Rare Fatal Event of Multidrug-resistant Shigella Septicemia and Cryptococcal Meningitis in an Immunocompromised Host.

Bacteriemia due to Shigella spp. is a rare event. However, an individual with impaired immunity remains at risk of such invasion where the infective dose is as low as 10 viable bacilli. This case report describes one such fatal event where a 40-year-old immunocompromised male presented to a tertiary-level healthcare facility with a history of diarrhea for 10 days and seizures. Prompt microbiological workup established multidrug-resistant Shigella flexneri septicemia and cryptococcal meningitis.

Mixed mycotic infection in an immunocompromised host with advanced HIV diagnosed with the assistance of a Karius test.

We report the case of a 35-year-old female with advanced HIV diagnosed with a mixed mycotic infection. Diagnosis was assisted by Karius test … he utility of building a broad differential diagnosis as mixed mycotic infections can coexist in an immunocompromised host, and of utilizing a Karius test early in the course of illness in an immunocompromised patient to expedite a diagnosis. This case also demonstrates the need to recognize the changing geographic distribution of fungal infections.

Delftia acidovorans Infections in Immunocompetent and Immunocompromised Hosts: A Case Report and Systematic Literature Review.

Delftia acidovorans (D. acidovorans) is a non-fermentative, aerobic, Gram-negative bacillus typically found in environmental sources such as soil and water. Although considered an opportunistic pathogen, it has been implicated in both immunocompromised and immunocompetent individuals. This study presents a case of persistent cathether-related bacteraemia in a 61-year-old haemodialysis patient and offers a systematic literature review of similar cases. The patient, affected by end-stage kidney disease and dependent on a central venous catheter (CVC), presented with septic shock. Blood cultures

Holy water not so holy: Potential source of Elizabethkingia pneumonia and bacteremia in an immunocompromised host.

Elizabethkingia species are Gram-negative, glucose-non-fermenting bacilli predominantly found in soil and water, with Elizabethkingia anophelis increasingly recognized as a human pathogen. E. anophelis has also been reported in hospital outbreaks, suggesting the potential role of contaminated institutional water sources. Conventional microbiological methods often lead to misidentifying this pathogen for other members of the genus Elizabethkingia, suggesting a role for molecular methods for identification. We report a 67-year-old female who developed multiorgan failure requiring intensive care

Low Antigen Titre Disseminated Cryptococcosis in Immunocompromised Hosts: Two Challenging Case Reports.

BACKGROUND Cryptococcus is a yeast-like fungus commonly found in soil and typically causes disease in immunocompromised individuals due to impaired cell-mediated immunity. While the central nervous system (CNS) and lungs are most frequently affected, disseminated disease can involve multiple organs, including the skin, liver, spleen, kidneys, and bone marrow. Symptoms such as headache, fever, and altered mental status often require extensive diagnostic evaluation. CASE REPORT We present 2 unusual cases of cryptococcal growth in the blood without meningoencephalitis, with low cryptococcal antig

Diagnosis and management of cutaneous infections in immunocompromised hosts part II: Neutropenia and humoral immunodeficiency.

Humoral immunodeficiency and neutropenia predispose patients to opportunistic infections and compromise their ability to defend against viral, bacterial, and fungal pathogens. This CME article is designed to educate clinicians on the diagnosis and therapy of infections in patients with humoral immunodeficiency and neutropenia.

How Immunocompromised Hosts Were Left Behind in the Quest to Control the COVID-19 Pandemic.

The immunocompromised population was disproportionately affected by the severe acute respiratory syndrome coronavirus 2 pandemic. However, these individuals were largely excluded from clinical trials of vaccines, monoclonal antibodies, and small molecule antivirals. Although the community of scientists, clinical researchers, and funding agencies have proven that these therapeutics can be made and tested in record time, extending this progress to vulnerable and medically complex individuals from the start has been a missed opportunity. Here, we advocate that it is paramount to plan for future p

Use of Maribavir for Multidrug-Resistant Cytomegalovirus in an Immunocompromised Host With HIV and Common Variable Immunodeficiency.

We present a case of multidrug-resistant Cytomegalovirus (CMV) and the use of maribavir, a novel oral treatment of resistant CMV, in a nontransplant immunocompromised host. Our patient initially was diagnosed with CMV pharyngitis on biopsy. Although his initial CMV pharyngitis resolved, the CMV isolated from blood became resistant to ganciclovir and cidofovir and eventually required treatment with foscarnet. Our patient was given a trial of maribavir. After 6 weeks, the patient's viremia resolved and remained undetectable for 6 months after treatment initiation with maribavir. This case demons…

Disseminated cutaneous Actinomyces bovis infection in an immunocompromised host: case report and review of the literature.

BACKGROUND: Actinomycosis is an uncommon endogenous bacterial infection caused by Actinomyces species, characterized by the development of abscesses, tissue fibrosis, and fistulisation. It remains a diagnostic challenge, due to its similarities with diverse aetiologies' presentation, such as neoplasms, tuberculosis, or fungal infections. Actinomyces bovis is a microorganism rarely reported as a cause of human disease. Cutaneous involvement is sporadic. In this case, Actinomyces bovis was responsible for disseminated cutaneous disease in an immunosuppressed patient. CASE PRESENTATION: We repor

EBV-Positive Lymphoproliferations of B- T- and NK-Cell Derivation in Non-Immunocompromised Hosts.

