Supine awake oximetry as a screening tool for daytime hypercapnia in super-obese patients.

BACKGROUND: Evidence-based screening tools are required for detection of daytime hypercapnia in high-risk patient populations. AIMS: To determine the validity of supine awake oximetry as a test for daytime hypercapnia and severe sleep disordered breathing (SDB) in super-obese patients. METHODS: This was a cross-sectional diagnostic test evaluation of super-obese adults (body mass index >50 kg/m2 ) presenting to Liverpool Hospital, Australia, between 2009 and 2015 for diagnostic polysomnography (PSG) and arterial blood gas measurement. Supine awake oxygen saturation (SpO2 ) was determined usi

The effect of environmental hypercapnia and size on nitrite toxicity in the striped catfish (Pangasianodon hypophthalmus).

Striped catfish (Pangasianodon hypophthalmus) are farmed intensively at high stocking densities in Vietnam where they are likely to encounter environmental hypercapnia as well as occasional high levels of aquatic nitrite. Nitrite competes with Cl(-) for uptake at the branchial HCO3(-)/Cl(-) exchanger, causing a drastic reduction in the blood oxygen carrying capacity through the formation of methaemoglobin and nitrosylhaemoglobin. Environmental hypercapnia induces a respiratory acidosis where the branchial HCO3(-)/Cl(-) exchange activity is reduced in order to retain HCO3(-) for pH recovery, wh

Ventilatory and chemoreceptor responses to hypercapnia in neonatal rats chronically exposed to moderate hyperoxia.

Rats reared in hyperoxia hypoventilate in normoxia and exhibit progressive blunting of the hypoxic ventilatory response, changes which are at least partially attributed to abnormal carotid body development. Since the carotid body also responds to changes in arterial CO2/pH, we tested the hypothesis that developmental hyperoxia would attenuate the hypercapnic ventilatory response (HCVR) of neonatal rats by blunting peripheral and/or central chemoreceptor responses to hypercapnic challenges. Rats were reared in 21% O2 (Control) or 60% O2 (Hyperoxia) until studied at 4, 6-7, or 13-14days of age.

Vagal afferent control of abdominal expiratory activity in response to hypoxia and hypercapnia in rats.

In the present study, we tested the hypothesis that vagal afferent information modulates the pattern of expiratory response to hypercapnia and hypoxia. Simultaneous recordings of airflow, diaphragmatic (DIA) and oblique abdominal muscle (ABD) activities were performed in anesthetized (urethane, 1.2g/kg), tracheostomized, spontaneously breathing male Wistar rats (290-320g, n=12). The animals were exposed to hypercapnia (7 and 10% CO2 for 5min) and hypoxia (7% O2 for 1min) before and after bilateral vagotomy. We verified that the percentage increase in DIA burst amplitude elicited by hypercapnia

The effect of early prenatal hypercapnia on the vascular network in the chorioallantoic membrane of the chicken embryo.

Over the last decade, the poultry sector has sought to develop novel ways to monitor chicken embryonic growth, health, and quality to control and optimize egg incubation conditions, particularly the concentration of dissolved gases (O(2), CO(2)). One of the parameters, which may change under different gas concentrations, is the angiogenesis in the chorioallantoic membrane (CAM), the organ for gas exchange of the chicken embryo. In this study, a newly developed methodology was used to quantify the angiogenesis in the CAM under normal and early hypercapnic conditions (i.e., increased CO(2) conce

A twin study of the common vulnerability between heightened sensitivity to hypercapnia and panic disorder.

For unknown reasons the inhalation of CO(2)-enriched air mixtures evokes acute panic-like symptoms in people with panic disorder and in their unaffected relatives. This study was set to determine whether, and to what extent, CO(2)-induced acute anxiety and panic disorder share the same genetic and environmental determinants. Cholesky structural equation models were used to decompose into genetic and environmental elements the correlation between self-assessed anxiety post-35%CO(2)-65%O(2) inhalation and interview-based DSM-IV lifetime diagnoses of panic disorder in 346 young adult twin pairs o

[Metabolic alkalosis despite hyperlactatemia and hypercapnia. Interpretation and therapy with help of the Stewart concept].

Acid-base disturbances are commonly found in critically ill patients and are often associated with fatal complications. The basis of a successful treatment is a thorough understanding of the causes of these disorders. The "classical methods" to explain acid-base disorders--pH, base excess and bicarbonate concentration--mostly do not provide a causal correlation to the underlying pathology. An unusual case of a combined respiratory-metabolic disorder with hyperlactatemia and hypercapnia is presented. An acidosis masked by hypochloremic and hypoalbuminemic alkalosis was identified with the help

Alteration of the piglet diaphragm contractility in vivo and its recovery after acute hypercapnia.

