Background: Hymenolepis diminuta, a rat tapeworm, is a rare zoonosis typically affecting children. We report the first confirmed human case in Yunnan Province, China, not able for its occurrence in an adult. Methods: A 42-year-old female farmer from Dali presented with unilateral painless vision loss. BACKGROUND: Studies on the inhibition of inflammation by infection with helminth parasites have, until recently, overlooked a key determinant of health: the gut microbiota. Infection with helminths evokes changes in the composition of their host's microbiota: one outcome of which is an altered metabolome (e.g., levels of short-chain fatty acids (SCFAs)) in the gut lumen. The functional implications of helminth-evoked changes in the enteric microbiome (composition and metabolites) are poorly understood and are explored with respect to controlling enteric inflammation.

Stool examination was performed on 2,083 Thai children from orphanages and primary schools. Hymenolepis nana infection was found only in children from orphanages with a prevalence of 13.12 per cent. Males had a statistically significant higher prevalence of infection than females. Most infected children were asymptomatic. In symptomatic infected children, the symptoms were mild and non-specific such as pruritus ani, abdominal pain, diarrhea, anorexia, headache, and dizziness. The aqueous extract of Albizzia anthelmintica bark at 10-150 g/kg orally showed no toxicity and high anthelmintic activity (68-100%) against experimental Hymenolepis diminuta infection in albino rats. Scanning electron microscopy of the surface of Hymenolepis nana indicated that dense populations of microtriches occur on scolex proper, suckers and strobila, with an average density of 20/micron2. The excellent preservation of microtriches proves the efficacy of the critical point drying method for preparing cestodes for study of SEM. The cytological structure of the tegument of H. nana corresponds in general to that of other tapeworms.

Individual worms from rats infected with different strains of Hymenolepis diminuta were incubated in vitro and the products lactate, succinate, acetate and ammonia assayed. Variability in excretion was not confined to differences between strains. Two metabolic types were identified. Where succinate was above 20 mumol g-1 h-1, lactate excretion was low. Where succinate was not detected, lactate excretion was high. Acetate excretion was variable. Lactate and ammonia excretion were positively correlated. All worms from one rat were of the same type but could be of either type from different rats.

Hymenolepsis diminuta is a tapeworm which is an intestinal parasite of rats and mice. Rarely, through accidental ingestion of an infected arthropod, man can become the definitive host. This report documents, for the first time, that such infections occur in Jamaica, West Indies. The life cycle of the parasite and its treatment are also discussed.
