Publicly Disclosed Pricing for Common Upper-Extremity Procedures: An Analysis of Hospital Chargemasters.

BACKGROUND: American hospitals are required to provide price transparency data (known as a chargemaster) for medical services, which is intended to allow consumers to accurately estimate the cost of medical services. Our purpose was to identify hospital compliance in publishing chargemaster documents and to assess the price information published for common upper-extremity services and procedures. METHODS: We performed a cross-sectional analysis of publicly available chargemaster data from 122 hospitals, which included the top-20-ranked Honor Roll hospitals from US News and World Report and 2

Observation Encounters and Length of Stay Benchmarking in Children's Hospitals.

BACKGROUND AND OBJECTIVES: Length of stay (LOS) is a common benchmarking measure for hospital resource use and quality. Observation status (OBS) is considered an outpatient service despite the use of the same facilities as inpatient status (IP) in most children's hospitals, and LOS calculations often exclude OBS stays. Variability in the use of OBS by hospitals may significantly impact calculated LOS. We sought to determine the impact of including OBS in calculating LOS across children's hospitals. METHODS: Retrospective cohort study of hospitalized children (age <19 years) in 2017 from the P

Ultrasound for Diagnosis of Appendicitis in a Community Hospital Emergency Department has a High Rate of Nondiagnostic Studies.

BACKGROUND: Radiation concerns are changing the way emergency physicians evaluate patients. This is especially prevalent in pediatrics, and exemplified by abdominal pain management. Large academic center-based studies suggest appendix ultrasound (U/S) is sensitive and specific for appendicitis, with low nondiagnostic rates. OBJECTIVES: We sought to determine the diagnostic rate of appendix U/S and incidence of follow-up computed tomography (CT) imaging for pediatric patients at a community hospital. METHODS: Design: Retrospective cohort. SETTING: Emergency department with 85,000 annual visi

Propranolol Versus Digoxin in the Neonate for Supraventricular Tachycardia (from the Pediatric Health Information System).

Conflicting data exist for the appropriate management of a neonate with supraventricular tachycardia (SVT). We sought to assess postnatal prescribing trends for neonates with SVT and to evaluate if there were therapy-specific mortality and resource utilization benefits. Nationally distributed data from 44 pediatric hospitals in the 2004 to 2015 Pediatric Health Information System database were used to identify patients admitted at ≤2 days of age with structurally normal hearts and treated with an antiarrhythmic medication. Outcome variables were mortality, cost, and length of stay (LOS). Multi

Development and Validation of a Multi-Modal Algorithm for Chronic Kidney Disease Detection in a Hospital Clinical Data Warehouse.

Chronic kidney disease (CKD) is often underdetected in hospitalized older adults due to incomplete coding and laboratory data. Using eHOP, the clinical data warehouse of Rennes University Hospital, we developed and validated a rule-based interpretable multimodal algorithm combining laboratory and textual information for CKD phenotyping. Among 15,112 hospital visits (7,556 with CKD and 7,556 without), the algorithm achieved an accuracy of 92.1 % in agreement with ICD-10, recall 96.5 %, and precision 86.6 %. Validation on a clinician-reviewed subset confirmed the reliability of classifications.

Protocol for a scoping review to understand how interorganisational electronic health records affect hospital physician and pharmacist decisions.

INTRODUCTION: Patient records are often fragmented across organisations and departments in UK health and care services, often due to substandard information technology. However, although government policy in the UK and internationally is strongly pushing 'digital transformation', the evidence for the positive impact of electronic information systems on cost, quality and safety of healthcare is far from clear. In particular, the mechanisms by which information availability is translated into better decision-making are not well understood. We do not know when a full interorganisational record is

From hospitalization records to surveillance: The use of local patient profiles to characterize cholera in Vellore, India.

