INTRODUCTION AND OBJECTIVES: Beta-blockers, angiotensin-converting enzyme inhibitors (ACE inhibitors), angiotensin-II-receptor-blockers (ARB), and mineralocorticoid-receptor antagonists decrease mortality and heart failure (HF) hospitalizations in HF patients with reduced left ventricular ejection fraction. The effect is dose-dependent. Careful titration is recommended. However, suboptimal doses are common in clinical practice. This study aimed to compare the safety and efficacy of dose titration of the aforementioned drugs by HF nurses vs HF cardiologists.

BACKGROUND: Knowledge on the association between heart failure (HF) etiologies, precipitant causes and clinical outcomes may help in ascertaining patient's risk and in selecting tailored therapeutic strategies.
METHODS: The prognostic value of both HF etiologies and precipitants for worsening HF were analyzed using the index cohort of BIOSTAT-CHF. The studied HF etiologies were: a) ischemic HF; b) dilated cardiomyopathy; c) hypertensive HF; d) valvular HF; and e) other/unknown. The precipitating factors for worsening HF were: a) atrial fibrillation; b) non-adherence; c) renal failure; d) acut…

The frequency of heart failure and hyperuricemia is increasing worldwide. Hyperuricemia as a complication of heart failure is known to be an indicator of poor prognosis. However, no consensus exists on whether treating hyperuricemia as a complication of heart failure would alleviate symptoms or improve prognosis in patients with heart failure. There is insufficient evidence regarding whether treatment with uric acid-lowering agents can alleviate symptoms and improve prognosis in patients with heart failure. However, considering the prevention of gout, hypertension, and chronic kidney disease c…

Heart failure (HF) is a major contributor to morbidity and mortality in the US and worldwide. HF is a complex condition characterized by the disruption of normal physiology and the activation of neurohumoral pathways, including the renin-angiotensin-aldosterone system, the sympathetic system, and inflammatory pathways. These have adverse effects on renal handling of salt and water balance, leading to salt and water retention and a vicious cycle of worsening congestive changes with progressive volume overload. Meanwhile, diuretics are pharmacologic agents that are essential in the management of…

Heart failure (HF) comorbidities are extremely common and complicate management due to increased prescriptions and adverse drug effects, declining functional status, and more frequent health care utilization. Comorbidities are associated with poor outcomes in these patients, with worsened mortality, hospitalization, and quality of life. Clinicians should understand how to address common HF comorbidities to provide optimal care of patients with HF and avoid adverse outcomes.

Management of ischemic cardiomyopathy is challenging, especially when treatment benefits do not clearly exceed the estimated risk of procedures. Myocardial viability assessment provides additional data in this setting to anticipate potential functional recovery and possibly prognostic improvement following revascularization. This evidence comes from the positive signals received from posthoc analyses of some clinical trials. There are multiple clinically available modalities to evaluate myocardial viability among which more sensitive ones such as cardiac magnetic resonance and PET are more pre

Heart failure (HF) is a condition characterized by high morbidity, mortality, and a substantial healthcare burden, in which inflammation plays a pivotal role. This review provides a comprehensive overview of inflammation in HF progression, highlighting the dynamic alterations in immune cell populations-such as monocytes/macrophages and neutrophils-and regulatory mechanisms of key signaling pathways, including JAK and NLRP3. Furthermore, the clinical relevance of inflammatory biomarkers in predicting disease prognosis is also discussed. Emerging evidence indicates that exercise intervention can…

PURPOSE OF REVIEW: Heart failure and chronic kidney disease are frequently comorbid, with both characterized by salt and water retention that lead to clinical deterioration. Peritoneal dialysis can effectively treat fluid overload and maintain euvolemia but remains underutilized. This review addresses the use of peritoneal dialysis in heart failure, including its potential advantages and also its challenges.
RECENT FINDINGS: The 2019 Advancing American Kidney Health initiative incentivized increased use of peritoneal dialysis in the United States. There is, therefore, growing recognition of p…

Heart failure (HF) is a complex clinical syndrome resulting from impaired myocardial function or structure, affecting approximately 56 million patients worldwide. Cardiometabolic risk factors, including hypertension, insulin resistance, obesity, and dyslipidemia play a pivotal role in both the pathogenesis and progression of HF. These risk factors frequently coexist as part of cardiometabolic syndrome and contribute to widespread organ and vascular dysfunction, leading to conditions such as coronary artery disease, chronic kidney disease, type 2 diabetes mellitus, non-alcoholic fatty liver dis…

