Island medial plantar artery perforator flap for reconstruction of plantar defects.

Three cases in which island medial plantar artery perforator flaps were successfully transferred for coverage of the plantar defects are described. This perforator flap is different from the medial plantar flap based on the medial plantar artery. The flap has no fascial component and is nourished only with the perforator of the medial plantar vessel. Therefore, transection of the medial plantar artery is usually unnecessary. This flap can cover defects on the forefoot and heel without transaction of the medial plantar system. The advantages of this flap are no need for deep or long dissection

Modifications of the great toe fibular flap for diabetic forefoot and toe reconstruction.

Diabetic forefoot and toe wounds are difficult to heal in a timely fashion. Despite proper continued offloading and protective shoe-gear techniques, these wounds are frequently complicated by recurrent breakdown and adjacent toe deformities. When properly performed, pedicle flaps from the fibular border of the great toe represent viable and cost-effective means of providing early and durable soft tissue coverage and osseous reconstruction of diabetic forefoot and lesser toe wounds. Two modifications of the traditional great toe fibular flap are described: the great toe fibular adipofascio-cuta

Long-term results of the modified Hoffman procedure in the rheumatoid forefoot.

BACKGROUND: Rheumatoid arthritis commonly affects the forefoot, causing metatarsalgia, hallux valgus, and deformities of the lesser toes. Various types of surgical correction have been described, including resection of the lesser-toe metatarsal heads coupled with arthrodesis of the great toe, resection arthroplasty of the proximal phalanx or metatarsal head, and metatarsal osteotomy. We report the results at an average of five and a half years following thirty-seven consecutive forefoot arthroplasties performed in twenty patients by one surgeon using a technique involving resection of all five

Automatic joint inflammation estimation based on regression neural networks.

BACKGROUND: Quantitative assessment of inflammation from hand and forefoot MRI scans is crucial for evaluating the severity, progression, and treatment response in inflammatory disease like rheumatoid arthritis (RA). Traditionally, this relies on visual evaluation of signs like bone marrow edema (BME), tenosynovitis, and synovitis, which is time-consuming, subjective, and prone to inherent inter/intra-reader variability. PURPOSE: This study aims at an automatic DL-based MRI analysis of inflammatory signs in RA system for inflammation assessment to facilitate related diagnoses and studies. ME

Three-Dimensional Kinematics of the Human Metatarsophalangeal Joint during Level Walking.

The objective of this study is to investigate the three-dimensional (3D) kinematics of the functional rotation axis of the human metatarsophalangeal (MP) joint during level walking at different speeds. A 12 camera motion analysis system was used to capture the 3D motion of the foot segments and a six force plate array was employed to record the simultaneous ground reaction forces and moments. The 3D orientation and position of the functional axis (FA) of the MP joint were determined based on the relative motion data between the tarsometatarsi (hindfoot) and phalanges (forefoot) segments. Effect of forefoot transverse arch stiffness on foot biomechanical response--based on finite element method.

BACKGROUND: The plantar vault, comprising the transverse and longitudinal arches of the human foot, is essential for impact absorption, elastic energy storage, and propulsion. Recent research underscores the importance of the transverse arch, contributing over 40% to midfoot stiffness. This study aimed to quantify biomechanical responses in the ankle-foot complex by varying the stiffness of the deep metatarsal transverse ligament (DTML). METHODS: Using CT image reconstruction, we constructed a complex three-dimensional finite element model of the foot and ankle joint complex, accounting for g

Landing Technique Effects on Lower-Limb Biomechanics During Half-Squat Parachute Landing.

INTRODUCTION: During a half-squat parachute landing, foot-strike pattern influences joint kinematics and injury risk. This study compared forefoot and full-foot landing techniques to determine their effects on joint biomechanics, impact forces, and landing stability. METHODS: A total of 17 male paratroopers performed both landing conditions. Lower-limb kinematics, ground reaction forces, and surface electromyography were synchronously recorded using infrared motion capture, a force platform, and wireless electromyography. Paired-sample t-tests or Wilcoxon signed-rank tests were applied (α = 0

The unexpected influence of a stiff minimalist insole on the forefoot and ankle joint biomechanics during walking.

UNLABELLED: The aim of this study was to explore the efficacy of a minimalist stiff insole in limiting/reducing first metatarsophalangeal joint (1MTP) range of motion and to explore the impact on the ankle joint during walking. It was hypothesized that the minimalist stiff insole would significantly reduce 1MTP kinematic and kinetic parameters whilst causing compensatory effects at the ankle joint. METHODS: Fourteen healthy participants were recruited. Three-dimensional lower limb and forefoot kinematics were measured during walking at a self-selected speed with a stiff and control insole. Di

Prophylactic forefoot decompression prevents acute compartment syndrome of the foot following elective surgery in children.

INTRODUCTION: Acute compartment syndrome (ACS) of the foot is practically unknown following elective surgeries. Therefore, the purpose of this study is to: 1. investigate the incidence, relative risk (RR) and odds ratio (OD) in relation to the type of foot surgery, 2. to study the benefits of prophylactic decompression of the forefoot on the occurrence of ACS, postoperative pain, hospital stay, additional surgeries and morbidity MATERIALS AND METHODS: This retrospective study was done in children operated between 2008 and 2022. The feet were divided into group -1 (2008-2018) with no prophylact

Reconstruction of extensive plantar forefoot defects with free anterolateral thigh flap.

