HIV has challenged humanity for decades, quietly establishing latent reservoirs that evade even the most potent antiretroviral therapies (ART). While ART transformed HIV from a fatal disease into a manageable condition, the virus's hidden persistence continues to fuel the global epidemic. This review narrates the story of HIV treatment, tracing the evolution of therapeutic strategies from conventional ART to innovative gene-editing and immunotherapeutic approaches. Central to this journey is the CRISPR-Cas system, a molecular scalpel that has enabled precise excision of integrated proviral DNA

Tuberculous meningitis disproportionately affects young children. As the most devastating form of tuberculosis, it is associated with unacceptably high rates of mortality and morbidity even if treated. Challenging to diagnose and treat, tuberculous meningitis commonly causes long-term neurodisability in those who do survive. There remains an urgent need for strengthened surveillance, improved rapid diagnostics technology, optimised anti-tuberculosis drug therapy, investigation of new host-directed therapy, and further research on long-term functional and neurodevelopmental outcomes to allow ta

The high incidence of tuberculosis (TB) in the world, especially in developing countries, the resurgence of TB in industrialized countries, and the global increase in the prevalence of Mycobacterium avium complex infections in immunocompromised hosts have prompted the quest for novel antimycobacterial drugs. However, the development of such antimicrobial chemotherapeutics is currently making very slow progress even with using the bioinformatics-based methodology for drug design. It thus appears that devising improved administration protocols for clinical treatment against intractable mycobacte

Human pathogenic Legionella replicate in alveolar macrophages and cause a potentially lethal form of pneumonia known as Legionnaires' disease(1). Here, we have identified a host-directed therapeutic approach to eliminate intracellular Legionella infections. We demonstrate that the genetic deletion, or pharmacological inhibition, of the host cell pro-survival protein BCL-XL induces intrinsic apoptosis of macrophages infected with virulent Legionella strains, thereby abrogating Legionella replication. BCL-XL is essential for the survival of Legionella-infected macrophages due to bacterial inhibi

Lens epithelium-derived growth factor (LEDGF) fusion proteins can direct HIV-1 DNA integration to novel sites in the host genome. The C terminus of LEDGF contains an integrase binding domain (IBD), and the N terminus binds chromatin. LEDGF normally directs integrations to the bodies of expressed genes. Replacing the N terminus of LEDGF with chromatin binding domains (CBDs) from other proteins changes the specificity of HIV-1 DNA integration. We chose two well-characterized CBDs: the plant homeodomain (PHD) finger from ING2 and the chromodomain from heterochromatin binding protein 1alpha (HP1al

Influenza-associated pulmonary aspergillosis (IAPA) is a severe fungal superinfection affecting critically ill patients with influenza. Current treatments target the causative pathogens but do not address the dysregulated host immune responses that drive morbidity. Host-directed immunotherapies could overcome this treatment gap. Here, we studied the host-pathogen factors driving IAPA using patient samples and an IAPA mouse model. We identified interleukin-1 (IL-1)-mediated inflammation, neutrophil activation, and neutrophil extracellular trap (NET) release as crucial features in IAPA pathogene

Inactivation or targeted disruption of a gene provides clues to assess the function of the gene in many cellular processes. Knockdown or knocking out a gene has been widely used for this purpose. However, recently CRISPR mediated genome editing has taken over the knockout/knockdown system with more precision. CRISPR technique has enabled us to perform targeted mutagenesis or genome editing to address questions in fundamental biology to biomedical research. Its application is wide in understanding the role of genes in the disease process, and response to therapy in cancer, metabolic disorders,

Intracellular pathogenic bacteria are often-overlooked agent of chronic, recurrent and difficult-to-treat infections in a context of escalating antimicrobial resistance. By hiding within professional and non-professional phagocytes, adopting dormant or slow-replicative states and rewiring host pathways, these pathogens successfully evade both immune clearance and conventional antibiotic therapy, thus contributing to treatment failure and, potentially, carcinogenesis. Here we synthesise how obligate and facultative intracellular bacteria invade and persist within host cells, and interpret these

Severe fever with thrombocytopenia syndrome (SFTS), caused by the Bandavirus dabieense (Severe fever with thrombocytopenia syndrome virus: SFTSV), is an emerging tick-borne disease with a case-fatality rate of 5–30%. Endemic to East Asia with increasing global incidence, SFTS poses a significant public health threat, yet no specific antiviral or vaccine is approved. This review provides a comprehensive analysis of current therapeutic candidates for SFTS. We reviewed progress in drug repurposing (e.g., favipiravir), host-directed therapies, immunomodulators (including problematic steroid use),

INTRODUCTION: Sepsis is a life-threatening organ dysfunction syndrome with persistently high global mortality, driven by dysregulated host immune response. Existing single-target therapies fail to simultaneously address hyperinflammation and impaired tissue repair, leading to limited clinical efficacy and repeated translational failures.
METHODS: Integrated multi-omics datasets (scRNA-seq, miRNA-seq, blood/lung RNA-seq) delineated immune cell dynamics and dysregulated pathways in sepsis. Guided by omics findings, we designed two small-molecule combinations (C1, C2) targeting the identified pa

