Use of Fibula Nails With Proximal and Distal Fixation.

BACKGROUND: Ankle fractures are a frequent injury in the adult population and a quarter of all ankle fractures are classified as unstable, requiring surgical intervention. Plate-and-screw construct is the traditionally used fixation method for fibula fractures. The use of an intramedullary nail is an alternative fixation method, with current literature supporting very low complication rates and hardware removal surgeries. The purpose of this study was to evaluate the outcomes, including complication rates and implant removal rates, using a fibula nail with both proximal and distal fixation cap

An irreducible distal fibula dislocation while in-line skating.

We present a case report of a variation of a Bosworth fracture, which is a posterior dislocation of the proximal fragment of a distal fibula fracture. Our patient had a distal fibula dislocation without fracture. He was treated with an open reduction and internal fixation using syndesmotic screws. At his most recent follow-up, he was ambulating without pain.

Fibular fracture stabilization with a guidewire as supplementary fixation in tibia fractures.

We present a novel technique of intramedullary fixation of the fibula using a humeral guide wire as an adjunct to tibial fixation, in the setting of tibial shaft fracture. Not only does this technique aid in determining length, alignment, and rotation of the tibial fracture, but it may also help the support of the lower extremity as whole by stabilizing the lateral column. In addition, this technique can be used to help maintain reduction of the fibula when there is concern for the soft tissues of the lower extremity secondary to swelling or injury. Our clinical case series demonstrates this s

The antiglide plate for distal fibular fixation. A biomechanical comparison with fixation with a lateral plate.

Posterior antiglide plates recently have been introduced as a method of fixation for the short oblique fracture (Type B, as defined by the AO Group) of the distal part of the fibula. This method has several advantages over the more commonly used lateral plate for the fixation of this type of fracture, including dissection of a smaller area and less operative time, minimum bending of the plate, and no potential for penetration of a screw into the joint. The system can also be applied without insertion of a screw into the distal fragment. This prompted us to compare the biomechanical properties

Maisonneuve ankle fracture.

Maisonneuve fracture (MF) is defined as an ankle fracture-dislocation associated always with a fracture of the proximal quarter of the fibula and rupture of the anterior and interosseous tibiofibular ligaments. Other injuries are variable. Recent CT studies have demonstrated that MF is a far more complex injury than initially supposed. Therefore it is necessary to change substantially the current concepts related to this issue. MF is combined in about 80% of cases with a fracture of the posterior malleolus and also with malposition of the distal fibula in the fibular notch. An exact assessment

Dislocation of the proximal tibiofibular joint in association with a tibial shaft fracture: two case reports and a literature review.

Dislocation of the proximal tibiofibular joint (PTFJ) in association with a displaced tibial shaft fracture and an intact fibula is an exceedingly rare injury. We present 2 cases of tibia fractures associated with an intact fibula and a PTFJ dislocation. The first case involves a man who sustained a closed spiral fracture of the distal tibial shaft, with an intact fibula, an anterolaterally dislocated PTFJ, and a partial tear of the lateral collateral ligament. The tibia was percutaneously plated, and the PTFJ was reduced and then stabilized with temporary screw fixation. The second case invol

Proximal tibiofibular joint dislocation associated with tibial shaft fractures - 7 Cases.

OBJECTIVE: Lower leg fractures of the tibia with or without fracture of the fibula are very common. Proximal tibiofibular joint (PTFJ) dislocation is a very rare injury that can occur together with a tibia shaft fracture. As there is only scarce literature about this injury available, we would like to present our experience with the treatment of this entity. METHODS: We present a small case series of seven patients. In most cases, the tibia fracture was nailed in a closed technique. After distal locking the proximal fibula was exposed by a lateral approach exposing and preserving the peroneal

Assessment of Bosworth-type fracture by external oblique radiographs.

INTRODUCTION: Bosworth described an unusual fracture dislocation of the ankle with fixed posterior fracture dislocation of the fibula. This ankle fracture variant is often not recognized in initial radiographs and requires a computed tomographic scan for verification. It is usually not reducible by the closed method, and repeated trials induce more damage. The purpose of this study was to verify the usefulness of simple external oblique radiographs for diagnosis of Bosworth-type fracture. METHODS: We reviewed the 327 patients who were diagnosed as unilateral malleolus ankle fracture in 2002 t

Valgus deformity after fibular resection in children.

