Root coverage: combined surgical procedures.

A number of surgical procedures have therapeutic goals to restore the gingival tissue to the level of the cementoenamel junction with attachment of soft tissue to the previously exposed root surfaces. These procedures are indicated where esthetic needs, root sensitivity, or shallow carious lesions involve the root surfaces. Surgical innovations over the past decade have enabled the periodontist to correct these problems. Case reports of root coverage by combined surgical procedures are described and presented. These reports illustrate greater predictability and improved esthetics compared to o

Mesenchymal stem cells from the oral cavity and their potential value in tissue engineering.

Periodontal disease is one of the most common conditions affecting humans, and current treatment strategies, which focus on the removal and long-term control of dental plaque, are generally successful in eliminating active disease and promoting tissue repair. However, regeneration of the supporting structures of the tooth remains an elusive goal and a challenge. The formation of new bone and cementum with supportive periodontal ligament is the ultimate objective, but current regeneration therapies are incapable of achieving this in a predictable way. The regeneration of periodontal tissue requ

Elucidating the Benefit of Perforated vs Nonperforated Membranes in Guided Bone Regeneration: An In Vivo Histologic Evaluation and Histomorphometric Analysis.

Nonperforated polytetrafluoroethylene (PTFE) membranes are effectively utilized in guided bone regeneration (GBR) but may hinder cell migration due to limited interaction with the periosteum. This study compared bone regeneration using occlusive or perforated membranes combined with acellular collagen sponge (ACS) and recombinant human bone morphogenetic protein-2 (rhBMP-2) in a canine mandibular model. Male Beagle dogs (n = 3) received two mandibular defects each to compare ACS/rhBMP-2 with experimental (perforated group) and control (nonperforated group) membranes (n = 3 defects/group). Tiss

Biomaterials for Periodontal Regeneration.

As a widespread chronical disease, periodontitis progressively destroys tooth-supporting structures (periodontium) and eventually leads to tooth loss. Therefore, regeneration of damaged/lost periodontal tissues has been a major subject in periodontal research. During periodontal tissue regeneration, biomaterials play pivotal roles in improving the outcome of the periodontal therapy. With the advancement of biomaterial science and engineering in recent years, new biomimetic materials and scaffolding fabrication technologies have been proposed for periodontal tissue regeneration. This article su

Tissue engineering of the esophagus.

Esophageal atresia occurs in 1 out of 3000 births. Current treatments involve esophageal replacement by using more distal parts of the gastrointestinal tract, such as the stomach, jejunum, and colon. Significant complications are associated with each treatment option. Tissue engineering may provide a therapeutic alternative for esophageal replacement. This article addresses the progress in esophageal tissue engineering using acellular and cell-seeded approaches. In addition, we discuss the potential direction of future approaches by critically appraising the results in the recent literature.

Tissue engineering in urothelium regeneration.

The development of therapeutic treatments to regenerate urothelium, manufacture tissue equivalents or neourethras for in-vivo application is a significant challenge in the field of tissue engineering. Many studies have focused on urethral defects that, in most cases, inadequately address current therapies. This article reviews the primary tissue engineering strategies aimed at the clinical requirements for urothelium regeneration while concentrating on promising investigations in the use of grafts, cellular preparations, as well as seeded or unseeded natural and synthetic materials. Healing patterns in recession defects treated with ePTFE membranes and with free connective tissue grafts. A histologic and histometric study in the beagle dog.

This study focussed on the biologic success of 2 different procedures for root coverage. In the maxillary canines of 7 beagle dogs, buccal recessions were created and treated on one side with an ePTFE membrane (GTR-group) and on the contralateral side with a free connective tissue graft from the palate (CT-group). 2 areas served as negative controls. After 4 months of healing, histologic sections were processed and histologically analyzed. The evaluated parameters were coverage height, bone, cementum and connective tissue attachment regeneration, length of the epithelium, resorption, and ankyl

Hyaluronic acid: A novel approach in regenerative/reconstructive periodontal therapy?

