Is there a role for low intensity pulsed ultrasound (LIPUS) in delayed or nonunion following arthrodesis in foot and ankle surgery?

BACKGROUND: Delayed union and nonunion following foot and ankle arthrodesis is a disabling complication for patients. There are no clinical … ents required further surgical revision surgery while 2 were treated conservatively. Isolated small foot joints demonstrated a higher incidence of fusion (9/10; 90%) after LIPUS in comparison to larger or multiple joint arthrodesis (3/8; 38%). CONCLUSIONS: There may be a role for the use of LIPUS as a treatment option in delayed union of isolated, small foot …

Contribution of ankle-foot orthosis moment in regulating ankle and knee motions during gait in individuals post-stroke.

BACKGROUND: Ankle-foot orthosis moment resisting plantarflexion has systematic effects on ankle and knee joint motion in individuals post-stroke. However, it is not known how much ankle-foot orthosis moment is generated to regulate their motion. The aim of this study was to quantify the contribution of an articulated ankle-foot orthosis moment to regulate ankle and knee joint motion during gait in individuals post-stroke. METHODS: Gait data were collected from 10 individuals post-stroke using a Bertec split-belt instrumented treadmill and a Vicon 3-dimensional motion analysis system. Each par

Charcot's joint: an overlooked diagnosis.

Charcot's joint, also known as neuropathic osteoarthropathy, is a common complication of diabetes that often is unrecognized and misdiagnosed. It may be present in up to 35% of patients with diabetic neuropathy. This disorder causes progressive destruction of weight bearing joints leading to dislocations, fractures, and deformities. We report a case of Charcot's joint in a 55-year-old man with type two diabetes. He presented with unilateral foot and ankle swelling, foot pain, warmth, and erythema. A magnetic resonance image of his foot revealed changes consistent with a Charcot's joint. Treatm

The dose effect of continuous passive motion in postoperative rehabilitation of the first metatarsophalangeal joint.

Continuous passive motion (CPM) has been utilized in postoperative rehabilitation of many joints of the body. Little has been published regarding its applications in the first metatarsal phalangeal joint or other joints of the foot. A review of the basic science and clinical studies regarding the dose effects on the healing of articular cartilage defects and periarticular tissues has led to possible correlations in treating disorders of the first metatarsal phalangeal joint. Proposed protocols are being presented which may be helpful in future clinical studies to confirm the benefits of CPM in

Changes in foot function, disease activity, and disability after forefoot resection arthroplasty in patients with rheumatoid arthritis.

The purpose of this study was to investigate the changes in foot function, disease activity, and disability in patients with RA after resection arthroplasty of the forefoot (arthroplasty). Arthroplasty was performed on 11 patients with RA. All study patients underwent clinical assessment to measure disease activity (Disease Activity Score in 28 Joints-C-reactive protein, DAS28-CRP), disability (Health Assessment Questionnaire-Disability Index, HAQ-DI) and foot function (Foot Function Index, FFI) at the following stages: preoperatively and 1, 3, and 12 months after surgery. Following arthroplas

Accessory Lesser Metatarsal Sesamoids in All of the Metatarsophalangeal JointsA Case Report.

Lesser metatarsal sesamoids are one of the most common accessory bones of the foot and are most commonly seen at the fifth metatarsophalangeal joint. They are rarely seen in other metatarsophalangeal joints. In the literature, there are reports of solitary accessory sesamoid bones seen at lesser metatarsophalangeal joints. We report the case of a 68-year-old woman with lesser metatarsal sesamoids accompanying all of the metatarsophalangeal joints.

[Progression of foot deformities in rheumatoid arthritis--a radiologic follow-up study over 5 years].

OBJECTIVE: The typical changes of the foot in patients with rheumatoid arthritis (RA) are the rear foot valgus and the flattening of the longitudinal arch as well as the splayfoot with hallux valgus and little toe deformities. These foot deformities are not so much a cosmetic problem, but are very painful and limit the patient's mobility. METHODS: The progression of rheumatic foot deformities with a follow-up of five years was described in 36 patients (70 feet) with RA and an average duration of the disease of 19.2 years (+/- 9.8 years). The analysis was based on standardized X-rays of the fe

Reconstructive foot surgery.