The authors present a retrospective study of 23 patients in their growing period who underwent resection of more than 2 cm of the fibula. Long-term effects in the ankle and tibia were analyzed. The patients were radiologically studied using the contralateral side as control. Representative radiologic findings were distal migration of the fibula head in 75% (but without clinical relevance), thickening of the external tibial cortex in 20%, talar tilt in 45%, proximal migration of the lateral malleolus in 55%, and diaphyseal valgus of the tibia in 20% of the cases. Incomplete regeneration of fibu

[Injuries of the tibiofibular syndesmosis].

Injuries of syndesmotic ligaments can occur in isolation or as a component of fractures with a latent or manifest diastasis between the distal tibia and fibula. Bony avulsions of the anterior or posterior tibial rim or the anterior fibula are frequently detected. Computed tomography imaging is essential for establishing the correct diagnosis. Relevant instability of the syndesmosis results from at least two ligament ruptures or avulsion fractures. The objective of surgical treatment is the anatomical reconstruction of the tibial incisura and the correct reduction of the fibula into it. Stable

Management of an Imminent Pathological Fracture of a Fibular Neomandible Via a Minimally Invasive Approach-A Case Report.

Fibular free flap reconstruction remains the workhorse of postmandibulectomy reconstruction. Dental implantation to support a dental prosthesis is a sought-after outcome when the area of resection involves tooth-bearing zones. Chronic perisoft tissue pedicle hyperplasia with secondary infection leading to gradual bone loss is a simple complication to manage in the general population, but it becomes a serious issue in the fibula mandibular reconstruction patient in that it can lead to pathological fracture of the fibula. A case of a patient with a near fracture of his fibula mandibular reconstr

Perils and pitfalls of open tibial fractures.

The status of the fibula is an index of the severity of injury. Open tibial fracture with associated fractured fibula is more likely to develop complications. Antibiotics should be started promptly. Judicious debridement is essential but bone fragments should be left whenever possible. When there is any question about contamination, the wound is best left open. Rigid occlusive dressings promote epithelial ingrowth. Fracture distraction and plate fixation are associated with a high incidence of complications.

Incidence of occult fracture in children with acute ankle injuries.

BACKGROUND: In skeletally immature children, it can often be difficult to differentiate occult Salter-Harris I fibula fractures from ankle sprains based on physical examination, and often, initial radiographs in both conditions are only notable for soft tissue swelling. The likelihood of a child having subsequent plain radiographic evidence of a fracture in this setting and the likelihood of subsequent fracture displacement have not been previously reported. The purpose of our study was to determine the incidence of occult fracture in these patients and the risk of fracture displacement. METH

Natural tibialization of fibula in non-union tibia: Two cases.

BACKGROUND: Non-union of tibia is known to be a common complication after fracture both bones of leg treated conservatively. During the course of natural healing, fibula usually unites early as it had more soft tissue attachment and vascular supply. Due to early union of fibula and absence of axial force across the tibia, it undergoes non-union. CASE CHARACTERISTICS: Two cases, a 32-year-old male and 65-year-old female treated conservatively for fracture both bones of leg long years back, presented to us with mild calf pain on and off. On radiological examination, there was non-union of tibia

Nonunion of fibula: a systematic review.

PURPOSE: The purpose of this systematic review was to analyse the available evidence regarding nonunions of the fibula. We focussed on the incidence, risk factors, evaluation, and treatment modalities for fibular nonunions as evident in the current literature and propose a treatment algorithm. METHODS: This was an Institutional Review Board (IRB) exempt study performed at a level one trauma centre. We systematically reviewed the published evidence on fibular nonunion or delayed union from 1950 to February, 2011. RESULTS: Twelve articles were included in this systematic review. In summary, no

[Treatment of luxation fractures of the talocrural joint].

Authors have analysed some characteristics of the functional anatomy, biomechanics, classification and treatment of fractures of the talocrural joint with special emphasis of sprained fractures. Based on data from different references, as well as on the authors' experience, they came to the conclusion that a great percentage of these fractures heal with shortening of the fibula; the latter causes disorders of the biomechanical mechanism in this joint. The importance of the fibula especially in the statics and the dynamics of the lower limb has been stressed out, as well as the possibility of p