Heart failure (HF) is a heterogeneous clinical syndrome marked by substantial morbidity and mortality. The natural history of HF is well established; however, epidemiological data are continually evolving owing to demographic shifts, advances in treatment and variations in access to health care. Although the incidence of HF has stabilized or declined in high-income countries over the past decade, its prevalence continues to increase, driven by an ageing population, an increase in risk factors, the effectiveness of novel therapies and improved survival. This rise in prevalence is increasingly n…

In the last years, major progress has occurred in heart failure (HF) management. The 2023 ESC focused update of the 2021 HF guidelines introduced new key recommendations based on the results of the last years of science. First, two drugs, sodium-glucose co-transporter-2 (SGLT2) inhibitors and finerenone, a novel nonsteroidal, selective mineralocorticoid receptor antagonist (MRA), are recommended for the prevention of HF in patients with diabetic chronic kidney disease (CKD). Second, SGLT2 inhibitors are now recommended for the treatment of HF across the entire left ventricular ejection fractio…

A dominant event determining the course of heart failure (HF) includes the disruption of the delicate sodium (Na+) and water balance leading to (Na+) and water retention and edema formation. Although incomplete decongestion adversely affects outcomes, it is unknown whether interventions directly targeting (Na+), such as strict dietary (Na+) restriction, intravenous hypertonic saline, and diuretics, reverse this effect. As a result, it is imperative to implement (Na+)-targeting interventions in selected HF patients with established congestion on top of quadruple therapy with angiotensin recepto…

PURPOSE OF REVIEW: Diuretics are the cornerstone therapy for acute heart failure (HF) and congestion. Patients chronically exposed to loop diuretics may develop diuretic resistance as a consequence of nephron remodelling, and the combination of diuretics will be necessary to improve diuretic response and achieve decongestion. This review integrates data from recent research and offers a practical approach to current pharmacologic therapies to manage congestion in HF with a focus on combinational therapy.
RECENT FINDINGS: Until recently, combined diuretic treatment was based on observational s…

Fluid retention is a major determinant of symptoms in patients with heart failure (HF), and it is closely associated with prognosis. Hence, congestion represents a critical therapeutic target in this clinical setting. The first therapeutic strategy in HF patients with fluid overload is optimization of diuretic intervention to maximize water and sodium excretion. When diuretic therapy fails to relieve congestion, renal replacement therapy represents the only alternative option for fluid removal, as well as a way to restore diuretic responsiveness. On this background, the pathophysiology of flui…

Dyspnea and Right Heart Failure Abstract. Acute right ventricular failure is a critical condition diagnosed by clinical presentation combined with echocardiography. Additional diagnostic tools including laboratory, ECG, right heart catheterization, and other imaging modalities are needed to confirm the diagnosis and determine the cause. The identification and treatment of the underlying pathology, the reduction of right ventricular afterload (if possible), optimization of preload (often diuretics, rarely volume), and hemodynamic support using vasopressors and/or inodilators are mainstays of tr…

In patients with heart failure (HF), the neuroendocrine systems of the sympathetic nervous system (SNS), the renin-angiotensin-aldosterone system (RAAS) and the arginine vasopressin (AVP) system, are activated to various degrees producing often-observed tachycardia and concomitant increased systemic vascular resistance. Furthermore, sustained neurohormonal activation plays a key role in the progression of HF and may be responsible for the pathogenetic mechanisms leading to the perpetuation of the pathophysiology and worsening of the HF signs and symptoms. There are biomarkers of activation of …

Simultaneous initiation of quadruple therapy with angiotensin receptor-neprilysin inhibitor, beta-adrenergic receptor blocker, mineralocorticoid receptor antagonist, and sodium glucose cotransporter 2 inhibitor aims at prompt improvement and prevention of readmission in patients hospitalized for heart failure with reduced ejection fraction. However, titration of quadruple therapy is time consuming. Lengthy up-titration of quadruple therapy may negate the benefit of early initiation. Quadruple therapy should start with a sodium glucose cotransporter 2 inhibition and a mineralocorticoid antagoni…