In rheumatoid arthritis (RA) we find a high rate of foot involvements, especially in progressive cases when we see patients submitted for hospital treatment. In about 50% of these patients the ankle joints are involved (25), in juvenile RA even in two thirds of all cases. More than half of all progressive cases show also joint affections of the forefoot. Mainly the metatarsophalangeal joints are involved, especially the first MTP joint. We also can find a high involvement rate of the interphalangeal joint of the big toe. In RA the peripheral toe joints II-V are less frequently involved. Change

Effects of Ankle Arthrodesis on Biomechanical Performance of the Entire Foot.

BACKGROUND/METHODOLOGY: Ankle arthrodesis is one popular surgical treatment for ankle arthritis, chronic instability, and degenerative deformity. However, complications such as foot pain, joint arthritis, and bone fracture may cause patients to suffer other problems. Understanding the internal biomechanics of the foot is critical for assessing the effectiveness of ankle arthrodesis and provides a baseline for the surgical plan. This study aimed to understand the biomechanical effects of ankle arthrodesis on the entire foot and ankle using finite element analyses. A three-dimensional finite ele

Talectomy by Medial Surgical Approach for Congenital Vertical Talus in Arthrogryposis Multiplex Congenita.

Arthrogryposis multiplex congenita involves stiff contracture of joints and weak atrophic muscles presenting at birth. The two most common forms are amyoplasia and distal arthrogryposis. Amyoplasia affects all 4 extremities: internally rotated shoulders, extended fixed elbows, flexed fixed wrists, extended fixed knees, clubfeet, and decreased muscle volume. Distal arthrogryposis is a group of syndromes with a genetic basis. The distal joints are contracted. Clubfeet and congenital vertical talus are the most common foot deformities. A 10-year-old boy presented with distal arthrogryposis with b

Plantar Pressure Distribution in Patients with Diabetic Peripheral Neuropathy and a First-Ray Amputation.

BACKGROUND: Elevated dynamic plantar pressures are a consistent finding in diabetic patients with peripheral neuropathy, with implications for plantar foot ulceration. This study aimed to investigate whether a first-ray amputation affects plantar pressures and plantar pressure distribution patterns in individuals living with diabetes and peripheral neuropathy. METHODS: A nonexperimental matched-subject design was conducted. Twenty patients living with diabetes and peripheral neuropathy were recruited. Group 1 (n = 10) had a first-ray amputation and group 2 (n = 10) had an intact foot with no

Foot and ankle biomechanics.

With the advent of imaging advancements there has been renewed interest in the foot and ankle. However, many of the basic functions and biomechanical considerations of the bones, joints, and specialized tissues of the normal and dysfunctional foot and ankle remain unfamiliar to many radiologists. This article focuses on the basic biomechanics, normal alignment, and common alignment disorders of the foot and ankle that are relevant to radiologists.

Destructive pseudo-neuroarthropathy associated with calcium pyrophosphate deposition.

BACKGROUND: Calcium pyrophosphate deposition (CPPD) disease is a metabolic disorder characterized by soft tissue calcific deposits formed primarily in articular cartilage. What can result is a crystal-induced arthropathy often referred to as pseudogout, which is variable in both presentation and severity. A particularly destructive and deforming arthritis is an uncommon but well-recognized subtype of this disease. Radiologically resembling the neuroarthropathy described by Charcot, a pattern of joint fragmentation and structural collapse occurs in the absence of peripheral neuropathy. This pse

Calcaneocuboid joint pressure after lateral column lengthening in a cadaveric planovalgus deformity model.

The purpose of this study is twofold: first, to measure the joint contact pressure across the calcaneocuboid joint in a planovalgus deformity and compare the results to pressures measured in a normal foot; and second, to determine the change in pressure across the calcaneocuboid joint after an Evan's-type calcaneal lengthening osteotomy. The effect of this procedure on the calcaneocuboid joint was evaluated using seven cadaver feet to measure peak pressure across the calcaneocuboid joint under a constant load. Each foot was sectioned medially to reproduce a deformity consistent with an adult,

Relationship between forward propulsion and foot motion during gait in healthy young adults.

The foot contributes to propulsion and postural stability function by changing its morphology during walking through the truss/windlass mechanisms. We quantified the truss mechanism regarding foot stiffness and the windlass mechanism regarding the movement coordination between the foot arch and metatarsophalangeal (MTP) joint. We aimed to clarify the relationship of these mechanisms with propulsive force and static foot alignment. Forty-eight healthy young adults participated and walked at a comfortable speed. The ground reaction force (GRF), ankle power, and sagittal plane motion of the foot
