Autoimmune hepatitis is a type of chronic hepatitis characterized by hypergammaglobulinemia, hypertransaminasemia, presence of autoantibodies, and active necroinflammatory process in the liver revealed by histology. Its onset is usually acute and has a bad prognosis. Tissue antibodies are found in large proportions of patients. Women outnumber men in a 2-3:1 ratio, and it is mostly young women who are affected. Ten percent of all affected have severe disease characterized by elevations of serum aminotransferase levels greater than five-fold, along with a two-fold elevation of gamma globulin or

Variant forms of autoimmune hepatitis have features that are intermixed with another disorder (overlap syndrome) or findings that are inconsistent with or insufficient for a confident diagnosis of classic disease (outlier syndrome). Diagnostic criteria have not been codified, but application of a modified scoring system provides a template that can be combined with clinical findings to ensure uniform evaluation and classification. Variant forms are frequently assimilated into conventional diagnostic categories. and they may distort analyses of natural history and outcome. Management strategies

AIM: Long term treatment by fibrates could induce chronic hepatitis associated with auto-antibodies. The aim of this study was to assess the features of this hepatitis.
METHODS: Baseline clinical and biological features, and liver biopsy in 5 patients with fibrate-induced chronic hepatitis were studied, as well as their outcome.
RESULTS: At enrollment, patients (4 men, mean age 65 years) had highly (n = 3) or mildly (n = 2) increased serum aminotransferase activity, hypergammaglobulinemia and high titers of anti-nuclear antibodies (with homogenous fluorescence in 3 cases). Liver biopsies dem

AIM: autoimmune hepatitis (AIH) is thought to present rarely in old people. The aims of the present study were to (i) review individuals diagnosed clinically as having AIH presenting over or under 65 years of age at Freeman Hospital, Newcastle, from 1979 to 1993 and (ii) compare diagnostic and histological scores, treatment and outcome, and hence provide a useful profile of the disease in older patients.
METHODS: 310 individuals with a histological diagnosis of chronic hepatitis were identified, and review of the medical notes of 286 revealed 54 with a diagnosis of AIH. Of these, 12 (22%) wer

In summary, type 1 autoimmune hepatitis is infrequently associated with serologic markers of viral infection, and there is no direct evidence that viruses are important causes of this disease. Patients seropositive for anti-LKM1 are commonly infected with HCV, but these patients have predominant features of chronic viral hepatitis and frequently lack antibodies to P450IID6. Such patients respond to therapy with interferon and should be distinguished from patients with type 2 autoimmune hepatitis who have anti-P450IID6, seronegativity for anti-HCV, and responsiveness to corticosteroids. In pati

In order to see if the term of "plasma cell hepatitis", dating back to the early sixties, is still valid as a morphological diagnosis for autoimmune chronic hepatitis (AICH), and to find out if the existence of several subgroups is reflected by histopathology, we investigated 26 patients with chronic hepatitis, who met the criteria of autoimmune hepatitis based on tests for antinuclear, anti-smooth muscle antibodies (SMA) and on immunoassays for liver-kidney-microsomal (LKM) antigen, liver membrane antigen (LMA), and soluble liver antigen (SLA). In our material autoimmune hepatitis represent t

Efavirenz is a widely used and effective non-nucleoside reverse transcriptase inhibitor (NNRTI) for the treatment of HIV. Hepatotoxicity, although recognized, typically occurs early in the course of NNRTI therapy. However, delayed liver injury resembling autoimmune hepatitis is rare. We report a 62-year-old HIV-positive man on long-term efavirenz who developed new, asymptomatic elevations in liver enzymes after years of stable antiretroviral therapy. Work-up revealed elevated levels antinuclear antibodies (ANA) IgG after exclusion of metabolic and viral hepatitis. Although biopsy findings sugg

Non-alphabetical hepatitis (Epstein Barr virus -EBV-, cytomegalovirus -CMV-, Herpes simplex virus -HSV-, varicella zoster virus -VZV-etc.) may be a mode of revelation of several underlying chronic liver diseases including autoimmune hepatitis (HAI). We report a peculiar case of acute EBV hepatitis, revealing type I autoimmune hepatitis confirmed by liver biopsy through puncture in a female patient on breast cancer treatment. The study involved a 29-year-old female patient on breast cancer treatment scheduled to receive radiotherapy and chemotherapy, hospitalized for acute severe hepatitis (fev

BACKGROUND/AIMS: It has been reported that severe cryptogenic chronic hepatitis may be a subgroup of autoimmune hepatitis. The aims of this study were to investigate the clinical features, liver function tests, human leukocyte antigens and response to immunosuppressive therapy in severe cryptogenic chronic hepatitis, and to compare the findings in such patients with those in patients with autoimmune hepatitis.
METHODS: History of alcohol and hepatotoxic drug intake, markers of metabolic liver disease, autoantibodies (antinuclear antibody, smooth muscle antibody, antibody to liver/kidney micro

Droxicam in a nonsteroid antiinflammatory from the oxicam family which acts as a pro-drug, being transformed into pyroxicam after being hydrolized in the stomach and has induced several cases of cholestatic or mixed hepatitis. A clinical observation in which droxicam provoked initial cholestatic hepatitis which later developed into chronic autoimmune hepatitis is presented. It has been postulated that, after causing cholestatic hepatitis by hypersensitivity and within the context of a previous autoimmune entity such as vitiligo, this drug triggered a silent autoimmune liver disease which was d

We describe a hepatitis B virus carrier who satisfied the criteria of autoimmune hepatitis proposed by the International Autoimmune Hepatitis Group. A 43-year-old Japanese female showed human leukocyte antigen typing including DR4 in addition to hypergammaglobulinemia, presence of autoantibodies, and liver histology suggestive of autoimmune hepatitis. Moreover, the predominant presence of hepatitis B core antigen in nuclei rather than in cytoplasm of hepatocytes suggested less of a possibility of liver cell damage related to hepatitis B virus infection. She completely responded to immunosuppre

CONTEXT.—: Drug-induced autoimmune-like hepatitis (DI-ALH) and idiopathic autoimmune hepatitis (iAIH) share similar clinical, biochemical, serologic, and histologic features. Differentiating between them is crucial for treatment and prognosis.
OBJECTIVE.—: To identify clinical and histologic factors that distinguish DI-ALH from iAIH and develop a predictive scoring algorithm.
DESIGN.—: We evaluated diagnostic laboratory data and histologic features from index liver biopsies of 14 well-characterized DI-ALH cases and compared them with those from 19 age- and sex-matched iAIH cases. Data includ