Despite availability of high quality medical records, health care systems often do not have the resources or tools to utilize these data efficiently. Yet, hospital-based, laboratory-confirmed records may pave the way for building reliable surveillance systems capable of monitoring temporal trends of emerging infections. In this communication, we present a new tool to compress and visualize medical records with a local population profile (LPP) approach, which transforms information into statistically comparable patterns. We provide a step-by-step tutorial on how to build, interpret, and expand

Automated Reconciliation of Radiology Reports and Discharge Summaries.

We study machine learning techniques to automatically identify limb abnormalities (including fractures, dislocations and foreign bodies) from radiology reports. For patients presenting to the Emergency Room (ER) with suspected limb abnormalities (e.g., fractures) there is often a multi-day delay before the radiology report is available to ER staff, by which time the patient may have been discharged home with the possibility of undiagnosed fractures. ER staff, currently, have to manually review and reconcile radiology reports with the ER discharge diagnosis; this is a laborious and error-prone

Measurement of Long-Term, Quality of Life Outcomes in Injury Databases.

INTRODUCTION: Traumatic injuries affect patients' long-term quality of life (QOL). In 2016, the National Academies highlighted the need to record long-term QOL outcomes within trauma databases. We studied national injury-specific databases to assess whether they capture QOL outcomes, and their key determinants, across the entire continuum of trauma care. METHODS: We coded variables in the Trauma Quality Program (TQP), Burn Care Quality Platform, and the Burn, Spinal Cord Injury (SCI), Traumatic Brain Injury (TBI) Model Systems databases to identify QOL and social determinants of health (SDOH)

Emergency Preparedness and Management of Mobile Cabin Hospitals in China During the COVID-19 Pandemic.

The healthcare systems in China and globally have faced serious challenges during the coronavirus disease (COVID-19) pandemic. The shortage of beds in traditional hospitals has exacerbated the threat of COVID-19. To increase the number of available beds, China implemented a special public health measure of opening mobile cabin hospitals. Mobile cabin hospitals, also called Fangcang shelter hospitals, refer to large-scale public venues such as indoor stadiums and exhibition centers converted to temporary hospitals. This study is a mini review of the practice of mobile cabin hospitals in China.

Patient-Centric Blockchain-Enabled Framework for Secure Medical Data Sharing in Indian EHR Systems.

Electronic health records (EHRs) are integral to modern hospital systems, especially in India's rapidly digitizing healthcare landscape. EHRs connect hospital records through shared networks, enabling quick and accurate data sharing. However, these systems are targeted by attackers as they store large volumes of sensitive patient information. Thus, this research proposed a secure EHR framework named the sequential belief neural network (SeqBNNet). The SeqBNNet integrates deep belief networks (DBN) with sequential convolutional neural networks (SCNN) to encrypt EHRs. The images are split into N

Deployment of a computerized ward visitor registration system in coronavirus disease 2019 epidemic: Experiences of a large academic medical center in Taiwan.

BACKGROUND: Visitors to hospitalized patients during an epidemic might themselves be carriers and are therefore likely to spread the disease in wards. Although measures were taken to restrict hospital visits, traditional paper-based registration is insufficient to screen and monitor the numbers of visitors to a large hospital. METHODS: Throughout March 2020, during the coronavirus disease 2019 crisis, a computer system was deployed in the 2800-bed Taipei Veterans General Hospital (Taipei, Taiwan) to register, screen, and monitor inpatient visitors. This system comprised three parts: online re

Decision support increases guideline adherence for prescribing postoperative nausea and vomiting prophylaxis.

BACKGROUND: Guidelines for postoperative nausea and vomiting (PONV) prevention are implemented widely but their effectiveness may be limited by poor adherence. We hypothesized that the use of an electronic decision support (DS) system would significantly improve guideline adherence. METHODS: Medical information of all patients undergoing elective surgery in our regional teaching hospital is routinely entered in an anesthesia information management system at the preoperative screening clinic. Our departmental PONV prevention guidelines identifies patients as "high-risk" and thus eligible for P
