A 74-year-old man was referred to our hospital due to deteriorating level of consciousness and desaturation. His Glasgow Coma Scale was 6, and his pupils were constricted but responded to light. Chest radiograph was negative for significant findings. Arterial blood gas evaluation on supplemental oxygen revealed severe acute on chronic respiratory acidosis: pH 7.15; PCO2, 133 mm Hg; PO2,64 mm Hg; and HCO3, 31 mmol/L. He regained full consciousness (Glasgow Coma Scale, 15) after receiving a 0.4 mg dose of naloxone, but because of persistent severe respiratory acidosis (pH 7.21; PCO2, 105 mm Hg),…

BACKGROUND AND AIMS: Conservative management of posterior fossa epidural hematoma in the pediatric age group has been increasingly considered in the last decade with good clinical outcomes and comparable results to surgical intervention in carefully selected patients. The purpose of this study is to evaluate the outcome of observation in the management of pediatric patients with posterior fossa epidural hematoma (PFEDH) in our tertiary hospital and present a literature review on PFEDH pediatric patients.
METHODS: We conducted a retrospective observational study at King Abdullah University Hos

A prospective observational study collected temperature data from 51 patients in 11 neurosurgical centers and follow-up outcome information at 6 months in 49 patients. Brain temperature (Tbr) was measured directly by an intraventricular temperature sensor. Axillary temperature (Tax) and rectal temperature (Tre) were measured by electric thermometers. Tbr was 0.4 to 1.5°C higher than body temperature. Tre correlated well with the Tbr (coefficient: 0.7378; p < 0.05). Among all patients, Glasgow Coma Scale (GCS) scores on admission were significantly lower in the patients with post-operatively ex…

BACKGROUND: Penetrating head injuries with impacted foreign bodies are rare, associated with a high incidence of morbidity and potentially life-threatening. In this study, we aimed at investigating the outcome of these cases as well as analyzing the factors affecting the prognosis.
METHODS: A retrospective study in which the records of 16 patients who had penetrating head injuries caused by low-velocity impacted foreign bodies were revised. All patients were males with a mean age of 28.9 years (range, 18 to 50 years). The follow-up period ranged from 4 to 13 months with a mean of 8.1 months.

The aim of this study was to evaluate the efficacy of brainstem auditory evoked responses as a prognostic tool in conditions of coma which interfere with cerebral metabolism. Thirteen patients in coma resulting from different systemic disorders (barbiturate intoxication, cardiac arrest, chronic hepatic insufficiency) were included in the study. Brainstem auditory evoked potentials were recorded and patients were graded according to the results. When used in conjunction with other clinical parameters (Glasgow Coma Scale), brainstem auditory evoked potentials provided a more reliable prognostic …

OBJECTIVE: To determine clinical and imaging predictors of post-stroke seizures among adult patients admitted with stroke.
METHODS: The prospective, observational study was conducted from April 2023 to March 2024 at Aga Khan University Hospital, Karachi, and comprised patients aged 18-85 years who were admitted with a diagnosis of stroke. Demographic variables, comorbid conditions, stroke characteristics, and neurological severity scores, as assessed with Glasgow Coma Scale and National Institutes of Health Stroke Scale, were recorded. The relationship between these factors and post-stroke se…

Sedation in neurointensive care is essential for managing patients with acute brain injuries. While sedation is commonly employed to alleviate stress responses and enhance patient comfort, the relationship between sedation practices and clinical outcomes remains unclear. This retrospective cohort study analyzed electronic medical records of patients with intracranial hemorrhage admitted to a tertiary care neurosurgical center from January 2020 to December 2023. Patients with a Glasgow Coma Scale (GCS) ≥ 13, early mortality, or prior treatment elsewhere were excluded. Sedation depth was assesse…

OBJECTIVE: This study aims to enhance prognostic accuracy in severe traumatic brain injury (STBI) by developing a novel nomogram that integrates clinical and paraclinical data.
METHODS: Data from 263 STBI patients were analyzed, focusing on critical variables such as age, Glasgow Coma Scale scores, pupil responsiveness, CT findings, and blood markers. A rigorous regression analysis was conducted to identify significant predictors. The nomogram underwent internal and external validation, and its predictive performance was compared with existing models through a meta-analysis.
RESULTS: The nov…

INTRODUCTION: The measurement of traumatic brain injury (TBI) 'severity' has traditionally been based on the earliest Glasgow Coma Score (GCS) recorded, however, the underlying parenchymal pathology is highly heterogonous. This heterogeneity renders prediction of outcome on an individual patient level inaccurate and makes comparison between patients both in clinical practice and research difficult. The complexity of this heterogeneity has resulted in generic all encompassing 'traumatic brain injury protocols'. Early management and studies of neuro-protectants are often done irrespective of TBI