Chronic hepatitis B (CHB) remains a leading cause of liver-related morbidity and mortality, affecting approximately 254 million people worldwide and accounting for more than 800,000 deaths annually. Although nucleos(t)ide analogues (NAs) durably suppress viral replication, the annual rate of HBsAg loss is only ~1%, leaving functional cure as the foremost unmet clinical need. Exhaustion of HBV-specific CD8+ T cells is now widely accepted as the central immunological barrier to viral clearance. Single-cell sequencing, spatial transcriptomics and epigenome profiling have collectively redefined th

Microbial diseases are a global health threat, leading to tremendous casualties and economic losses. The strategy to treat microbial diseases falls into two broad categories: pathogen-directed therapy (PDT) and host-directed therapy (HDT). As the typical PDT, antibiotics or antiviral drugs directly attack bacteria or viruses through discerning specific molecules. However, drug abuse could result in antimicrobial resistance and increase infectious disease morbidity. Recently, the exosome therapy, as a HDT, has attracted extensive attentions for its potential in limiting infectious complications

The biofilms of Mycobacterium Tuberculosis (Mtb) use extracellular DNA (eDNA) as a protective structural scaffold that shields resident bacteria from host immunity and antibiotics, promoting persistent infection. An FDA-approved DNase can partially hydrolyze eDNA but has limited affinity and nucleolytic activity against Z-form DNA (Z-DNA). After confirming the abundance of Z-DNA in Mtb biofilms, we develop Nano-NucleOTAC, a nano-nucleolysis targeting chimera that combines an eDNA-binding ligand (lactoferrin), a DNA intercalator (chloroquine) and DNase. Nano-NucleOTAC anchors to Z-DNA in Mtb H3

Chronic hepatitis B virus (HBV) infection drives immune-mediated liver injury, which represents a central mechanism underlying the progression of viral hepatitis. Although current antiviral therapies effectively suppress viral replication, they remain inadequate in reducing the incidence and mortality of liver cirrhosis and hepatocellular carcinoma. A key factor limiting treatment success is the establishment of immune tolerance by persistent antigen exposure, which impedes viral clearance. Therapeutic vaccines designed to re-establish virus-specific immunity have thus emerged as a pivotal str

Persistent HIV reservoirs in long-lived macrophages pose a unique and formidable challenge to achieving HIV cure. HIV-infected macrophages are more resistant than CD4+ T cells to both virus- and immune-mediated death pathways including apoptosis, facilitating their persistence in tissue sanctuary sites and potential to contribute to viral rebound upon therapy cessation. This resistance is driven by HIV-induced modulation of both intrinsic and extrinsic apoptotic pathways, alongside survival mechanisms including autophagy. In this review, we examine the biological mechanisms promoting macrophag

Autophagy is a critical host defense mechanism that restricts intracellular pathogens such as Mycobacterium tuberculosis (Mtb). A key step in this process is the ubiquitination of Mtb or Mtb-associated structures. The E3 ligase SMURF1 catalyzes K48-linked ubiquitination, promoting bacterial clearance. However, the function of its homolog, SMURF2, in host defense remains undefined. Here, we demonstrate that Smurf2 deletion in murine macrophages increases SMURF1 levels, enhances LC3B lipidation, augments K48 ubiquitination of Mtb-associated structures, and reduces intracellular Mtb replication.

Increasing antibiotic-resistant bacterial infections pose a global public health challenge that demands therapeutic strategies beyond conventional antibiotics. The cyclic guanosine monophosphate-adenosine monophosphate synthase stimulator of interferon genes (cGAS-STING) pathway is crucial to innate immune defense through cytosolic DNA detection and antimicrobial response initiation. Emerging evidence suggests that antibiotic-resistant bacteria can subvert or overactivate this pathway, leading to immune evasion and excessive inflammation. Intracellular pathogens such as Mycobacterium tuberculosis (Mtb) evade host defence mechanisms to infect and survive within host cells. Host-directed therapy (HDT) offers a promising alternative to antibiotics and may overcome antimicrobial resistance. Using high-content screening, we identified benztropine (BZT), an approved Parkinson's disease drug, as a potent inhibitor of intracellular Mtb. BZT is active in both human and murine macrophages but is inactive in broth. In an aerosol Mtb mouse infection model, oral administration of BZT reduced the burden of Mtb in the lungs by up to 70%. BZT

The success of antiretroviral therapy (ART) for people with HIV has been a result of direct acting antiviral small molecules that target key components of the viral life cycle, however ART must be taken life long and there is no cure. The major barrier to a cure for HIV is the persistence of a long lived and proliferating reservoir of latently infected cells that persist on ART. Cure strategies for HIV currently target host proteins to either reduce the size of the reservoir or enhance HIV-specific immunity. A major challenge of targeting a host protein is the lack of specificity for HIV and t

Effective and safe liver-directed gene therapy has great promise in treating a broad range of liver diseases. While adenoviral (Ad) vectors have been widely used for efficacious in vivo gene delivery, their translational utilities are severely limited due to the short duration of transgene expression and solicitation of host immune response. Used as a promising polymeric vehicle for drug release and nucleic acid delivery, carboxymethyl chitosan (CMC) is biocompatible, biodegradable, anti-microbial, inexpensive, and easy accessible. Here, by exploiting its biocompatibility, controlled release c