BACKGROUND: Although hyaluronic acid (HA) has long been used for many medical applications, only in recent years has it gained greater popularity in the field of periodontics because of its biological effects during wound healing. Even today, most clinicians are not aware that more than one type of HA exists and that the extent of its biological functions may vary depending upon the particular characteristics of the biomolecule itself. AIM: To review and synthesize the current preclinical and clinical evidence on the biological effects and therapeutic applications of HA in periodontology, wit

Role of Injectable Platelet-Rich Fibrin in the Management of Soft and Hard Tissue Periodontal Regeneration in Dentistry: Protocol for a Systematic Review.

BACKGROUND: Injectable platelet-rich fibrin (i-PRF) has the capacity to release great amounts of several growth factors, as well as to stimulate increased fibroblast migration and the expression of collagen, transforming growth factor β, and platelet-derived growth factor. Consequently, i-PRF can be used as a bioactive agent to promote periodontal tissue regeneration. OBJECTIVE: We aim to compare and evaluate the effectiveness of i-PRF in periodontal tissue regeneration. METHODS: We will conduct an electronic search in the following databases: PubMed, Cochrane Library, Google Scholar, Semant

In vivo assessment of a nanofibrous silk tube as nerve guide for sciatic nerve regeneration.

A nanofibrous silk nerve conduit has been evaluated for its efficiency based on the promotion of peripheral nerve regeneration in rats. The designed tubes with or without Schwann cells were implanted into a 10 mm gap in the sciatic nerves of the rats. Four months after the surgery, the regenerated nerves were monitored and evaluated by macroscopic assessments and histology. The results demonstrated that the nanofibrous grafts, especially in the presence of Schwann cells, enabled reconstruction of the rat sciatic nerve trunk with a restoration of nerve continuity and formation of nerve fibres w

Periodontal Tissue Regeneration by Transplantation of Autologous Adipose Tissue-Derived Multi-Lineage Progenitor Cells With Carbonate Apatite.

We have developed an autologous transplantation method using adipose tissue-derived multi-lineage progenitor cells (ADMPCs) as a method of periodontal tissue regeneration that can be adapted to severe periodontal disease. Our previous clinical study confirmed the safety of autologous transplantation of ADMPCs and demonstrated its usefulness in the treatment of severe periodontal disease. However, in the same clinical study, we found that the fibrin gel used as the scaffold material might have caused gingival recession and impaired tissue regeneration in some patients. Carbonate apatite has a h

Nerve growth factor combined with an epineural conduit for bridging a short nerve gap (10 mm). A study in rabbits.

The purpose of this study was to evaluate the effect of direct administration of nerve growth factor (NGF) into an epineural conduit across a short nerve gap (10 mm) in a rabbit sciatic nerve model. The animals were divided into two groups. In group 1, n = 6, a 10-mm defect was created in the sciatic nerve and bridged with an epineural flap. A dose of 1 μg of NGF was locally administered daily for the first 21 days. NGF administration was made inside the epineural flap using a silicone reservoir connected to a silicone tube. In group 2, n = 6, the 10-mm defect was bridged with a nerve graft. T

Mechanobiologically-optimized non-resorbable artificial bone for patient-matched scaffold-guided bone regeneration.

Scaffold-guided bone regeneration is poised to revolutionize the management of critical-sized bone defects. However, translation into clinical practice has been hampered by the focus on bioresorbable scaffolds where the rate of degradation needs to match the rate of bone formation and metal plates are required to overcome their mechanical limitations. Metal plates are problematic because they cause stress shielding and X-ray perturbation, increasing the likelihood of hardware failure and interfering with post-operative radiotherapy and imaging. Segmental defects of the mandible are challenging

Novel 3-D helix-flexible nerve guide conduits repair nerve defects.

Flexible nerve guide conduits (NGCs) are desired in peripheral nerve reconstruction near the joints. Inspired by the engineered structure of the helical tube, we addressed the problems of conventional NGCs often mentioned in clinical feedback. We developed a type of helix-flexible NGC (HF-NGC) and evaluated its mechanical properties as well as its flexibility, and it performed excellently during in vitro tests. During the in vivo tests, HF-NGCs and conventional nonflexible NGCs (NF-NGCs) were implanted in a rat sciatic nerve defect model. A short-term investigation showed that Schwan cells (SC
