Hearing loss and motorcyclists.

Motorcyclists are known to be exposed to excessive wind noise levels when riding. The potential adverse effects of this exposure on their hearing was investigated. Temporary threshold shift (TTS) was assessed by asking 18 riders to undertake a standard test run of one hour at a steady 80 mph, and performing audiometry before and immediately afterwards. Permanent threshold shift (PTS) was assessed by performing pure-tone audiograms on a highly screened group of 246 motorcyclists and comparing their hearing thresholds with those of an appropriate control group obtained from the MRC National Stud…

Genetic diseases of hearing.

The study of genetic hearing loss has always been hindered by the difficulty of separating sporadic genetic cases from cases affected by environmental influences. This is because audiologic methods are largely insensitive to causes, and the anatomical location of the affected structures in the middle and inner ear makes histopathologic verifications difficult. With recent progress in genetic investigation, eg, the mapping of the gene location for the Waardenburg and Usher syndromes, both syndromic and nonsyndromic genotypes have been elucidated in humans and in animal models. In the future, mo…

[Hearing disorders in childhood].

Hearing impairment in children may severely affect language acquisition. Therefore, early identification and expedient auditory rehabilitation is important. Hearing impairment should be diagnosed within the first year of life. Unfortunately, hearing handicap is still recognized at an average age of two years. In 70% of the cases, the hearing impairment is first suspected by the parents. The hearing of children at risk should be assessed using systematic screening programs. Parents who suspect that their child is hearing impaired must be taken seriously, and a thorough pedaudiologic evaluation …

Hearing loss in diabetics.

The relationship between diabetes and sensorineural hearing loss has been disputed. This study compares 44 insulin-dependent diabetics with 38 age and sex matched controls. All had pure tone and speech audiometry performed, with any diabetic showing sensorineural deafness undergoing stapedial reflex decay tests. In 14 diabetics stapedial reflex tests showed no tone decay in any patient, but seven showed evidence of recruitment. Analysis of variance showed the diabetics to be significantly deafer than the control population. The hearing loss affected high frequencies in both sexes, but also low…

Hearing aids and otosclerosis.

The purpose of this article on the use of hearing aids in patients with otosclerosis is to emphasize that all otosclerosis patients cannot be assumed to be good users of hearing aid amplification. Patients with losses of 60 dB or less and with purely conductive lesions may be able to use aids well. Individuals with mixed-type impairments due to otosclerosis may have significant problems utilizing amplification. These cases demonstrate the use of hearing aid amplification in patients with mixed-type hearing impairment due to otosclerosis. This is a very different group of patients from the grou…

Hearing and visual impairments.

Hearing impairments, with their resultant difficulties in communication function, represent a common chronic condition affecting one third of aged individuals. Loss of hearing ability pervades multiple domains of function in aged individuals, decreasing activity, increasing depressive symptoms, and confounding assessment of cognitive ability. Although they are not curable, hearing impairments are often remediable with appropriate audiologic evaluation and the prescription of amplification devices. The role of the geriatrician in managing hearing impairment is to recognize its presence, provide…

Longitudinal study of hearing.

Our knowledge of the progression and aetiology of hearing impairments is mainly inferred from cross-sectional studies of populations or individual case studies of relatively rare conditions. Longitudinal studies of carefully stratified samples enable scientific analysis of these two aspects of the ageing auditory system and also provide much needed incidence data on the basis of which to plan comprehensive hearing services. This preliminary paper using data from two studies over relatively short periods (between 2-4.5 years in Great Britain (GB) and up to 8 years in Denmark (DK] confirms (a) t…

Multi-programmable hearing aid.

A basic problem in hearing-aid fitting is the difficulty in finding one setting optimal to all listening situations that might occur. The objective was to develop a behind-the-ear hearing-aid with a very flexible analog signal processor which is digitally controlled, and a memory with logic, so that the hearing-impaired person can select from eight completely different fittings. To program and adjust this multi-programmable hearing-aid (called MemoryMate) a hearing evaluation and recommendation system (called Master-Fit) has been developed, based on an IBM PS/2 computer. This system offers the…

Treatable sensorineural hearing loss.

Sensorineural hearing loss is generally felt to be an untreatable medical condition. However, in some cases, prompt diagnosis and treatment of the underlying condition may reverse the deafness. This article summarizes various treatable forms of sensorineural hearing loss and provides illustrative cases histories of patients who have had sensorineural hearing losses that were improved by medical or surgical intervention. Patients with reversible sensorineural deafness due to inadvertent aminoglycoside over-dosage, congenital cholesteatoma, Meniere's syndrome, blood coagulopathy, and perilymphat…

[Objective hearing aid fitting].

In many cases speech and pure tone audiometry are not practicable. Therefore, the hearing aid fitting is based on objective techniques, like the measurement of the stapedius-reflex threshold or acoustically evoked neural responses. In particular, brain stem responses are very helpful for an objective hearing-aid fitting. They represent the full scale of hearing dynamics and supply direct information for selection and adjustment of the hearing aid. Control of the fitting can also be effected directly by measuring brain stem responses.

Idiopathic sudden hearing loss.

Idiopathic sudden hearing loss is an otologic emergency with an obscure cause. It is the acute onset of hearing loss of 30 cB in three contiguous frequencies, which may occur instantaneously or progressively over several days. Suspect etiologies include viral infections, cochlear artery occlusion, rupture of the intracochlear membranes, and biochemical alterations in the metabolism maintaining the endocochlear membrane potentials. Histopathology of the temporal bone most commonly reveals atrophy of the organ of Corti and striae vascularis. Treatments proposed include close observation for spon…

Hereditary disturbance of hearing.

As 40-60% of all heredity disturbances are of genetic origin, it is necessary to apply complex human genetic methods in diagnosis and prevention. Formerly clinicogenetic examinations in most cases only implied the analyses of hereditary constitution. An observation as demonstrated through the examples of several kinships with hereditary disturbances of hearing may serve as a model of complex clinicogenetic investigations. In all cases the diagnosis of hereditary disturbances of hearing must be followed by human genetic family advice.

Unexplained conductive hearing loss.

We report a series of unusual clinical cases which exhibit what we have referred to as unexplained conductive hearing loss. Audiometrically, these cases typically display mild to moderate conductive or mixed-type hearing loss and good word discrimination (80% to 100%). The otologic evaluations also lend support to the audiometric findings. When polytome roentgenograms were obtained thr results showed a normal inner ear system. Most often, the otologists' clinical impression was a fixed malleus or congenital stapes fixation, and exploratory tympanotomy with inspection of the ossicular chain was…
