Navigating spatial barriers in healthcare: health equity and end-of-life care infrastructure in South Korea and China.

INTRODUCTION: East Asia is experiencing an unprecedented demographic transformation, with rapidly aging populations creating an urgent public health demand for accessible hospice and palliative care facilities. However, infrastructural planning regulations may implicitly marginalize these essential health services as stigmatized facilities, creating regulatory and institutional conditions that can constrain community-adjacent access to end-of-life care. METHODS: This study conducts a structured comparative legal and health policy analysis of facility-siting frameworks in South Korea and China

Documenting best practices for maintaining access to HIV prevention, care and treatment in an era of shifting immigration policy and discourse.

Changes in the United States federal-level political landscape have been felt within immigrant communities, and the public health clinics that serve them. We sought to document how HIV prevention and care clinics are reaching and retaining their immigrant community patients during a period of retrenchment of accessible public resources and immigrant rights. From May 2018 through January 2019, we conducted 20 in-depth interviews with clinicians, case workers, advocates, legal experts, and peer navigators in Northern and Central California. Interviews were recorded and transcribed. Several theme

Pandemic Preparedness: Analyzing National Plans for Respiratory Pathogen Pandemics in the Americas Region.

BACKGROUND: To identify gaps and define priority actions to strengthen national pandemic preparedness and response plans, we assessed the concordance between national plans for respiratory pathogens against the World Health Organization (WHO) pandemic checklist and the States Parties Annual Report (SPAR) in the Americas. METHODS: In this retrospective, semiquantitative study, we reviewed the most recent respiratory pandemic plans for 35 Pan American Health Organization (PAHO) member states and assessed their concordance with (1) actionable guidelines in WHO pandemic checklist and (2) Internat

Random or predictable?: Adoption patterns of chronic care management practices in physician organizations.

BACKGROUND: Theories, models, and frameworks used by implementation science, including Diffusion of Innovations, tend to focus on the adoption of one innovation, when often organizations may be facing multiple simultaneous adoption decisions. For instance, despite evidence that care management practices (CMPs) are helpful in managing chronic illness, there is still uneven adoption by physician organizations. This exploratory paper leverages this natural variation in uptake to describe inter-organizational patterns in adoption of CMPs and to better understand how adoption choices may be related

Preconception Care in Publicly Funded U.S. Clinics That Provide Family Planning Services.

INTRODUCTION: Federal recommendations for providing quality family planning services were published in 2014 and included preconception care (PCC). This paper aims to describe the prevalence of PCC delivery among publicly funded clinics, prior to the recommendations. METHODS: Prevalence of providing occasional or frequent PCC in the last 3 months and having written protocols for recommended PCC screenings were estimated in 2015 using survey data collected from a nationally representative sample of publicly funded clinic administrators (2013-2014, N=1,615). Analyses included examination of diff

Evaluating health workers' potential resistance to new interventions: a role for discrete choice experiments.

BACKGROUND: The currently recommended approach for preventing malaria in pregnancy (MiP), intermittent preventive treatment with sulphadoxine-pyrimethamine (SP-IPT), has been questioned due to the spread of resistance to SP. Whilst trials are underway to test the efficacy of future alternative approaches, it is important to start exploring the feasibility of their implementation. METHODS AND FINDINGS: This study uses a discrete choice experiment (DCE) method to assess the potential resistance of health workers to changing strategies for control of MiP. In Ashanti region in Ghana, 133 antenata

Implications of CATIE for mental health services researchers.

The authors discuss the implications of the Clinical Antipsychotic Trials of Intervention Effectiveness (CATIE) for mental health services researchers, in particular the need to monitor and change prescriber behavior to encourage informed medication selection. Given the complexity and variability of response to antipsychotic medications in CATIE, use of restricted formularies is not the answer. Rather, services researchers should collaborate with service systems to develop interventions to identify questionable prescriber practices and develop interventions to change them. The CATIE results al

Including patient experiences from online sources in guidelines: A natural language processing study on scabies.

