Investigations of the goiter prevalence in children, aged from 0 to 18 years of age living in Lower Silesia were performed. The children were our clinic outpatients in the years 1985-1990. A separate analysis of children from each of the four districts was made because of a different geographical structure. The highest number of goiter was observed in the Wrocław district. Response of the normal and goiterous thyroid gland to the information on the forthcoming operation was studied in 12 patients with varicose veins of the lower limbs and in 67 patients with thyrotoxicosis; this was done by determination of protein-bound iodine (PBI). PBI proved to increase before the operation in both groups of the patients. Due to undesirable hyperfunction of the thyroid gland prior to the operation in thyrotoxicosis, the psychodepressive premedication component should be increased.

This work reports the presence of endemic cretinism in a small district located inside an endemic goiter area in north-eastern Sicily, personally described. The study covers 19 mental defectives (11 females and 8 males, mean age 35.8 +/- 15.5 yr) selected on the basis of severe mental retardation recognized by the local doctors. Long-term L-thyroxine (L-T4) suppression therapy was applied to 133 patients with euthyroid nodular goiter (126 females/7 males) and to 148 patients (136 females/12 males) with post-thyroidectomy recurrence between the years 1980 and 1986. The results are interpreted as "Complete Success" if the nodule disappeared, "Partial Success" if it became smaller, "Late Unresponsiveness" if the nodule first decreased then increased in volume and "Unresponsiveness" when it did not change. Forty-two patients with non-toxic goitre and with a high 10-minute uptake of I(131) were treated with 100 mug. of l-triiodothyronine per day for periods up to 36 months. All of the goitres were reduced in size and 10 showed marked regression within one week. Iodine is the essential constituent of thyroid hormones. Iodine deficiency disorders, ranging from endemic goiter to cretinism, result from low dietary intake of iodine. The fetus and the newborn are more sensitive than adults to a reduced environmental iodine supply, and in iodine-deficient areas, transient neonatal hypothyroidism is frequently observed. This transient thyroid failure may be associated with neonatal goiter. Border-line elevated neonatal TSH levels frequently occur in iodine deficient areas, and result in a higher recalling rate in the screening for congenital hypothyroidism.
