The many faces of human prion diseases in Belgium and the world.

Prion diseases are rare neurodegenerative disorders that always lead to death and that can be transmissible under certain conditions. Although sporadic Creutzfeldt-Jakob's disease (CJD) is the best known human variant of these transmissible spongiform encephalopathies with an incidence of about 1 in 106 inhabitants, several other types of human prion disease have been described (e.g. Familial CJD, Gerstmann-Sträussler-Scheinker syndrome, Fatal Familial Insomnia,...). In 1996, a variant of CJD has been linked to the epidemic of bovine spongiform encephalopathy (BSE). Therefore, vigilance concer

Neuropathology of human prion diseases (spongiform encephalopathies).

The human prion diseases (spongiform encephalopathies) comprise Creutzfeldt-Jakob disease (CJD), Gerstmann-Sträussler-Scheinker syndrome (GSS), and kuru. Their clinical characteristics are progressive neurological illness with dementia and ataxia as the most prominent signs. The typical neuropathological changes are limited to the central nervous system; they consist of spongiform degeneration, amyloid plaques, astrocytic gliosis, and nerve cell loss. The aetiology of human spongiform encephalopathies remained unclear for many years, until in the 1960s they were finally recognized as transmiss

Arteriovenous malformations of the brain: recent results of endovascular therapy.

Results of embolization and subsequent surgical resection were reviewed in 38 patients with arteriovenous malformations (AVMs) of the brain during the past 3 years. Advanced techniques were used for embolization, including real-time digital subtraction angiofluoroscopy; directable microcatheters; and a mixture of microfibrillar collagen, polyvinyl alcohol sponge microparticles, and ethanol, in addition to acrylic glue. Embolization caused a permanent deficit in four patients (10.5%) (quadrantanopia [n = 3], cerebellar ataxia [n = 1]). Thirty-two patients (84%) underwent surgery after embolizat

[Human spongiform encephalopathies. Diseases caused by prions].

Spongiform encephalopathies or prion diseases are common denominators for a group of diseases, all fatal, which show characteristic neuropathological changes. In man the group includes four diseases: kuru, Creutzfeldt-Jakob's disease (CJD), Gerstmann-Sträussler-Scheinker's disease (GSS) and fatal familial insomnia (FFI). In animals it comprises the following six: scrapie (sheep and goats), transmissible mink encephalopathy (mink), chronic wasting disease (mule/elk), exotic ungulate encephalopathy (antelopes) and feline and bovine spongiform encephalopathy (cats and cattle). The diseases can be

The influence of PRNP polymorphisms on human prion disease susceptibility: an update.

Two normally occurring polymorphisms of the human PRNP gene, methionine (M)/valine (V) at codon 129 and glutamic acid (E)/lysine (K) at codon 219, can affect the susceptibility to prion diseases. It has long been recognized that 129M/M homozygotes are overrepresented in sporadic Creutzfeldt-Jakob disease (CJD) patients and variant CJD patients, whereas 219E/K heterozygotes are absent in sporadic CJD patients. In addition to these pioneering findings, recent progress in experimental transmission studies and worldwide surveillance of prion diseases have identified novel relationships between the

Epitope scanning indicates structural differences in brain-derived monomeric and aggregated mutant prion proteins related to genetic prion diseases.

Genetic Creutzfeldt-Jakob disease, Gerstmann-Sträussler-Scheinker syndrome, fatal familial insomnia and prion protein cerebral amyloid angiopathy are clinically and neuropathologically distinct neurodegenerative diseases linked to mutations in the PRNP gene encoding the cellular prion protein (PrPC). How sequence variants of PRNP encode the information to specify these disease phenotypes is not known. It is suggested that each mutation produces a misfolded variant of PrPC with specific neurotoxic properties. However, structural studies of recombinant PrP did not detect major differences betwee

Intracerebral distribution of infectious amyloid protein in spongiform encephalopathy.

