Lateral arm free flap for oral tongue reconstruction: an analysis of surgical details, morbidity, and functional and aesthetic outcome.

The purpose of this study was to report the effectiveness of the lateral arm free flap (LAFF) in the reconstruction of oral tongue defects, the subsite in which it may have advantage over the other donor sites. This is a retrospective analysis of 48 consecutive cases of LAFF used for the reconstruction of partial glossectomy defects for squamous cell carcinoma of the oral tongue. Primary defect and donor-site morbidity and the functional and aesthetic outcomes were assessed in 37 evaluable patients, with a minimum of 6 months follow-up. Patient-reported Visual Analog Scale score from 0 (minimu…

[Anatomic study and advantages of the free rectus abdominis flap for head and neck reconstruction].

INTRODUCTION: The main goal of using the free flap is to be able to carry out extensive surgical resection resulting in good functional outc … (flap necrosis and postoperative abdominal hernia). A reconstruction was performed after near total glossectomy. RESULTS: With seven perforators and four cutaneous branches of intercostal nerves, the paraumbilical region is the best area to harvest this flap. The rectus abdominis free flap seems to be adapted for near total glossectomy reconstruction. Previo …

[Amyloid disease and extreme macroglossia. Apropos of a case].

A case of extreme macroglossia was observed in a patient with amyloid disease and dysglobulinaemia. A medial diamond-shaped glossectomy was required to reduce the size of the tongue. The details of the surgical and anaesthetic procedure are presented. This type of surgery, greatly appreciated by the patient, can only be undertaken after careful preparation. The reported cases are rare and the indication for surgery is difficult due to the unpredictable nature of the local and general clinical course of the disease.

Symmetrical lipomatosis of the tongue-a rare cause of macroglossia: diagnosis, surgical treatment, and literature review.

Symmetrical lipomatosis of the tongue is an extremely rare condition. To date, only eight cases have been reported in the literature. We present an extremely rare case of intramuscular lipomatosis of the tongue in a 67-year-old male. The present case is unique because of its infiltrating nature and the extension of the lesion. Glossectomy was performed to reduce the size of the tongue and for diagnosis. The literature is reviewed and clinical characteristics, pathology, and surgical treatment are discussed.

[Enormous lingual lymphangioma].

The authors report a huge lymphangioma of the tongue in a sixty-seven years old female patient. The remarkable progression in dimensions of this lesion, leading to the inevitable protrusion of the tongue, led to the realization of an extended glossectomy, with a functional objective in mind. This simple procedure showed an excellent five years follow-up results. From this case report, the authors stress up on the etiopathogenic, pathological, clinical as well as therapeutic aspects of lingual lymphangioma.

Final Tongue Size in the Treatment of Macroglossia Associated With Beckwith-Wiedemann Syndrome: Review of 29 Cases.

Beckwith-Wiedemann syndrome (BWS) is a rare congenital disorder whose main clinical feature is macroglossia. This frequent manifestation may impair respiratory, feeding, speech, and orofacial developmental functions. When significant functional and/or aesthetic repercussions are evident, surgical excision is the primary management approach, with several techniques described. In this study, we analyzed medical records from 29 patients diagnosed with BWS and macroglossia, and evaluated 26 cases of partial glossectomy performed between 2015 and 2025. Indications, surgical technique, postoperative…

Multi-dimensional analysis of oral cavity and oropharyngeal defects following cancer extirpation surgery, a cadaveric study.

BACKGROUND: Defects following resection of tumors in the head and neck region are complex; more detailed and defect-specific reconstruction … all demonstrated unique dimensional geometry for the various anatomic sites. Using 2D analysis, hemiglossectomy defects revealed right triangle geometry, whereas total glossectomy geometry was a square. Finally, the base of tongue defects exhibited a trapezoid shape. CONCLUSIONS: Customizing the geometry and dimensions of fasciocutaneous free flaps so that t …

A case report of conservative management of extensive proliferative verrucous leukoplakia using a carbon dioxide laser.

OBJECTIVE: The purpose of this report was to describe a clinical case of conservative management of extensive proliferative verrucous leukoplakia (PVL) using a carbon dioxide laser, as an alternative to complete glossectomy, which led to a sustained successful outcome over a 12-year follow-up period. BACKGROUND DATA: Patients with oral PVL, a high risk premalignant condition, are often treated by wide surgical excision. This approach can lead to significant morbidity and compromised quality of life. CASE DESCRIPTION: A 50-year-old male with PVL and superficially invasive carcinoma, who had p…

Machine Learning Prediction of Tongue Pressure in Elderly Patients with Head and Neck Tumor: A Cross-Sectional Study.

Background: This investigation sought to cross validate the predictors of tongue pressure recovery in elderly patients' post-treatment for h … [95% confidence interval (CI): 0.409-0.806]. This model distinctly highlighted the significance of glossectomy (p = 0.039), the presence of functional teeth (p = 0.043), and the patient's age (p = 0.044) as pivotal factors influencing tongue pressure, setting the threshold for statistical significance at p < 0.05. Conclusions: The analysis underscored the cr …

Significant association of Yamamoto-Kohama classification and pathological depth of invasion with cervical lymph node metastasis in early-stage tongue squamous cell carcinoma (Stage I/II).

BACKGROUND/PURPOSE: Tongue squamous cell carcinoma (SCC) has a poor prognosis due to a high rate of cervical lymph node metastasis (CLNM). We aimed to determine clinicopathological features related to the prediction of CLNM in tongue carcinomas (Stage Ⅰ/Ⅱ). MATERIALS AND METHODS: Data from 89 patients with tongue SCC (Stage I/II) were analyzed retrospectively. Patients were treated only with partial glossectomy and not with chemotherapy or radiotherapy until CLNM was observed. No cervical lymph node metastasis survival (NCLNMS) was estimated using the Kaplan-Meier method. The difference in NC…

Orthodontic treatment of a patient with Duchenne muscular dystrophy and macroglossia: how informed consent was critical to success.

This article describes the complex orthodontic treatment of a 22-year-old patient with Duchenne muscular dystrophy and macroglossia. His orthodontic treatment hinged on providing proper informed consent and management of the malocclusion with glossectomy, extractions, fixed appliances, and elastics. Challenges to traditional treatment are outlined, and compromises to both process and outcome are discussed from an informed consent point of view because of the serious risks involved. The treatment objectives were met, and the outcome was considered a success.

Lymphangioma of the tongue with macroglossia: a case report.

Lymphangioma are rare benign lesions of the lymphatic vessels, found mainly in the head and neck. Almost all cases of macroglossia in young children are caused by lymphangioma of the tongue. Aesthetic, occlusal, functional and psychosocial problems may arise as a result of this condition. The preferred treatment is surgery but in some cases sclerosant therapy may be used as an adjunct treatment. We report a case of lymphangioma of the tongue with macroglossia that was treated successfully with a Vshaped anterior glossectomy and discuss the surgical considerations.

Dysphagia following head and neck cancer surgery.

Surgical resection of head and neck cancer results in predictable patterns of dysphagia and aspiration due to disruption of the anatomic structures of swallowing. Common procedures undertaken in the treatment of head and neck cancer include tracheostomy, glossectomy, mandibulectomy, surgery on the palate, total and partial laryngectomy, reconstruction of the pharynx and cervical esophagus, and surgery of the skull base. An overview is presented of normal swallowing physiology, as well as swallowing perturbations that are frequently encountered in postoperative head and neck cancer patients.
