Heartburn: cardiac potassium channels involved in parietal cell acid secretion.

Hydrochloric acid is produced in parietal cells of the gastric glands by an H(+)/K(+)-ATPase. This proton pump couples the outwards movement of H(+) to the inwards movement of K(+) thus requiring the presence of luminal K(+) to operate. To maintain the activity of the pump, K(+) recirculates over the apical membrane via conductive pathways, the molecular nature of which has been identified in the past few years. This review gives a short overview about the recent advances in the understanding of the role of K(+) channels in the process of parietal cell H(+) secretion and focuses on the identif

Sleep issues in gastroesophageal reflux disease: beyond simple heartburn control.

There is a burgeoning interest in the relationship between sleep-related gastroesophageal reflux (GER) and the development of esophageal and extra esophageal complications. The physiological changes associated with sleep, such as suspension of the regulation of body temperature, may influence nocturnal GER and esophageal acid clearance. Data indicate that sleep induces considerable risk of prolonged acid mucosal contact and facilitates the occurrence of proximal migration of acid, thereby increasing the probability of pulmonary aspiration. Nocturnal GER can lead to the development of esophagit

Review of recent evidence on the management of heartburn in pregnant and breastfeeding women.

Gastroesophageal reflux disease (GERD) is one the most common medical complaints in pregnant women. Some women continue to experience GERD symptoms after delivery. Effective management of GERD symptoms is important to improve productivity and quality of life. Management of heartburn in pregnant and breastfeeding women involves lifestyle modifications, dietary modifications, non-pharmaceutical remedies and pharmaceutical drugs. For most patients, lifestyle/dietary modifications are helpful in reducing GERD symptoms. For patients who require a more intense intervention, various types of pharmace…

Heartburn as a Marker of the Success of Acid Suppression Therapy in Chronic Cough.

PURPOSE: Gastro-oesophageal reflux disease (GORD) is commonly thought to play an important role in chronic cough and patients are often empirically treated with acid suppression therapy. We sought to investigate the response rate to acid suppression treatment in patients with and without heartburn attending two specialist cough clinics. METHODS: A retrospective review of 558 consecutive patients referred to two specialist cough clinics was performed (UK and USA). Patients who were treated with acid suppression were included and their documented response to treatment was collected. Binary logi…

Esophageal acid sensitivity and mucosal integrity in patients with functional heartburn.

BACKGROUND: Patients with functional heartburn (FH) experience troublesome heartburn that is not related to gastroesophageal reflux. The etiology of the heartburn sensation in FH patients is unknown. In patients with reflux disease, esophageal hypersensitivity seems associated with impaired mucosal integrity. We aimed to determine esophageal sensitivity and mucosal integrity in FH and non-erosive reflux disease (NERD) patients. METHODS: In this prospective experimental study, we performed an acid perfusion test and upper endoscopy with biopsies in 12 patients with NERD and nine patients with …

An 11-year-old male patient with refractory asthma and heartburn.

Achalasia is characterized by obstruction of the distal esophagus and subsequent dilation of the proximal esophagus, and is considered to be a rare disorder in children. Patients commonly present with gastrointestinal (GI) symptoms such as dysphagia; however, pulmonary symptoms may also occur. Rare pulmonary symptoms due to achalasia are dyspnea and wheeze due to tracheal compression. The authors describe an 11-year-old boy who was referred to a pediatric respiratory clinic for asthma that was not responsive to inhaled medications. The child presented with a one-year history of dyspnea on exer

[If no-acid, no-heartburn? New physiopathological mechanisms of the gastroesophageal reflux disease].

The role of acid is very well established in the pathogenesis of gastroesophageal reflux disease (GERD). In the current era of frequent use of proton pump inhibitors, we are seeing increasing numbers of patients with symptoms that are refractory to acid suppression. Recent studies suggest that in patients being treated with proton pump inhibitors, non-acid reflux (composed of buffered gastric contents), esophageal hypersensitivity, esophageal motor dysfunction and psychological comorbidity can cause persistent symptoms. Concepts surrounding possible pathogenetic mechanisms leading to heartburn…

What is heartburn worth? A cost-utility analysis of management strategies.

OBJECTIVE: To determine the best treatment strategy for the management of patients presenting with symptoms consistent with uncomplicated heartburn. METHODS: We performed a cost-utility analysis of 4 alternatives: empirical proton pump inhibitor, empirical histamine2-receptor antagonist, and diagnostic strategies consisting of either esophagogastroduodenoscopy (EGD) or an upper gastrointestinal series before treatment. The time horizon of the model was 1 year. The base case analysis assumed a cohort of otherwise healthy 45-year-old individuals in a primary care practice. MAIN RESULTS: Empiri…