PURPOSE: The aim of the soft tissue reconstruction of plantar forefoot should yield weight-bearing function and aesthetic contour, which poses a significant challenge for reconstructive surgeons to provide an appropriate flap according to the "like for like" reconstructive principle. Local flaps and pedicled flaps have been described for the reconstruction of small- to medium-sized defects of plantar forefoot and achieved optimal results. However, reconstruction of extensive defects of plantar forefoot is rarely investigated. In this study, we present our experience using the free anterolatera

An Algorithm for Forefoot Reconstruction With the Innervated Free Medial Plantar Flap.

BACKGROUND: Because of shearing forces, the forefoot is more prone to trophic ulcers than the heel. Reconstruction of trophic ulcers and other forefoot defects is a vexing challenge. We favor the innervated free medial plantar flap to replace like-with-like and confer protective sensation. We investigate the feasibility of this flap with the largest series to date and the first to describe ipsilateral flap transfer. METHODS: Between 2009 and 2013, 7 patients with forefoot defects were treated with innervated free medial plantar flaps. The average age of 4 men and 3 women was 35.1 years (range

Anatomic basis and clinical application of the distally based medialis pedis flaps.

BACKGROUND: Soft-tissue defects of the forefoot are difficult to cover adequately, particularly, although multiple options for reconstruction are available. This study especially focused on the vascularization of the medial side of the foot and the determination of the contribution of the nutrient vessels to medialis pedis flap viability. METHODS: Thirty cadavers were available for this anatomical study. Microdissection was conducted under a microscope, and details of the course and distribution and the communication of the first plantar metatarsal artery with the fascial vascular network of

Biomechanical behavior of valgus foot in children with cerebral palsy: A comparative study.

Valgus foot (VF) is the most common foot deformity in children with cerebral palsy (CP), which seriously affects the foot balance in standing and posture control in walking. Little information about the locus and stress of internal bones was available. To accurately describe the biomechanical behavior of the internal bones of VF in CP, we compared the locus and stress of internal bones between the normal foot (NF) and VF by finite element models. Compared with the NF, displacement of the talus and navicular drop in VF increased by 109% and 171% in vertical direction respectively, and the locus

The effect of subtalar joint position on dorsiflexion of the ankle/rearfoot versus midfoot/forefoot during gastrocnemius stretching.

BACKGROUND: Limited ankle joint dorsiflexion passive range of motion (PROM) has been associated with common chronic lower extremity conditions, and clinicians often instruct patients in stretching exercises to increase dorsiflexion. However, little is known about how subtalar joint (STJ) position affects dorsiflexion at the midfoot/forefoot versus ankle/rearfoot during gastrocnemius stretching. The purpose of this study was to determine if more dorsiflexion occurs at the ankle/rearfoot and less at the midfoot/forefoot during gastrocnemius stretching with the STJ positioned in supination versus

Plantar pressures in children with congenital talipes equino varus--a comparison between surgical management and the Ponseti technique.

The Ponseti technique for treatment for Congenital Talipes Equinovarus (CTEV) was introduced in the authors' institution in 2001, substituting a more traditional orthopaedic approach which involved surgery. There is currently limited published information regarding the differences in clinical outcomes between these two techniques, particularly in relation to plantar pressure analysis. This study aims to determine differences in pedobarographic outcome in children with CTEV, treated with either a surgical or Ponseti approach. A high resolution pedobarograph was used to record plantar pressure d

Midfoot ulcers treated with gastrocnemius-soleus recession.

BACKGROUND: Foot ulcers in patients with neuropathy are a common cause of hospital admission for infection sometimes resulting in amputation in patients with neuropathy. Tendon lengthening alone has been reported to be successful in treating neuropathic forefoot ulcers. Tendon lengthening has also been recommended as an adjunct to bony procedures (exostectomy or fusion) for treating midfoot ulcers. The author reports the results of gastrocnemius-soleus recession as the sole treatment of diabetic midfoot ulcers. MATERIALS AND METHODS: This study evaluated the results of 11 patients with 11 neu

Differential diagnosis of Charcot arthropathy and osteomyelitis.

Foot problems are common causes of morbidity in patients with diabetes mellitus. Foot ulcers are the leading cause of hospitalization in diabetic patients. Bones may be involved in two different clinical conditions: osteomyelitis and Charcot osteoarthropathy. Osteomyelitis usually develops by spreading from contiguous soft tissue to underlying bone. Charcot foot is deformation of foot as a result of muscle athrophy, bone and joint structure changes in a joint as a secondary complication of neuropathy. To distinguish bone infection from non-infectious bone disorders as in Charcot joint may be d

Structural changes in the forefoot of individuals with diabetes and a prior plantar ulcer.

BACKGROUND: Plantar ulcers produced by diabetic foot disease are devastating and costly. Better understanding of the ulcer-producing process is important to improve detection of feet that are at risk and to improve intervention. We identified and quantified soft-tissue and osseous structural changes in the forefoot of diabetic patients with a prior plantar ulcer. METHODS: Thirty-two individuals with a mean age (and standard deviation) of 57 +/- 11 years were studied; sixteen had diabetes (of a mean of 20 +/- 11 years' duration), peripheral neuropathy, and a prior plantar ulcer, and sixteen we

The deep peroneal nerve in the foot and ankle: an anatomic study.

Injury to the deep peroneal nerve in the foot and ankle may result from trauma, repetitive mechanical irritation, or iatrogenic harm. The nerve is most susceptible to injury along its more distal anatomic course. Dissection of 17 cadaver specimens was undertaken to describe the course of the deep peroneal nerve and quantify its branch patterns. In the distal one third of the leg, the nerve was located superficial to the anterior tibial artery between the tibialis anterior and extensor hallucis longus muscles. Typically, the nerve crossed deep to the extensor hallucis longus tendon to enter the
