5-HT) inhibits neural systems mediating panic attacks. Thirty never treated or drug free PD patients (16 females) aged 22-49 y (mean ± SD, 32.9 ± 8) received single doses of d-fenfluramine (n = 10; 30 mg, p.o.), pindolol (n = 10; 5 mg, p.o.), or placebo (n = 10) 90 and 45 min before a challenge test with flumazenil…

Autopsy findings of heart and lungs in a patient with primary pulmonary hypertension associated with use of fenfluramine and phentermine.

A 36-year-old woman (height, 157 cm; weight, 117 kg; body mass index, 47.5) received fenfluramine and phentermine (fen-phen) for 7 months, and pulmonary hypertension subsequently developed. Her pulmonary arterial pressure was 56 mm Hg, and echocardiography showed right ventricular dilatation and hypokinesia. Cardiopulmonary arrest developed during right-heart catheterization, and she died 3 days later. At autopsy, right ventricular dilatation with fibroproliferative tricuspid valve was identified. The pulmonary arteries, including the main arteries and elastic arteries to the arterioles, revea…

Complex vascular lesions at autopsy in a patient with phentermine-fenfluramine use and rapidly progressing pulmonary hypertension.

Anorectic agents, such as aminorex and fenfluramine derivatives, have been associated previously with the development of primary pulmonary hypertension. The combination diet drug phentermine-fenfluramine (or "phen-fen") was recently marketed in the United States. We describe a case of a 39-year-old woman with rapidly progressing, fatal pulmonary hypertension following administration of phentermine-fenfluramine. Autopsy was remarkable for complex pulmonary vascular lesions most consistent with thrombotic arteriopathy.

Evaluation of central serotonergic function in affective and related disorders by the fenfluramine challenge test: a critical review.

Plasma prolactin levels following oral administration of the serotonin (5-HT) releasing agent, fenfluramine hydrochloride, have been extensively used to evaluate central serotonergic function in affective and related disorders. Cortisol responses to fenfluramine have generally been a less informative measure. In healthy subjects, prolactin release by fenfluramine is dose-dependent, blocked by antagonists of serotonin receptors of the 5-HT-2a/2c type, negatively correlated with age and increased in young females. In major depression, a preponderance of studies have found blunted prolactin respo…
