Addressing Infection Prevention and Control in the First U.S. Community Hospital to Care for Patients With Ebola Virus Disease: Context for National Recommendations and Future Strategies.

Health care personnel (HCP) caring for patients with Ebola virus disease (EVD) are at increased risk for infection with the virus. In 2014, a Texas hospital became the first U.S. community hospital to care for a patient with EVD; 2 nurses were infected while providing care. This article describes infection control measures developed to strengthen the hospital's capacity to safely diagnose and treat patients with EVD. After admission of the first patient with EVD, a multidisciplinary team from the Centers for Disease Control and Prevention (CDC) joined the hospital's infection preventionists to

Intravenous Drug Use in the Hospital Setting: Advancing Reconciliation for Indigenous Canadians Using In-Hospital Harm Reduction and Culturally Safe Care.

People who inject drugs are likely to end up admitted to a hospital due to complications associated with substance use. While in hospital, many of these patients will continue the self-administration of nonprescribed drugs. When implemented without a harm reduction approach, self-administration can lead to an increase in the acquisition of infectious diseases, injection-related infections, and fatal and nonfatal overdoses. Often, administrators and providers use punitive approaches to manage this behavior among patients and providers. This abstinence-based approach has, and continues to, dispr

Post-Disaster Community Transition of Psychiatric Inpatients: Lessons from the Fukushima Nuclear Accident.

This study sought to explore factors related to community transition after the mandatory evacuation of psychiatric inpatients to other hospitals owing to the Fukushima Daiichi Nuclear Power Plant accident. A retrospective cohort design was adopted and 391 psychiatric patients were examined. Univariate and multivariate analyses were conducted to confirm the association between the achievement or non-achievement of discharge to community living and their backgrounds (age, gender, evacuation destination, psychiatric diagnoses, and physical complications). Multivariate analysis indicated that pati

Religious Literacy as a Clinical Competency in Clinical Pastoral Education in the USA: Reframing Hospital Chaplaincy Formation for Pluralistic Healthcare.

Clinical Pastoral Education (CPE) has long shaped hospital chaplaincy through supervised clinical practice, pastoral reflection, self-awareness, and formation in the use of self. These strengths remain essential, but religious diversity in the USA increasingly requires chaplains to navigate beliefs, rituals, family systems, and moral frameworks that can affect medical decision-making. Using a theoretical and conceptual synthesis of interdisciplinary literature from chaplaincy studies, religion and health, palliative care, and religious literacy scholarship, this article examines the USA health

Reappraisal of community-acquired bacteremia: a proposal of a new classification for the spectrum of acquisition of bacteremia.

In recent years, dramatic changes in health care systems have shifted much of the care of sick individuals from hospitals to the community. Consequently, infections traditionally classified as community-acquired or hospital-acquired infections cannot now be readily classified into either category. We thus propose a new classification based on a wider spectrum of acquisition. A total of 1028 episodes of bloodstream infection (BSI) were divided into 5 categories: true community-acquired infections (370 episodes [36%]), infections in recently discharged patients (110 [11%]), infections associated

Improved strategies to counter the COVID-19 pandemic: Lockdowns vs. primary and community healthcare.

COVID-19 pandemic mitigation strategies are mainly based on social distancing measures and healthcare system reinforcement. However, many countries in Europe and elsewhere implemented strict, horizontal lockdowns because of extensive viral spread in the community which challenges the capacity of the healthcare systems. However, strict lockdowns have various untintended adverse social, economic and health effects, which have yet to be fully elucidated, and have not been considered in models examining the effects of various mitigation measures. Unlike commonly suggested, the dilemma is not about

Voluntary regionalization and associated trends in perinatal care: the Nova Scotia Reproductive Care Program.

The Nova Scotia Reproductive Care Program is a system of voluntary regionalization that involves the 37 hospitals in the province that provide obstetric care to a population of 850,000. Between 1971 and 1980, the perinatal mortality rate in the central tertiary care unit for nonreferred patients fell progressively from 12.5 per 1,000 total births to 5.16. For all cases, including high-risk referrals, this rate has fallen from 12.7 to 7.2. During the same interval, the perinatal mortality rate for the province's seven regional hospitals fell from 18.7 to 12.2, and that for the 28 community hosp

Integrated Hospital, Emergency Department, and Community Surveillance for Respiratory Viruses in Milan, Italy.

Respiratory virus circulation may vary across healthcare settings and age groups, potentially limiting the interpretability of surveillance based on a single source. In Milan, Italy, established community- and emergency department-based systems coexist with hospital laboratory testing, but hospital ward-level data are not routinely integrated. The aims of this study were to describe respiratory viruses' epidemiology and assess whether ward-based surveillance complements community and ED surveillance. We analyzed molecular test results from respiratory samples collected from January 1, 2024 (W1

Direct costs of neonatal infection acquired in the community in full-term newborns and low risk at birth, Cundinamarca, Colombia.

Introduction: Half of the episodes of neonatal sepsis are acquired in the community with a high percentage of mortality and complications. Objective: To estimate the direct costs of hospitalizations due to systemic neonatal infection acquired in the community in low-risk newborns. Materials and methods: For the estimation of costs, we used the perspective of the health systems and the microcosting technique and we established the duration of hospitalization as the time horizon. We identified cost-generating events through expert consensus and the quantification was based on the detailed bill o

Valvular heart disease. A study of mortality in the Sheffield population.

Talbot, S. (1973).Thorax, 28, 622-626. Valvular heart disease: a study of mortality in the Sheffield population. A study of deaths due to valve disease in the community and in hospital showed that aortic valve disease was a more common cause of sudden death than was mitral valve disease. Mitral valve disease was a frequent cause of death in the elderly, but all valve lesions in the elderly were found more often in the community than in the hospital series; these were often (66%) undiagnosed prior to death, particularly in men. Evidence of combined aortic and mitral valve disease was less often

Leading through crisis: care home managers' COVID-19 experiences examined through systems-led setbacks and social positioning theory.

INTRODUCTION: Care homes in England experienced disproportionate harm during the COVID-19 pandemic, yet existing policy analyses offer limited explanation of the structural and positional vulnerabilities faced by care home managers. This conceptual paper applies systems-led setbacks and social positioning theory to examine how pandemic policies shaped care home management across macro, meso and micro contexts. METHODS: Qualitative research evidence relating to care home management during the pandemic was examined using a multi-level thematic analysis. Evidence was organised across six policy
