An 18-year-old man with a fracture-dislocation of the thoracic spine showed remarkable displacement, but without neurologic complications. The halo-femoral traction allowed a reduction, which has been only partially maintained. Despite this, the clinical result is satisfactory. The lesion appears to be more common than the few cases published in the literature would suggest.

A 20-month-old infant sustained an irreducible epiphyseal fracture-dislocation of a proximal interphalangeal joint. The dorsal dislocation of the joint was associated with a 90 degree rotational displacement (Salter-Harris Type I fracture) of the epiphysis of the middle phalanx. This injury, which seems not to have been described previously, required open reduction through both volar and dorsal incisions because the intact collateral ligaments and extensor central slip did not permit adequate distraction of the joint.

INTRODUCTION: Radiocarpal dislocation (RCD) and fracture-dislocations (RCFD) are severe but rare injuries for which the treatment and outcomes are not well defined. The aim of this retrospective study was to describe the prevalence of the various injury types and their long-term outcomes.
PATIENTS AND METHODS: Between 1992 and 2014, 41 patients with RCFD were seen at our institution. According to the Dumontier classification, there were 4 cases of type 1 and 37 cases of type 2. Thirteen patients were reviewed again after a mean follow-up of 168 months (20-260).
RESULTS: Among these 41 patien

Twenty-two consecutive patients (23 wrists) underwent open reduction internal fixation of dorsal perilunate dislocations and fracture-dislocations through combined dorsal and volar approaches. One of 5 experienced wrist surgeons performed these procedures within an average of 3 days of injury (range, 0-26 days) and intercarpal fixation was kept within the proximal carpal row. Motion was instituted an average of 10 weeks (range, 5-16 weeks) after injury. All patients were males. The average age at the time of injury was 32 years (range, 16-60 years). The average follow-up period was 37 months (

Reports on late results in perilunate dislocations and fracture dislocations are rare. In a series of 22 male patients with perilunate dislocation or fracture dislocation, treated operatively between 1976 and 1987, 15 patients (68%) were investigated 12 years (8 to 19 years, median/range) postoperatively. The average range of motion of the injured wrist was decreased by one third in comparison with the uninjured wrist and posttraumatic arthrosis was observed in all patients radiologically. Additional dynamic or static scapholunate dissociation was found in 53% (n = 8) of the patients. In contr…

INTRODUCTION: Concomitant ipsilateral fractures of the distal radius and scaphoid are uncommon and are typically associated with high-energy trauma. The combination of a comminuted intra-articular distal radius fracture with scaphoid fracture-dislocation and carpal instability represents a particularly rare and complex injury pattern. Early recognition of associated carpal injuries is important to restore and maintain anatomical alignment.
CASE REPORT: A 32-year-old right-hand-dominant man presented with severe pain, swelling, and deformity of the left wrist following a high-energy motorcycle

Scaphoid dislocation represents a rare injury with only a few case reports and limited case series reported in the literature. The majority of scaphoid dislocations result from a high-energy trauma causing hyperextension and ulnar deviation of the wrist. The severity of a scaphoid dislocation depends on the degree of periscaphoid ligamentous injury as well as the presence of concomitant injuries, such as axial carpal dissociation. The most common complication after a scaphoid dislocation is scapholunate dissociation, which emphasizes the importance of scapholunate ligament repair/reconstructio

STUDY DESIGN: This was a prospective study of 50 patients of thoracolumbar fracture dislocation treated at a single institution with short-segment fixation with the inclusion of fracture level.
PURPOSE: To assess the outcomes of including the fracture level in short-segment fixation for thoracolumbar fracture dislocation.
OVERVIEW OF LITERATURE: Traditionally, thoracolumbar fracture dislocation is treated with long-segment posterior fixation. However, to save motion segments, short-segment fixation has been used instead in many cases of thoracolumbar trauma.
METHODS: In this study, 50 patie…

Canale and Kelly modified the Hawkins classification of talar neck fractures by adding a type IV fracture dislocation to the original 3 types described. Type IV injury occurs rarely and is the only type involving dislocation of the talonavicular joint. To our knowledge, only 2 unusual cases have been described in published studies in which a fracture of neck of the talus occurred in association with talonavicular dislocation but the ankle and subtalar joints remained intact. We describe a very unusual case of an adult male with a fracture of the neck of the talus with talonavicular dislocation…

Volar dislocation of the distal radioulnar joint (DRUJ) is an uncommon traumatic injury that may be overlooked due to subtle clinical findings and difficulty obtaining proper radiographs in the acute setting. The mechanism typically involves a strong pronation force applied to a fixed wrist, leading to disruption of the stabilizing soft tissue structures, particularly the triangular fibrocartilage complex (TFCC). Although standard wrist radiographs are recommended initially, computed tomography (CT) is often required when positioning is inadequate or the diagnosis remains uncertain. We report

Isolated dislocation of the capitate is an exceedingly rare carpal injury and is typically associated with high-energy trauma and complex ligamentous disruption. We present a case of a delayed-diagnosis isolated volar dislocation of the capitate with a small associated chip fracture, not conforming to classic perilunate instability patterns, and review relevant literature that describes similar case presentations and treatment modalities. A 29-year-old man presented with wrist pain and median nerve paresthesias 2 weeks after the injury. Imaging demonstrated volar migration of the proximal capi

The rate of leadless pacemaker implantation is increasing worldwide. To date, there have been few reports of leadless pacemaker dislocation and extraction. This article describes two cases of leadless pacemaker tine fracture and dislocation. One case was an elderly male, and the pacemaker was found to have fallen off to the right pulmonary artery one month after surgery, and a new leadless pacemaker was implanted; the other was an elderly female, and the leadless pacemaker fell off to the right ventricle 9 months after surgery. The leadless pacemaker was removed, and a dual-chamber permanent p

The shoulder is the commonest major joint involved in dislocations. These are often associated with fractures of the surgical neck and/or of the greater tuberosity of the proximal humerus. A good functional recovery is associated with a successful union of the tuberosity fragment, as this carries the insertion of the superior and posterior rotator cuff tendons. A 29-year-old male patient presented to our Emergency Department (ED) after a fall off his motorbike, resulting in a left shoulder fracture dislocation and an axillary nerve injury. His shoulder was reduced under sedation in the ED, wit…
