Implementation of clinical practice guidelines using the Plan-Do-Study-Act framework: The methodology and experiences of the Academy of Nutrition and Dietetics Health Informatics Infrastructure Registry Study on gestational diabetes mellitus.

BACKGROUND: Registered dietitian nutritionists (RDNs) use clinical practice guidelines (CPGs) to inform evidence-based practice. Despite the availability of CPGs, guidelines are not always translated into practice. This challenge is central to implementation science (IS), which seeks to understand how evidence can be adopted and sustained. METHODS: The Gestational Diabetes Mellitus (GDM) Registry Study was a multiphase, multisite hybrid implementation study that explored guideline implementation using quality improvement (QI) methods grounded in the Model of Improvement and guided by Plan-Do-…

Can an evidence-based mental health intervention be implemented into preexisting home visiting programs using implementation facilitation? Study protocol for a three variable implementation effectiveness context hybrid trial.

BACKGROUND: Perinatal mental health conditions are the most common complication of pregnancy and childbirth (1 in 8 women). When left untreated, perinatal depression and anxiety adversely affects the entire family with pregnancy complications and negative outcomes including preterm birth, impaired mother-infant bonding, impaired lactation, substance abuse, divorce, suicide, and infanticide. Significant disparities persist in the diagnosis and treatment of perinatal depression and anxiety and these inequities are often intersectional. Preliminary research with stakeholders including community a

Application of the consolidated framework for implementation research to understand implementation context of a cardiovascular disease risk-reduction intervention in rural churches.

Rural populations in the USA face higher rates of cardiovascular disease (CVD) incidence and mortality relative to non-rural and often lack access to health-promoting evidence-based interventions (EBIs) to support CVD prevention and management. Partnerships with faith organizations offer promise for translating preventative EBIs in rural communities; however, studies demonstrating effective translation of EBIs in these settings are limited. We used the Consolidated Framework for Implementation Research (CFIR) and a multiple case study approach to understand the role of internal organizational

A case study of the co-creation of the PrEP-SIC: a roadmap to support introduction of pre-exposure prophylaxis into new settings.

BACKGROUND: Pre-exposure prophylaxis (PrEP) is an effective biomedical intervention to prevent HIV, however uptake of PrEP across the United States has been slow and uneven. Understanding "what it takes" to implement PrEP so that it reaches those groups with the highest rates of new diagnoses is a critically important and understudied question. This descriptive case study details the development of an implementation science tool to guide the introduction of PrEP into new settings. METHODS: The Universal Stages of Implementation Completion (U-SIC) was customized for the provision of PrEP servi…

Improving Access to HIV Prevention Services in Community Pharmacies in the US Southeast: Protocol for a Hybrid Type 1 Effectiveness-Implementation Study.

BACKGROUND: Despite advancements in HIV prevention, many Americans, particularly those from historically underserved communities (eg, racially and sexually minoritized individuals and people who use drugs), continue to face significant barriers to accessing crucial HIV prevention services such as HIV testing and pre-exposure prophylaxis. Integrating these services into community pharmacies is a viable yet underused solution to overcoming access-related challenges. However, few studies have used an implementation science approach to assess the implementation and effectiveness of such services i…

Evaluating the implementation of a prisoner re-entry initiative for individuals with opioid use and mental health disorders: Application of the consolidated framework for implementation research in a cross-system initiative.

Given the interrelated nature of opioid use, criminal justice interaction, and mental health issues, the current opioid crisis has created an urgent need for treatment, including medication assisted treatment, among justice-involved populations. Implementation research plays an important role in improving systems of care and integration of evidence-based practices within and outside of criminal justice institutions. The current study is a formative qualitative evaluation of the implementation of a cross-system (corrections and community-based) opioid use treatment initiative supported by Opioi

Comparing different forms of structured follow-up support in the treatment of multiple health risks for chronic disease: a randomized controlled trial protocol.

BACKGROUND: Primary care clinics need effective, low-cost tools to screen for and address multiple health risks - and technology can provide important efficiencies. My Own Health Report (MOHR) is a web-based intervention with 3 core functions: (1) risk flagging; (2) goal-setting for patient-prioritized risks; and (3) providing resources. A prior randomized trial in primary care settings showed that MOHR was feasible and effective. However, we identified two key gaps: a need for a flexible menu of delivery options to adopt and sustain MOHR and a pragmatic way to provide structured follow-up. M

The STop UNhealthy substance use now (STUN II) trial: protocol for a 48-site cluster randomized 2 × 2 factorial implementation trial to improve evidence-based screening and interventions for substance use disorder within primary care.

BACKGROUND: Despite substance use disorders (SUD) being a leading cause of preventable death in the US, most people who visit primary care in the US are not screened for SUD. There are multiple barriers to screening for, identifying, and managing SUD in primary care. However, there are also promising strategies available to address these barriers, including practice facilitation (PF), learning collaboratives (LC), and performance incentives (PI). METHODS: This study is a 48-site cluster-randomized 2 × 2 factorial implementation trial that aims to compare the effectiveness of several strategie