We studied the regional distribution of infectious amyloid protein by western immunoblots of brain tissue extracts from 37 patients with different forms of spongiform encephalopathy, i.e., 16 sporadic cases, 18 familial cases with a variety of mutations, and 3 iatrogenic cases. In sporadic and familial Creutzfeldt-Jakob disease, amyloid protein concentrations were usually highest in the frontotemporal regions of the cerebral cortex, whereas iatrogenic Creutzfeldt-Jakob disease and Gerstmann-Sträussler-Scheinker syndrome had as high or higher concentrations in the deep cerebral nuclei and cereb

Automatic quantification of amyloid plaque formation in human spongiform encephalopathy.

This study compared a panel of three different prion protein antibodies with conventional plaque staining methods--Congo Red, Periodic acid Schiff and sulphated Alcian blue--to investigate amyloid plaque formation in cases of human spongiform encephalopathy (HSE). Tissue samples were taken from the cerebellum in nine sporadic cases of Creutzfeldt-Jakob Disease, with plaque formation noted on routine histology, and one case of Gerstmann-Straussler Scheinker syndrome. Using image analysis techniques, a semi-automatic system of plaque quantification was devised to measure the relative performance

Immunohistochemical verification of senile and kuru plaques in Creutzfeldt-Jakob disease and the allied disease.

We investigated paraffin-embedded brain sections from 41 patients with Creutzfeldt-Jakob disease (CJD) and from 9 with Gerstmann-Sträussler syndrome (GSS) using anti-human prion protein (PrP) antisera (anti-GSS kuru plaque cores and anti-PrP synthetic peptide) and anti-beta protein antiserum. The anti-human PrP antiserum reacted with the plaques in CJD and GSS, with or without degenerative neurites (neuritic components). In addition, the anti-beta protein antiserum immunolabeled kuru plaque-like compact plaques in some cases of CJD. Therefore, previous morphological classifications of the plaq

Thienyl pyrimidine derivatives with PrP(Sc) oligomer-inducing activity are a promising tool to study prions.

Transmissible spongiform encephalopathies (TSEs), also called prion diseases, are fatal, infectious, genetic or sporadic neurodegenerative disorders of humans and animals. In humans, TSEs are represented by Creutzfeldt-Jakob disease (CJD), Gerstmann-Sträussler-Scheinker syndrome, Fatal Familial Insomnia and Kuru. In animals, the most prominent prion diseases are scrapie of sheep and goats, bovine spongiform encephalopathy (BSE) of cattle and chronic wasting disease (CWD) of deer and elk. A critical event in prion diseases is the accumulation in the central nervous system (CNS) of the abnormall

Chapter 37: alexia and agraphia.

Studies of alexia and agraphia have played important roles in understanding how complex cognitive functions are related to brain structure and activity. Modern interests in brain-behavior relations began during the second half of the 19th century as an outgrowth of flawed correlative studies by neuroanatomist Franz Gall and subsequent clinical-pathological analyses by Jean-Baptiste Boulliaud on speech and the frontal lobes. In 1856, Louis Victor Marcé drew attention to writing disorders and postulated a cerebral faculty for writing. Following Paul Broca's epochal reports on aphemia, many Europ

A chicken monoclonal antibody with specificity for the N-terminal of human prion protein.

Chickens were immunized with human prion protein (PrP) peptide H25 (amino acid residues 25-49) coupled to keyhole limpet hemocyanin. From a fusion experiment using the chicken fusion partner cell line MuH1 and immune spleen cells, one mAb, HUC2-13, was generated which reacted with the peptide. HUC2-13 was specific for a pentapeptide (RPKPG) of the N-terminal of the peptide H25. In Western blotting analysis, the mAb reacted with PrP materials from a human Creutzfeldt-Jakob disease (CJD) case and the membrane fraction from normal murine brain, but not with the same materials pretreated with prot

Molecular genetic studies of Creutzfeldt-Jakob disease.

