Echocardiographic evaluation of patent foramen ovale and atrial septal defect.

Patent foramen ovale (PFO) is a common variant present in up to 25% of the population. Atrial septal defect (ASD) is a direct communication between the 2 atrial chambers, of which the ostium secundum variety is the most common. This manuscript is an in depth review of the complex atrial septation, the diagnosis of PFO and ASD and its clinical and therapeutic implications.

Imaging characteristics of ischemic strokes related to patent foramen ovale.

BACKGROUND AND PURPOSE: Subclinical atrial fibrillation (AF) and patent foramen ovale (PFO) are the major causes of cryptogenic stroke, and neuroimaging may help distinguish the cause. We compared the imaging characteristics of ischemic stroke caused by PFO (PFO-stroke) and AF (AF-stroke). METHODS: We recruited 117 patients with PFO-stroke and 358 patients with AF-stroke after excluding other causes. The lesion patterns were classified according to number, location, size, and pertinent vascular territory and were compared between the 2 groups. Occlusion of the corresponding artery and its rec…

Indications, techniques and results of percutaneous patent foramen ovale closure.

Patent foramen ovale (PFO) has been shown to be more prevalent in certain disease states, suggesting a potentially causative role in some patients with cryptogenic stroke, migraine headache or a number of other conditions. Percutaneous PFO closure has become a reasonable treatment option in a subset of those patients. Our objective is to review the possible indications of PFO closure. We further elucidate the technical aspects of PFO closure, provide an overview of the available devices and summarize results of PFO closure procedures.

Amebic liver abscess, extensive thrombosis, and patent cardiac foramen ovale.

Extensive venous thrombosis is usually seen postmortem in amebic liver abscess because of its dismal prognosis. Herein, we describe amebic liver abscess, whose late diagnosis led to multiple deep thromboses, pulmonary embolism, and right atrial thrombosis, in this patient with patent foramen ovale.

Original Investigations. Potential faces of patent foramen ovale (PFO PFO).

BACKGROUND: Patent foramen ovale (PFO) is diagnosed on echocardiography by saline contrast study with or without color Doppler evidence of shunting. PFO is benign except when it causes embolic events. METHODS AND RESULTS: In this report, we describe unique additional manifestations related to the diagnosis and presentation of PFO. These include demonstration of PFO during the release phase of "sigh" on the ventilator in the operating room, use of a separate venipuncture to allow preparation of blood-saline-air mixture after multiple failed saline bubble injections, resting and stress hypoxemi…

[Patent foramen ovale: "les liaisons dangereuses" between anatomy and genetics].

We reported a case of two 24-year-old and 17-year-old male patients with episode of transient ischemic attacks and diagnosed as having patent foramen ovale (PFO). One patient had heterozygosity for the factor V Leiden mutation, and one other had heterozygosity for prothrombin G20210A mutation. Both of them were also carriers for MTHFR 677T genotype with elevated plasma levels of homocysteine (22.3 +/- 3.9 micromol/L). These findings strongly confirm and emphasize the importance of the genetic screening for thrombotic mutations in young patients with PFO-related ischemic events in order to impr…

Is transesophageal echocardiography still necessary to exclude patent foramen ovale?

OBJECTIVES: Current guidelines still recommend transesophageal echocardiography (TEE) as reference method to diagnose interatrial shunts. Th … unts by both TTE and TEE, but 10 shunts (5%) only by TTE. CONCLUSIONS: Our study shows that patent foramen ovale can safely be demonstrated with high-end transthoracic contrast- echocardiography. If additional studies confirm our results, TTE has the potential to become the method of choice in the diagnosis of PFO.

Does patent foramen ovale promote cryptogenic stroke and migraine headache?

Cryptogenic stroke is a diagnosis of exclusion. These are strokes that occur in people who are usually less than 55 years old, without an identifiable cause. Our sensitivity to these events has been heightened because of the new definitions of a transient ischemic attack. Transient ischemic attack (TIA) is a clinical diagnosis of a neurologic deficit without MRI abnormalities: if there is an MRI abnormality, whether or not that person is symptomatic, it is now defined as a stroke. With these new definitions, and the sensitivity of MRI, we are seeing more cryptogenic strokes. It has been hypoth

Scuba diving has become a popular leisure time activity with distinct risks to health owing to its physical characteristics. Knowledge of th … toms without evident cause, transoesophageal echocardiography must be performed to exclude a patent foramen ovale. If a cardiac right-to-left shunt is present, we advise divers with a history of severe decompression illness to stop diving. If they refuse to do so, it is crucial that they change their diving habits, minimising the amount of nitrogen load on the …

[Ebstein's anomaly with patent foramen ovale and supraventricular paroxysmal tachycardia].

A case of asymptomatic 36 years old men with Ebstein anomaly of tricuspid valve, patent foramen ovale and paroxismal supraventricular tachycardia is presented in this paper. A routine physical examination revealed a heart systolic murmur along left and right side of sternum, with right bundle branch block on ECG. It led to the detailed non-invasive cardiologic examination (transthoracic, transeophageal and stress echocardiography, ambulatory ECG), what disclosed above mentioned Ebstein anomaly. Also he was operated on upon superior lip fissure at the first year of life. Regarding his very mild…

Percutaneous biopsy of cavernous sinus tumors through the foramen ovale.

