Where lymphatic filariasis has diminished since about the 1950s, it has most frequently, though not always, been a direct result of chemotherapeutic intervention against the parasite. Diethylcarbamazine (DEC), a well-established drug, has been the single agent of chemotherapeutic control and has been successful in a wide variety of regimens. This paper reviews the experience with one strategy: long-term, low-dose treatment through DEC-medicated common salt. Diethylcarbamazine-medicated salt played a major role in the Chinese filariasis control program and has been successful in more limited tr…

Lymphatic filariasis is the most widespread of human filarial infections, a group of vector-borne infestations. After the discovery of diethylcarbamazine (DEC), little advance was made in the development of new chemotherapeutic agents for the treatment of lymphatic filariasis until 1985. Since then, several new initiatives have occurred as the result of a global effort by the World Bank/UNDP/WHO Special Programme on Tropical Diseases and the Onchocerciasis Control Programme. Some of these global research initiatives are reviewed in this paper. Recent observations throw a new light on the ratio…

Three factors are involved when vector control is incorporated into an integrated control program for filariasis: (1) the vector species or species complex of each filarial worm; (2) type of control measures employed; (3) pesticide application. Recent advances in vector control in filariasis include the use of bed nets, environmental manipulation, insecticides and biological control. Three promising advances in vectrol control are the use of pathogenic bacteria such as Bacillus thuringiensis H-14 and B. sphaericus, juvenile hormone mimics or insect growth regulator (IGR) and predatory crustace…

A study of 325,000 residents of 314 villages in six governorates of the Nile delta area of Egypt revealed that the prevalence of lymphatic filariasis increased from < 1% in 1965 to > 20% in 1991, especially in the governorates of Qalyubiya, Monufiya, Dakhaliya, and Giza. The distribution of the communites with endemic filariasis is focal. Clusters of villages with high prevalences are surrounded by others in which the disease is absent, although their environmental, social, and agricultural features appear similar. The article analyses why the significant decline in filariasis between 1945 and…

In 1984-1990 longitudinal or sectional serological surveillance on filariasis by ELISA with Brugia malayi adult antigen was carried out in 6 villages, Yuhang County, Zhejiang Province, where malayan filariasis had been under control. During that period the microfilaraemia rate of inhabitants had been declining continuously till nil, the positive rate and mean optical density (OD) value in ELISA for positive inhabitants concurrently declined year by year, especially for children under age of 10. By the 5th year after control both the positive rate and mean OD value were close to those of health…

In some parasitic infection such as toxoplasmosis, specific IgA is a highly reliable marker of active infection. In bancroftian filariasis, only 10 of 20 (50%) and 3 of 20 (15%) of the microfilaremic patients were positive for IgA anti-Brugia malayi using respectively indirect ELISA and immunocapture ELISA tests. As regard to these low sensitivities, the detection of specific IgA is unlikely to be a useful test for the diagnosis of active Wuchereria bancrofti infection.

Acute lymphatic filariasis developed in an American traveling recreationally to Asia. The illness was characterized by fatigue, eosinophilia, and lymphedema of the arm and chest wall, but no lymphangitis, lymphadenitis, or pain. Complete resolution occurred over 1-2 years. We discuss this syndrome and describe the use of new diagnostic tests in its diagnosis and management.

This study examines the relationship between the dynamic of Wuchereria bancrofti infection and the development of chronic lymphatic disease. … ately 11% of the population at risk eventually develop lymphoedema. These observations suggest that filariasis endemic populations consist of those individuals who remain amicrofilaraemic and asymptomatic, and those who progress through the sequence: uninfected, microfilaraemic, amicrofilaraemic, to develop irreversible obstructive lymphatic pathology.

In this paper, Bryan Gren fell, Edwin Michael and David Denham review the appropriateness of feline filariasis as a model of the population dynamics of human lymphatic filarial infection and disease. Because of the longevity of infection and our inability to measure the adult parasite population in humans, research in filariasis is particularly dependent on the use of laboratory animal models. We demonstrate that Brugia pahangi infection patterns in the cat closely parallel those of Brugia and Wuchereria in humans. Although primary infections in 'susceptible' cats are long-lived, repeatedly in…

We describe a useful technique for removing filaria from the anterior chamber. In the case described, an immature female filaria approximately 15 mm in length was removed from the anterior chamber of a 32-year-old man in western Oregon and identified as a species of Dipetalonema. Features of four cases of filarial infection in the anterior chamber from the same geographic area are described. In three out of four cases, the worms were removed alive without difficulty by means of simple irrigation aspiration system.

Twelve rodents (Mastomys natalensis and Meriones persicus) were infected, with L3 larvae of Litomosoides carinii and Dipetalonema viteae. Eighty days after infection the number of microfilariae (L1 larvae) was determined by smears from the peripheral venous blood. It was found that there were 48 and 69 X 10(3) microfilariae per ml blood. The hosts were killed and the eyes (conjunctiva, cornea, uvea, posterior part of the globe, optic nerve) as well as other organs (skin, brain, liver, heart and skeletal muscles, kidney, diaphragm and lung) were embedded for electron microscopic studies. Eight

A study on bancroftian filariasis in Jakarta has indicated that one person in one year could be exposed to 223,000 bites of Culex pipens fatigans and to 1,941 infective-stage larvae of Wuchereria bancrofti. Blood surveys with 20 mm3 samples revealed a microfilaria rate of 6%. Although some cases of hydrocele (4% of 272 males examined) were found, there was little evidence of severe filarial disease in either males or females. Amongst wild-caught mosquitoes only 0.3% contained infective larvae, but much higher levels of vectorial competence were established under laboratory conditions. High dai…

An important hypereosinophilia is usually regarded as an interesting argument of presumption in lymphatic filariasis. This present report concerns 278 Polynesians people infected with Wuchereria bancrofti pacifica. Six kinds of patients were analysed: carriers of microfilarias, patients with elephantiasis, with lymphangitis attacks, new infections, carriers of microfilariasis recently treated with Diethylcarbamazine and, at least, 100 fellows without filariasis. The results which were observed differ, in some aspects, from the usually admitted ideas.