BACKGROUND: The effects of hypercapnic acidosis on the diaphragm and its recovery to normocapnia have been poorly evaluated. The authors studied diaphragmatic contractility facing acute variations of arterial carbon dioxide tension (Paco2) and evaluated the contractile function at 60 min after normocapnia recovery. METHODS: Thirteen piglets weighing 15-20 kg were anesthetized, ventilated, and separated into two groups: a control group (n = 5) evaluated in normocapnia (time-control experiments) and a hypercapnia group (n = 8) in which animals were acutely and shortly exposed to five consecutiv

Effects of hypercapnia and hypocapnia on ventilatory variability and the chaotic dynamics of ventilatory flow in humans.

In humans, lung ventilation exhibits breath-to-breath variability and dynamics that are nonlinear, complex, sensitive to initial conditions, unpredictable in the long-term, and chaotic. Hypercapnia, as produced by the inhalation of a CO(2)-enriched gas mixture, stimulates ventilation. Hypocapnia, as produced by mechanical hyperventilation, depresses ventilation in animals and in humans during sleep, but it does not induce apnea in awake humans. This emphasizes the suprapontine influences on ventilatory control. How cortical and subcortical commands interfere thus depend on the prevailing CO(2)

A 10-year-old child with status asthmaticus, hypercapnia and a unilateral dilated pupil.

This article reports the case of a 10-year-old child with an exacerbation of asthma requiring mechanical ventilation. His immediate course was complicated by significantly elevated arterial CO(2) tensions and a unilateral dilated pupil. A computed tomography scan of his brain failed to demonstrate any evidence of intracranial hypertension or cerebral edema, and he went on to make an uncomplicated recovery, with no evidence of neurological sequelae. The most likely diagnosis appeared to be local contamination with ipratropium when he was receiving frequent nebulizers prior to mechanical ventila

The effect of high ambient temperature and hypercapnia on postprandial intestinal hyperemia in domestic cockerels.

Hubbard cockerels (2.8 to 3.6 kg) with chronically implanted electromagnetic blood flow probes placed on the celiac artery were used to determine the effect of elevated ambient temperature on postprandial intestinal hyperemia. Celiac mean blood flow (MBF) increased (P less than .05) from approximately 25 to 50 ml/min in response to feeding. When a thermoneutral temperature of 25 C was maintained, celiac MBF remained above 40 ml/min up to 210 min but fell below 25 ml/min by 300 min postprandial. In response to an acute heat exposure of 37 C, postprandial celiac MBF was reduced by approximately

The effects of hypercapnia and cooling the ventral medullary surface on capsaicin induced respiratory reflexes.

The effect of right atrial (RA) injection of 3 micrograms/kg capsaicin on phrenic, hypoglossal and recurrent laryngeal activities was studied in chloralose anesthetized, paralyzed and artificially ventilated cats. Within 2 sec following capsaicin injection, the phrenic and hypoglossal activities completely disappeared (apnea), while the recurrent laryngeal activity markedly increased. Similar responses were also obtained with RA injection of phenyldiguinide (PDG), suggesting that the respiratory responses of both drugs are essentially similar. Sino-aortic denervation did not affect the capsaic

Cardiovascular Effects of Hypercapnia During Lower Body Negative Pressure and Head-Up Tilt.

INTRODUCTION: Exposure to head-to-toe acceleration (Gz) may lead to G-induced loss of consciousness due to reduced head-level blood pressure and flow. Despite various countermeasures, G-induced loss of consciousness remains a safety concern, causing accidents and fatalities. Inhalation of 5% carbon dioxide (CO2) and higher concentrations can improve G-tolerance but lead to adverse symptoms. This study investigated the impact of 5% and lower concentrations of CO2 on the cardiovascular system during orthostatic stress. METHODS: To simulate +Gz cardiovascular effects, 20 healthy subjects (11 mal

Impact of mild hypercapnia on renal function after out-of-hospital cardiac arrest.

BACKGROUND: Acute kidney injury (AKI) is a serious complication of out-of-hospital cardiac arrest (OHCA). Post-resuscitation cardiogenic shock (CS) is a key contributing factor. Targeting a higher arterial carbon dioxide tension may affect AKI after OHCA in patients with or without CS. METHODS: Pre-planned exploratory study of a multi-national randomised trial comparing targeted mild hypercapnia or targeted normocapnia. The primary outcome was AKI defined by Kidney Disease: Improving Global Outcomes (KDIGO) criteria with modifications. Secondary outcomes included use of renal replacement ther