OBJECTIVE: Including patients' experience-based knowledge in the development of clinical and public health guidelines has been shown to enhance the quality, relevance, and applicability of guidelines. However, the meaningful and methodologically sound inclusion of patient experiences remains a challenge. This study aimed to showcase the potential of NLP methods as an innovative tool for guideline development to gain insights into patients' experiential knowledge and to incorporate this into the guideline development process. METHODS: For the revision of the Dutch public health guideline for s

Human Immunodeficiency Virus, Other Sexually Transmitted Infections, and Sexual and Reproductive Health in Lesbian, Gay, Bisexual, Transgender Youth.

Lesbian, gay, bisexual, transgender (LGBT), and questioning youth represent a diverse population who are affected by many sexual health inequities, including increased risk for human immunodeficiency virus (HIV) and sexually transmitted infections (STIs). To provide comprehensive sexual health care for LGBT youth, providers should set the stage with a nonjudgmental, respectful tone. Providers should be competent in recognizing symptoms of STIs and HIV and aware of the most up-to-date screening guidelines for LGBT youth. Sexual health visits should also focus on prevention, including safer sex

Developing and sustaining inclusive language in biomedical informatics communications: an AMIA Board of Directors endorsed paper on the Inclusive Language and Context Style Guidelines.

OBJECTIVES: In 2023, AMIA's Inclusive Language and Context Style Guidelines (the "Guidelines") were approved by the Board of Directors and made a publicly available resource. This work began in 2021 through AMIA's DEI Task Force and subsequent DEI Committee; many members provided input, feedback, and time to create the Guidelines. In this paper, the authors provide a transparent account of the origin, development, contents, and dissemination of the Guidelines and share plans for their future development and use. MATERIALS AND METHODS: Our approach to drafting, refining, and distributing the G

Differences in reproductive health discussions in an urban Hispanic population with SLE: lessons from the field.

OBJECTIVE: Management of reproductive health-related issues is crucial for patients with SLE, given this is a disease that primarily affects women of childbearing age. Little is known as to how the 2020 American College of Rheumatology (ACR) Reproductive Health in Rheumatic Disease Guideline is experienced by an underserved, primarily Hispanic population and their physicians as it relates to pregnancy planning and contraception conversations. Given this population experiences high rates of unplanned pregnancies and worse SLE outcomes compared with the non-Hispanic white population, it is cruci

Setting up a Nuchal Translucency Clinic: What Radiologists Need to Know.

The purpose of this article was to discuss the process of setting up a nuchal translucency (NT) screening clinic in clinical practice, how to interpret the information in combination with other clinical tests, what to do if abnormal results are obtained, and to illustrate some of the fetal anomalies that are associated with an increased NT. The NT was initially implemented to predict the likelihood of a fetus with Down syndrome. Maternal age can be combined with fetal NT and maternal serum biochemistry (free β-hCG and PAPP-A) at 11 to 14 weeks to identify about 90% of affected fetuses. Setting

Appropriate use of estrogen replacement therapy in adolescents and young adults with Turner syndrome and hypopituitarism in light of the Women's Health Initiative.

Adolescents and young adult females who are hypoestrogenic need gonadal hormone therapy for sexual development, enhancement of growth, and maintenance of reproductive tissues, cyclic menses, and psychosocial health. In addition, prevention of chronic disease, specifically bone loss and possibly early heart disease, needs to be addressed in these patients. The etiology of hypopituitarism should also be considered when evaluating therapeutic options. The issues concerning estrogen replacement therapy (ERT), including the doses used and the length of therapy, are different for young patients than

A flexible national living donor kidney exchange program taking advantage of a central histocompatibility laboratory: the Dutch model.

The shortage of deceased donor kidneys for transplantation has resulted in the expansion of living donation programs. A number of possibilities have been explored, since it became clear that donors do not need to be genetically related to their recipients. Apart from classical direct donation, other options such as paired exchange, list exchange, altruistic donation and domino paired exchange programs have been implemented. In the Netherlands, patients who cannot be transplanted with their potential living donor because of ABO blood group incompatibility or a positive crossmatch, have the opti