Genetic study of over 200 cases of Creutzfeldt-Jakob disease (CJD), Gerstmann-Sträussler-Scheinker disease (GSS), fatal familial insomnia (FFI), and kuru have brought a reliable body of evidence that the familial forms of CJD and all known cases of GSS and FFI are linked to germline mutations in the coding region of the PRNP gene on chromosome 20, either point substitutions or expansion of the number of repeat units. No pathogenic mutations have so far been found in sporadic or infectious forms of CJD, although there are features of genetic predisposition in iatrogenic CJD and kuru. In FFI and

[Transmissible and non-transmissible brain amyloidoses: neurodegenerative disorders of different etiologies and the same pathogenesis].

I report here the molecular, ultrastructural and immunohistochemical similarities between transmissible (scrapie, Creutzfeldt-Jakob disease and Gerstmann-Sträussler-Scheinker syndrome) and non-transmissible (Alzheimer's disease, hereditary cerebral hemorrhage with amyloidosis--Dutch (HCHWA-D) and Icelandic (HCHWA-I) types) brain amyloidoses. The central pathogenic event in both types of amyloidoses is a synthesis and a processing of amyloid precursor followed by an accumulation of a final deposit. PrPc and APP are amyloid precursors and PrPsc and beta-A4 are final deposits in transmissible and

Immunohistochemical investigations of the prion protein accumulation in human spongiform encephalopathies. Special report II.

Creutzfeldt-Jakob disease (CJD) in a proportion of cases may have nonspecific clinical signs and symptoms and no characteristic neuroimaging and EEG picture. Thus, neuropathological studies are mandatory for a diagnosis. However, spongiform change, neuronal loss and astrocyte proliferation--the hallmarks of prion diseases, may also be absent or variable. In such cases, the diagnosis should be supported by the detection of prion protein (PrP) by Western blotting or immunohistochemistry (ICC). PrP may not be visualised under "regular" conditions, but it is unmasked following pretreatment procedu

[Epidemiology of human prion diseases].

Prions(proteinaceous infectious particles) are responsible to subacute spongiform encephalopathies(SSE) in man and animals. Recent outbreak of bovine SSE(BSE), or mad cow disease in UK provoked concerns on its possible human hazards. A statement of the British Government in March 1996 upset the world, which was based on 10 cases of "new variant" form of Creutzfelds-Jakob disease(CJD). Prion diseases in animals are often epizootic and may be spread to different species through various routes including ingestion of contaminated meats. It is possible that mad cow disease cause a CJD-like malady i

Successful transmission of Creutzfeldt-Jakob disease from human to mouse verified by prion protein accumulation in mouse brains.

The accumulation of prion protein (PrP) was revealed in the brains of mice inoculated with the brain homogenate from seven patients with Creutzfeldt-Jakob disease (CJD) by immunohistochemistry using hydrolytic autoclaving. It was not found in the brains of mice inoculated with material from either two patients with Gerstmann-Sträussler syndrome or two with other dementing illnesses. PrP accumulation took the forms of diffuse neuropil accumulation in the gray matter and plaque-like accumulation in the white matter and was observed in particular areas in the supratentorial structure. Difficulty differentiating a case of posterior cortical atrophy from a psychogenic disturbance of vision.

Differentiating posterior cortical atrophy (PCA) from other diseases can be difficult and time-consuming, and there is a particularly high possibility of misdiagnosis when psychiatrists diagnose complaints related to visual perception. Here, a case of PCA involving prominent visual perceptual disorders is reported; PCA was difficult to distinguish from psychogenic disturbance of vision in this case. For a year, a 59-year-old woman had had visual perceptual disorders, including a distorted view and prosopagnosia. She underwent examinations at multiple clinical departments at several medical ins

Towards authentic transgenic mouse models of heritable PrP prion diseases.

Attempts to model inherited human prion disorders such as familial Creutzfeldt-Jakob disease (CJD), Gerstmann-Sträussler-Scheinker (GSS) disease, and fatal familial insomnia (FFI) using genetically modified mice have produced disappointing results. We recently demonstrated that transgenic (Tg) mice expressing wild-type bank vole prion protein (BVPrP) containing isoleucine at polymorphic codon 109 develop a spontaneous neurodegenerative disorder that exhibits many of the hallmarks of prion disease. To determine if mutations causing inherited human prion disease alter this phenotype, we generate
