Successful clinical transplantation of the upper extremity has been performed in several centers. In contrast with the recipients of other immediately vascularized organ allografts, the candidate for upper extremity transplantation has been, at least during the initial effort, a healthy patient. Although it is clear that composite tissue transplantation (CTA) is a form of allografting that behaves in many ways similarly to immediately vascularized organ allografts, the issue of developing immunologic understanding of such new allografts, awaits greater clinical experience. A summary of the imm

The term immunosenescence refers to the collective effects of aging on the immune system, which involves both the innate and the adaptive immunity and plays a dominant role in patients' morbidity and mortality. Senescent T lymphocytes are in a state of replicative arrest and have several unique features: they typically lack costimulatory molecules, they express shortened telomeres, and they have the capacity to produce large amounts of proinflammatory cytokines. The B-lymphocyte compartment and the cells of the innate immune system also demonstrate important changes due to aging. The immunosen

Scar contracture deformity, which can lead to dysfunction of hand and low quality of life, is one of the common complication after hand burns. The prevention measures of scar contracture after hand burns include large skin grafting, prevention of infection, insistence on wearing pressure gloves, use of silicone sheets, wearing orthosis, accepting proper physical therapy, and early functional exercise. The primary treatments of postburn contractures of the hand are surgery, drugs, laser treatment, and rehabilitation therapy. Excision of scars, release of muscle, joints or bones, and soft tissue

BACKGROUND: Early reconstruction of burn sequelae of the hand can be challenging owing to high goals for functional and aesthetic outcome. A variety of reconstructive procedures with ascending levels of complexity exists and warrants careful indication.
METHODS: In this case series, the main reconstructive techniques for reconstruction of burn defects of the hand are described, illustrated, and discussed: split thickness skin grafting (STSG) with fibrin glue, dermal matrices with STSG, distant random pattern (abdominal bridge) flap, distant pedicled flap (superficial circumflex iliac artery f

The concept of including external oblique aponeurosis along with the time-tested groin flap as a single vascularized unit heralds a versatile and unique application in the armamentarium presently available. This concept, though simple, has not been previously reported in the world literature. In view of its potential benefits and simplicity, we felt the need to bring this concept to the attention of our colleagues.

Correct acute care is critical in the treatment of burns of the hand, particularly in the younger patient in order to avoid complications that may interfere with the patient's quality of life and ability to be self-sufficient and self-supportive. Objectives of management are to prevent edema, prevent prolonged immobilization and poor position and prevent infection and preserve viable tissue. The various modalities that are used to achieve these objectives and how they may be individualized to meet the patient's specific needs are described and discussed.

The unquestionable clinical success of left ventricular assist device (LVAD) implantation has, nevertheless, been accompanied by complications arising from interactions between the implanted biomaterial and the host immune system. The aberrant state of monocyte and T-cell activation resulting from these host/device interactions is accompanied by two parallel processes: (1) selective loss of Th1 cytokine producing CD4 T-cells through activation-induced cell death; and (2) unopposed activation of Th2 cytokine producing CD4 T-cells resulting in B-cell hyperreactivity and dysregulated immunoglobul

A few severe hand injuries represent a gray zone for the reconstructive surgeon, for which autologous tissue transfer and allotransplantation can yield satisfactory results, but not without downsides or morbidity. This article is written to address the dilemma of application of both techniques in those selected cases.

A case of heterotopic replantation of the metacarpus is demonstrated. This procedure shows the successful primary reconstruction of a severely injured hand. The result seventeen months after the injury is shown. The patient is rehabilitated and is able to manage basic activities of daily living.

Enchondromas are benign bone tumours originating from cartilages. It is mainly discovered incidentally in radiographs or due to symptoms like pathological fracture or pain. Conservative treatment through regular check-up and surgical excision using curettage are the two major treatment methods for enchondromas. This review concludes that small localized asymptomatic lesions can be treated conservatively while most expanding or symptomatic lesions should be treated with simple curettage. Adjuvant treatments like high-speed burring or alcohol instillation are not recommended.

PURPOSE: The authors present an anatomical study of the flexor digitorum superficialis synovial flap and its clinical application for palmar soft tissue defects in hand trauma.
METHODS: Green latex was injected into the brachial artery of thirty-one human upper limbs from corpses donated to science. Anatomical features of this pedicled flap were assessed: arterial vascularization, dimensions and covering surface.
RESULTS: The anatomical support of this flap is the synovial tissue of the flexor tendons, and particularly the superficial layer of the synovial system, covering the flexor digitor

Over several years, the author has devised different techniques for difficult bony reconstruction in hand surgery. It is based on compound island transfers of ulnar origin, harvested from the forearm's lower third with reverses flow vascularization. Because of the variety of arterial ramifications, these transplants can consist, either of a simple bone ulnar fragment or when associated, of compound cutaneous-osseous, or even cutaneous-tendineous-osseous transplants. These transplants based on a clinical experience of 49 cases can be used by retrograde rotation for a variety of indications such

The management of intractable onychomycosis involving multiple fingernails continues to be optimized. Failure of pharmacological treatment necessitates operative intervention. Current surgical procedures are complex and have frequent donor site complications. This report describes a safe, simple, and effective surgical treatment that eradicates the disease, results in low morbidity, and yields high patient satisfaction.

A 54-year-old alcoholic was admitted with keratomalacia in 2001. The right cornea had an ulceration and the left cornea had stromal necrosis and a perforation. Serum retinol and serum betacaroten were extremely low. Treatment consisted of high-dose retinol and left sided cornea transplantation. The right eye healed with little scarring, vision: 0.5. The left eye was stabilized after a second cornea transplantation and covering with conjunctiva, vision: hand movement at a distance of 1 m. This proves the existence of symptomatic vitamin A deficiency among socially exposed subjects in Denmark.

BACKGROUND AND OBJECTIVES: Historically, nephrectomy for autosomal dominant polycystic kidney disease was performed by an open technique. We performed this study to compare outcomes in hand-assisted laparoscopic nephrectomy with open nephrectomy in this population.
METHODS: Charts of patients with autosomal dominant polycystic kidney disease who underwent nephrectomy by a transplant surgeon from January 1, 2000, to December 31, 2011, were reviewed. The hand-assisted laparoscopic nephrectomy group was compared with the open group. Data collected included unilateral versus bilateral nephrectomy

BACKGROUND: Complex defects of the forearm and hand are associated with the loss of important structures. Single-stage reconstruction of these defects requires composite tissue transplantations. The subscapular region offers various components for the coverage of complex defects. The scapular and the parascapular flaps can be used each as cutaneous, fasciocutaneous and osteocutaneous or as a combined flap as well. The purpose of this study was to present our experience with the combined scapular-parascapular free flap for defect coverage of the forearm and hand in 13 patients.

In burn injuries due to their exposed situation the hands are frequently injured and in the moment of the accident they are often used as a shield in front of the face. Mainly the back of the hands are involved. In reconstruction after burns of the back of the hands the scared skin is excised under tourniquet, the displaced interdigital webs are corrected by the method of Tanzer and the skin defect is covered by a thick split-skingraft. From case to case additional operations are: replacement of lost extensor tendons, capsulotomies of the MP-joints, arthrodeses of destroid fingerjoints and amp
