We are reporting a rare case of acute liver injury that developed after an internal hemorrhoid treatment with the aluminum potassium sulfate and tannic acid (ALTA) regimen. A 41-year-old man developed a fever and liver injury after undergoing internal hemorrhoid treatment with a submucosal injection of ALTA with lidocaine. Rubber band ligation was used in 160 patients with internal hemorrhoids; 43 of them also had rectal anterior mucosal prolapse; 13 had prolapse alone. Two thirds of the patients underwent a single rubber band ligation and one third a double rubber band ligation in one session without anesthesia. Ninety-four required repeated ligations. A retrospective study of 118 cases of haemorrhoids (77 cases), fissures (7 cases) and fistulas (34 cases) was done to demonstrate epithelial changes in each of the three microscopic components (transitional, rectal mucosa and keratinized squamous epithelium) of the anal canal. Twenty-five patients with second- or third-degree hemorrhoids were treated on an outpatient basis using a modified form of pile suture of Farag. There were no postoperative complications such as acute urinary retention, hemorrhage, infection, or anal stenosis. After follow-up for 12 to 18 months, the method has relieved bleeding in all patients and anal prolapse in 96 per cent of them. The modified pile suture is useful in a tropical setting where excisional hemorrhoidectomy is a costly operation.