The contribution of Epstein-Barr virus (EBV) to the development of specific types of benign lymphoproliferations and malignant lymphomas has been extensively studied since the discovery of the virus over the last 50 years. The importance and better understanding of the EBV-associated lymphoproliferative disorders (LPD) of B, T or natural killer (NK) cell type has resulted in the recognition of new entities like EBV+ mucocutaneous ulcer or the addition of chronic active EBV (CAEBV) infection in the revised 2016 World Health Organization (WHO) lymphoma classification. In this article, we review

Invasive trichosporonosis due to Trichosporon asahii in a non-immunocompromised host: a rare case report.

A case of invasive Trichosporonosis due to Trichosporon asahii in an otherwise healthy young adult male presenting as meningoencephalitis and pneumonia is reported here. T. asahii was isolated from cerebrospinal fluid and sputum of the patient and morphologic forms of organism was also demonstrated on direct Gram stain of sputum. The isolate was identified as T. asahii. The patient partially responded to fluconazole therapy. Our case suggests that T. asahii can no longer be linked with Trichospronosis in immunocompromised patient alone and any case of meningitis needs thorough mycological work

Infections in the immunocompromised host: a prospective multicenter survey in patients receiving chemotherapy for acute leukemia.

Patients receiving chemotherapy for acute leukemia were prospectively followed up to determine the frequency, nature and outcome, of episodes of infection during a 6 or 12 month period at each of the participating centres. A total of 605 cycles of chemotherapy were surveyed. Of these, 490 cycles were received by patients with acute lymphoblastic leukemia (ALL) and 115 by patients with acute non-lymphoblastic leukemia (ANLL). 241 episodes of infection were recorded during the survey: 179 among ALL patients and 62 among patients of ANLL. Infections were more common during chemotherapy for ANLL t

Lung abscess due to nontyphoid Salmonella in an immunocompromised host. Case report with review of the literature.

Lung abscess due to nontyphoid Salmonella (NTS) with or without other intestinal or extra-intestinal involvement is very rare. A literature review (Medline search) revealed only 20 cases including ours with this extra-intestinal manifestation of Salmonella infection. The case of a 49-year-old, HIV-positive man from Zaire is reported. Diagnosis was established by direct transthoracal CT-guided puncture of the abscess, a hitherto not reported procedure in this setting. Treatment with oral ciprofloxacin resulted in clinical and radiographic improvement. Underlying immunodeficiency seems to play a

Pathology of Toxoplasma gondii infection in the nude rat. An experimental model of toxoplasmosis in the immunocompromised host?

Genetically athymic nu/nu (nude) rats, deficient in T cells, die from infection with various Toxoplasma gondii strains, including RH and Prugniaud strains. In contrast, these strains cause chronic infections without apparent symptoms in immunocompetent non-nude rats. We show here that nude rats die in two to three weeks after RH infection and three to four weeks after Prugniaud infection. Histological examination of brains from nude rats at different time points after infection, revealed an absence of lesions after RH strain infection and cysts with usually no inflammation after Prugniaud infe

Catheter infection caused by Methylobacterium in immunocompromised hosts: report of three cases and review of the literature.

Three cases of catheter infection due to Methylobacterium extorquens are reported. Each patient had a history of acute leukemia and was immunocompromised; two had undergone bone marrow transplantation, and the third was receiving consolidation chemotherapy. All three patients survived after removal of the central venous catheter and antibiotic treatment. The clinical features of these cases are compared with those of the 12 previously reported cases of infection due to Methylobacterium species.

Disseminated strongyloidiasis in AIDS and non-AIDS immunocompromised hosts: diagnosis by sputum and bronchoalveolar lavage.

In conclusion, disseminated strongyloidiasis is a fatal disease that commonly affects the lungs. The disease should be suspected in an immunocompromised host who came from an area endemic for S stercoralis even years before the onset of symptoms or in patients with unexplained gram-negative bacteremia or meningitis. Treatment should be started promptly and should be maintained for a long time.

Mycobacterium celatum encephalitis in an immunocompromised host mimicking autoimmune striatal encephalitis: the first case report.

BACKGROUND: Encephalitis is rarely caused by nontuberculous mycobacteria (NTM), which is generally not considered a highly virulent pathogen. However, NTM encephalitis in immunocompromised hosts occurs with varied clinical presentations, posing a diagnostic challenge in clinical practice. This study aims to describe an atypical case of NTM encephalitis caused by Mycobacterium celatum, which has not previously been reported to infect the central nervous system of immunocompromised hosts, mimicking autoimmune striatal encephalitis (ASE). CASE PRESENTATION: A 35-year-old immunosuppressed woman p…

Case Report: Duodenal Papillary Stenosis Secondary to Strongyloides stercoralis Infection in a Non-Immunocompromised Host.

Strongyloides stercoralis infection is a common infection in tropical and subtropical regions and is considered one of the "neglected tropical diseases." Owing to its life cycle, this infection can remain undetectable for years, preventing an early diagnosis and prompt treatment. We report the case of a 65-year-old woman who presented to us with a history of nausea, abdominal pain, bloating, and weight loss and, after initial radiologic and laboratory workup, was diagnosed with a periampullary mass with no secondary spread. She underwent an uneventful pylorus-preserving pancreatoduodenectomy,
