The authors describe the clinical findings, radiographic appearance and surgical treatment of a spinal epidural hematoma (SEH) associated with spinal fracture in diffuse idiopathic skeletal hyperostosis (DISH). An 81-year-old male patient injured after falling from a 2.5 m tree presented with severe back pain. On plain radiographs and CT images, DISH with anterolateral osteophytes and fused thoracolumbar vertebrae was found along with a T12 fracture. Patient was initially treated with bed rest followed by placement into a body cast. Three weeks later, he presented with incomplete paraplegia of…

PURPOSE: The effect of diffuse idiopathic skeletal hyperostosis (DISH) on spinopelvic alignment in patients with lumbar spinal stenosis (LSS) remains unclear. The aim of this study was to investigate the association between DISH and sagittal spinopelvic alignment in patients undergoing surgery for LSS.
METHODS: A total of 132 patients aged >40 years who required surgical procedures for LSS were investigated. DISH was defined by Resnick's and Mata's criteria on preoperative standing whole-spine radiographs. We investigated the prevalence and localization of DISH and its relation to preoperativ…

We report on a fetus of 27 weeks of gestation whose clinical, radiological, and histopathological findings are compatible with the prenatal form of Caffey disease (cortical hyperostosis). Prenatal ultrasound examination showed polyhydramnios and markedly short and angulated long bones, which had led to the incorrect diagnosis of lethal osteogenesis imperfecta. We found 43 published descriptions on the antenatal onset of cortical hyperostosis symptomatic in utero or at birth. Two groups of prenatal cortical hyperostosis (PCH) can be distinguished: (1) severe (25 reports and the fetus presented …

Van Buchem disease (hyperostosis corticalis generalisata; OMIM 239100 [http://www3.ncbi.nlm.nih. gov:80/htbin-post/Omim/dispmim?239100]) is an autosomal recessive disorder characterized by hyperostosis of the skull, mandible, clavicles, ribs, and diaphyseal cortices of the long bones. The most striking clinical features are the enlargement of the jaw and the thickness of the skull, which may lead to facial nerve palsy, hearing loss, and optic atrophy. Increased formation, by osteoblasts, of qualitatively normal bone has been proposed as the underlying pathological mechanism, but the molecular …

We describe 2 sibs with lethal, prenatal-onset cortical hyperostosis. Antenatal sonographic diagnosis of the first sib was that of short limb dwarfism and thoracic dysplasia (nonspecific), possibly osteogenesis imperfecta. The second sib had a similar appearance on ultrasonography. The thickened, irregularly echodense diaphyses were an aid to diagnosis. Although sporadic cases and autosomal dominant inheritance have been described, an autosomal recessive cause for the lethal prenatal onset cases cannot be excluded.

A middle-aged man presented with a 10-year history of intermittent low back pain. Radiological examination of the spine disclosed findings of both ankylosing spondylitis and diffuse idiopathic skeletal hyperostosis (DISH). Radiography of the hands showed the typical enthesopathy of DISH. To our knowledge this is the 2nd published case on the coexistence of the two diseases.

We report two cases of retroperitoneal fibrosis which were found in 38 patients with "spondarthritis hyperostotica pustulo-psoriatica" (spond.hyp.p.-p.). Spond.hyp.p.-p. is associated with sternocosto-clavicular hyperostosis and resembles pustulotic arthroosteitis and SAPHO syndrome. However, as dermatologically and osteo-histopathologically documented, it is an enthesiopathic and hyperostotic form of spondarthritis psoriatica. Both cases show a lumbosacroiliac hyperostosis (LSIH) which is a rare analogue to sternocosto-clavicular hyperostosis (SCCH). Facultative paraosseous connective-tissue …

INTRODUCTION: Diffuse idiopathic skeletal hyperostosis (DISH) primarily affects the axial skeleton, but its association with hip osteoarthritis (OA) remains unclear. We investigated the prevalence and 5 year progression of hip OA in individuals with DISH and explored risk factors for progression.
MATERIALS AND METHODS: This retrospective cohort study reviewed 694 asymptomatic individuals who underwent whole-body computed tomography (CT) for cancer screening (2016-2023) and had follow-up CT at ≥ 5 years. DISH was diagnosed using CT-based criteria. We identified 143 individuals with DISH and se…

INTRODUCTION: Diffuse idiopathic skeletal hyperostosis (DISH)-related fractures have a high frequency of delayed diagnosis and paralysis even if caused by low-energy trauma, which makes diagnosing vertebral fractures (VFs) with DISH challenging. This study compared the clinical and radiologic features of VFs with DISH.
METHODS: This study included 252 patients (70 men and 182 women; mean age±standard deviation, 81.0±8.6 years) with VFs in this study. Patients were divided into two groups depending on DISH (group D) or not (group N). We measured the sex, age, body mass index, hemoglobin A1c, a…

OBJECTIVE: Diffuse idiopathic skeletal hyperostosis (DISH) is characterized by calcification and ossification, predominantly affecting the axial skeleton. DISH is associated with obesity, diabetes mellitus, and hypertension, all included in the metabolic syndrome (MetS). The aim of this study was to investigate the prevalence of DISH in patients with MetS and the influence of obesity on spinal radiographic changes related to DISH vs degenerative changes.
METHODS: Patients diagnosed with MetS were categorized into 4 obesity classes (World Health Organization criteria). Based on Resnick criteri…

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) is characterized by ankylosing spinal alterations which are often asymptomatic but may typically cause back pain and spinal stiffness. Presence of DISH may complicate spinal trauma and lead to unstable fractures requiring surgical intervention. Treatment options include physical activity, symptomatic treatment, local heat application, and optimization of metabolic comorbidities.
CASE: A multimorbid older patient was admitted to the gastroenterological ward for the investigation of progressive dysphagia and weight loss. Gastroscopy re…

BACKGROUND: Persons with autism spectrum disorder (ASD) can have restrictive diets due to stereotyped behaviors. These restrictive diets can manifest with nutritional deficiencies, such as Vitamin A deficiency. The most frequent manifestation of hypovitaminosis A is vision loss secondary to xerophthalmia. Here the authors report six cases of males with a clinical triad of hypovitaminosis A, cranial hyperostosis, and optic neuropathy.
METHODS: A retrospective case series of six males (ages 5-17 years old) with ASD who presented with several weeks of vision loss and nyctalopia were reviewed.

INTRODUCTION: Stress fracture is generally a result of cumulative and repetitive stress in athletes, which accelerates the normal remodeling … he stress fracture of the midshaft of the clavicle in this case was caused by sternocostoclavicular hyperostosis (SCCH). Because the clavicular fracture had no displacement or callus formation, conservative treatment with a clavicle band was undertaken. Shoulder function at the final follow-up visit was satisfactory.
DISCUSSION: SCCH is a rare chronic inflamm …