OBJECTIVE: We present a new application to the widely used Hartel percutaneous route through the foramen ovale to the parasellar region to obtain aspiration biopsies of lesions arising from or invading the posterolateral part of the cavernous sinus. METHODS: Thirteen patients with cavernous sinus lesions that were diagnosed using imaging underwent aspiration biopsy by this route. Nine of them were subsequently operated on, and two will undergo surgery in the near future. For the remaining two, surgical removal of the tumor was not indicated. One of these two patients had a meningioma without …

Bilateral pterygomeningeal arteries penetrating the skull anterior to the foramen ovale.

PURPOSE: The pterygomeningeal (accessory meningeal) artery is an inconstant branch of the middle meningeal or maxillary artery. A variant that penetrates the skull anterior to the foramen ovale is rare, and bilateral coexistence has not been reported. A cadaver with bilateral variants of this type is described. METHODS: The head of a 79-year-old man was examined during a gross anatomy course. Origin, course, and relations of the variant vessels were recorded on both sides. RESULTS: On the left, an accessory middle meningeal branch of the middle meningeal artery penetrated the greater wing ap…

Thrombus in Transit Through a Patent Foramen Ovale: A Clinical Conundrum.

BACKGROUND: Thrombus in transit through a patent foramen ovale (PFO) is a rare clinical entity that can be associated with concurrent pulmonary embolization and lead to cryptogenic stroke. CASE SUMMARY: A 56-year-old patient presenting with shortness of breath and chest pain was diagnosed with large bilateral pulmonary emboli. Computed tomography demonstrated thrombus in the right atrium. Transthoracic echocardiogram demonstrated a large burden of thrombus in transit trapped in a PFO. After multidisciplinary discussion, emergency surgical thrombectomy and PFO closure were carried out successf…

Stroke Prevention in Pregnancy and Postpartum in Patients With Patent Foramen Ovale.

Pregnancy and patent foramen ovale (PFO) are both stroke risk factors in young people. There are currently no guidelines for mitigating PFO-related stroke risk during pregnancy. We summarize the pathophysiology of PFO-related stroke and offer our approaches to primary and secondary stroke prevention for pregnant patients with PFO. We present 5 clinical scenarios: incidentally identified PFO in a pregnant patient without a stroke history; considerations for PFO screening and primary stroke prevention in select patients at unusually high risk of stroke in pregnancy; planning for pregnancy after …

Transcatheter closure of patent foramen ovale in children with migraine or dizziness.

UNLABELLED: In recent years, several studies have suggested that transcatheter patent foramen ovale (PFO) closure can relieve migraine and may also improve unexplained dizziness in adults, but evidence in children remains limited. We retrospectively analyzed 95 children (45 boys; age 5.00-18.00 years) who underwent transcatheter PFO closure between June 2019 and September 2024 and were followed for a median of 18.00 months (interquartile range [IQR]: 9.00-27.00). Indications were severe migraine (n = 83; 33 also had dizziness) or recurrent dizziness without migraine (n = 12). Efficacy was eval…

Pectus Excavatum, Patent Foramen Ovale, and Migraine in the Same Patient.

BACKGROUND: A patent foramen ovale (PFO) can be found in patients with pectus excavatum (PE); PFO can be associated with migraines. CASE SUMMARY: A 22-year-old woman presents a PE and a history of recurrent migraines. Pulsed oxygen saturation at effort decreased from 97% to 92%. A transthoracic echocardiography showed a PFO. Five years after the Ravitch procedure, pulsed oxygen saturation remained stable at effort, no PFO was shown on transthoracic echocardiography, and migraine disappeared. DISCUSSION: Our observation is the first description of the PE, PFO, and migraine association. The co…

The Hidden Dangers of Additional Shunts in Patent Foramen Ovale Treatment.

BACKGROUND: Unroofed coronary sinus (UCS) is a rare congenital defect that causes a left-to-right shunt, often associated with anomalies such as persistent left superior vena cava. Its diagnosis and management are challenging. CASE SUMMARY: A 49-year-old woman with worsening dyspnea, orthopnea, and peripheral edema unresponsive to diuretics was referred for evaluation. She had a history of epilepsy and had had a patent foramen ovale (PFO) closure 1 year before. Transthoracic echocardiography revealed right ventricular dilation, preserved biventricular function, and increased pulmonary pressur…

Pulmonary embolism with thrombus in transit across a patent foramen ovale.

Thromboembolism-in-transit, specifically impending paradoxical embolism (IPDE), is a rare and life-threatening condition with limited reported cases. We present a case of a 51-year-old male with obstructive sleep apnea, initially diagnosed with deep vein thrombosis and pulmonary embolism. Further evaluation revealed a saddle pulmonary embolus extending into the right atrium, straddling a patent foramen ovale (PFO), confirmed by transesophageal echocardiogram. Despite a critical left anterior descending coronary artery stenosis, surgical thrombectomy, PFO closure, and coronary artery bypass gra…
