Distal Femoral Physeal Bar Resection Combined With Guided Growth for the Treatment of Angular Limb Deformity Associated With Growth Arrest: A Preliminary Report.

PURPOSE: Distal femoral growth arrest can result in progressive deformities and functional disability. The treatment is challenging given the significant growth potential of the distal femoral physis. This study addresses the short-term outcomes after distal femoral physeal bar resection combined with guided growth for the treatment of angular limb deformity. METHODS: We conducted a retrospective analysis of patients treated with distal femoral physeal bar resection, fat graft interposition, and growth modulation with a tension-band plate. Data recorded included patient demographics, growth a

Do Patient Sex and Age Affect Hemiepiphysiodesis Outcomes?

(1) Background: The purpose of this study was to assess the effects of sex and age on the outcomes of hemiepiphysiodesis performed for genu valgum and varum deformity correction. (2) Methods: We analyzed patients who had undergone O-Plate hemiepiphysiodesis due to genu valgum or varum in the period of 2020-2023. The study group comprised 22 females and 20 males aged between 3 and 14 years at the time of surgery. Age-stratification yielded a subgroup of 3-10-year-olds (16 patients, 20 treated limbs) and a subgroup of 11-14-year-olds (26 patients, 28 treated limbs). We assessed the following par…

Combined distal femoral osteotomy and tibial tuberosity distalization is effective in patients presenting with patellar instability and patellofemoral pain due to patella alta and femoral malalignment.

INTRODUCTION: Patellar malalignment has been considered one of the major pathomechanical causes of patellofemoral instability and pain. The results have been reported after femoral varization/torsional osteotomy and tibial tuberosity distalization osteotomy (TTD-O). However, the combination of a femoral deformity (genu valgum/increased femur antetorsion) and patella alta remains underreported. Therefore, the aim of this study was to investigate the clinical outcomes of patients simultaneously treated by distal femoral osteotomy and TTD-O. The hypothesis was that restoration of patellofemoral (…

Most Coronal Knee Deformities of Healed Nutritional Rickets Under 8 Years of Age Remodel Spontaneously: Building Evidence for Practice Change.

BACKGROUND: Nutritional rickets remains a global health burden. The optimal management of coronal knee deformities in children/adolescents with healed nutritional rickets as to the indications of conservative treatment versus guided growth surgery remains unsettled. The objective was to report the natural history of disease regarding the remodeling potential of coronal knee deformities and isolate potential patient- and deformity-related factors that are predictors of favorable deformity remodeling. METHODS: We prospectively assessed 109 knees in 57 patients-mean age, 6.1 years (range, 2 to 1

Effects of K-wire diameter and insertion angle on femoral bone medial closing-wedge osteotomies: a finite element study.

Medial closing-wedge surgery for distal femoral osteotomy is employed to correct genu valgum by correcting coronal plane malalignment. This procedure involves pre-surgery planning, creating a wedge incision, performing the osteotomy, and stabilizing with plates and screws. However, hinge fractures during wedge closure present significant challenges, often necessitating revisions. Contemporary solutions have explored the use of k-wires, and this study investigates their biomechanical implications. The interplay between k-wire insertion angle and diameter, often overlooked in existing literature…

Correlation between clinical tibiofemoral angle and body mass index in normal Nigerian children.

PURPOSE: The tibiofemoral angle (TFA) is a reliable tool for determining lower-limb axial alignment and defining the degree of deformity in pathologic angular malalignment of the knee in children. METHODS: We clinically examined 471 normal Nigerian elementary school children whose ages ranged from three to ten years to determine the clinical tibiofemoral angle and to establish its relationship with body mass index (BMI). RESULTS: We found maximum knee valgus of 7.87° at three years, decreasing to 1° at ten years. We also found significant negative correlation between the tibiofemoral angle a

Dyggve-Melchior-Clausen syndrome: report of seven patients with the Smith-McCort variant and review of the literature.

Dyggve-Melchior-Clausen (DMC) syndrome is a rare autosomal recessive disorder affecting skeletal development. The patients have a striking "barrel-shape" chest, shortened trunk, and various distal deformities, including genu valgum or varum, and minimal decrease in joint mobility. The most notable radiographic findings are a lacy iliac crest apophysis, hip dysplasia, double vertebral hump, and odontoid hypoplasia with atlanto-axial instability. Patients may require orthopedic femoral osteotomy, total hip arthroplasty, early meniscectomy, realignment osteotomy, or posterior cervical spine fusio…

Arch fingerprints, hypotonia, and areflexia associated with X linked mental retardation.

A syndrome with distinctive facies, poor muscle tone, absent deep tendon reflexes, tapered fingers, excessive fingerprint arches, genu valgum and mild-moderate mental retardation has occurred in four males in two generations of a white family of European ancestry. The facies are characterised by square configuration, tented upper lip, and thickening of the helices, upper eyelids, and alae nasi. At birth and at maturity, growth (head circumference, height, weight) of affected males is comparable to or greater than unaffected male sibs. Moderate impairment of cognitive function was documented (I…

Genetic disorders on the island of St Helena.

The island of St Helena lies in the South Atlantic ocean and is isolated by distance from any land mass and by lack of a harbour or airfield. In these circumstances there could be an unusual pattern of inherited disease, and for this reason we have attempted to document the presence and frequency of genetic conditions in the population, which numbers approximately 5 500. Genetic disorders identified included autosomal dominant genu valgum, hereditary angioneurotic oedema, Christmas disease and pseudo-achondroplasia. The presence of the autosomal recessive form of retinitis pigmentosa provides …

Lower Extremity Abnormalities in Children.

Rotational and angular variations of the lower extremities in children are common conditions seen in primary care. These visits are often due to parental concerns about the appearance of their child's lower extremities. Common variations include intoeing (metatarsus adductus, internal tibial torsion, and femoral anteversion); out-toeing (external tibial torsion, femoral retroversion, and pes planus); and angular variations (genu varum and genu valgum). History, particularly age of onset; duration; progression; appearance and impact; prenatal and birth history; nutritional deficiencies; growth …

An MRI-based technique for assessment of lower extremity deformities-reproducibility, accuracy, and clinical application.

The hypothesis of this study was that length, torsion, and axis of a leg phantom can be measured accurately and reproducibly by magnetic resonance imaging (MRI) and that this can be applied to patients with leg deformities. Two phantoms and 30 patients (genu varum, n = 15; genu valgum, n = 15) were investigated using an optimized MRI technique. Reference measurements were performed with a micrometer screw and a goniometer. Patient leg length and axis were compared with long radiographs in bipedal stance. Intra- and interobserver reproducibility and accuracy were calculated using the mean absol…

Influence of the antero-posterior position of a pin on the risk of a medial hinge fracture during a lateral opening wedge distal femoral varus osteotomy. A biomechanical study.

INTRODUCTION: Lateral opening wedge distal femoral varus osteotomy (LOWDFO) is indicated for early isolated lateral compartment osteoarthritis in young patients with genu valgum due to an isolated distal femoral metaphyseal deformity. A medial hinge fracture is considered a poor prognostic factor for radio-clinical outcomes, hence its prevention is crucial. The protective role of a temporary pin has been demonstrated in the literature. The primary objective of this study was to evaluate the influence of the antero-posterior position of this pin on hinge protection. HYPOTHESIS: The main hypoth…
