Mortality and neurodevelopmental outcome after Staphylococcus aureus bacteremia in infants.

We compared outcomes in infants with methicillin-resistant and methicillin-sensitive Staphylococcus aureus bacteremia. Infants with methicillin-resistant S. aureus infection had a longer median duration of bacteremia (4.5 versus 1 day, P = 0.01), but no difference in length of hospital stay, mortality, or neurodevelopmental impairment.

Hyperglycemia and morbidity and mortality in extremely low birth weight infants.

OBJECTIVE: The purpose of this study was to determine the association between hyperglycemia and mortality and late-onset infections (>72 h) in extremely low birth weight (ELBW) infants. STUDY DESIGN: Retrospective analysis of a prospective cohort study of 201 ELBW infants who survived greater than 3 days after birth. Mean morning glucose levels were categorized as normoglycemia (<120 mg/dl), mild-moderate hyperglycemia (120 to 179 mg/dl) and severe hyperglycemia (> or =180 mg/dl). Hyperglycemia was further divided into early (first 3 days of age) and persistent (first week of age). Logistic r

Replacement of estrogens and progestins to prevent morbidity and mortality in preterm infants.

BACKGROUND: A potential therapeutic effect of sex steroids on premature infants has been proposed from animal data and observational studies in humans. Purported benefits include reduction in chronic lung disease, improved bone density and improved neurodevelopmental outcome. OBJECTIVES: To determine if estrogens or progestins, either alone or in combination, when compared to placebo or no treatment, reduce morbidity and/or mortality in preterm infants. SEARCH STRATEGY: The standard search strategy of the Cochrane Neonatal Review Group as outlined in the Cochrane Library (Issue 2, 2004) was

Vitamin E supplementation for prevention of morbidity and mortality in preterm infants.

BACKGROUND: Treating very low birth weight (VLBW) infants with pharmacologic doses of vitamin E as an antioxidant agent has been proposed for preventing or limiting retinopathy of prematurity, intracranial hemorrhage, hemolytic anemia, and chronic lung disease. However, excessive doses of vitamin E may result in side effects. OBJECTIVES: The aim of this systematic review was to assess the effects of vitamin E supplementation on morbidity and mortality in preterm infants. SEARCH STRATEGY: We searched MEDLINE (October 2002), EMBASE (March 2002), the Cochrane Controlled Trials Register (CCTR) f

Adulthood mortality of infants isolated at birth due to tuberculosis in the family.

AIMS: In 1936 the Finnish Anti-Tuberculosis Association founded the first nursery, "Joulumerkkikoti", into which infants born into tuberculous families were admitted and given BCG vaccination to reduce the risk of tuberculosis. This prophylactic regimen was effective in reducing infant mortality and morbidity of tuberculosis. We investigated the mortality of these children later in childhood and adulthood. METHODS: The index cohort consisted of 3,020 subjects born between 1945 and 1965 in Finland and isolated from their family immediately after birth. The average separation time was 218 days.…

Birth weight in relation to morbidity and mortality among newborn infants.

BACKGROUND: At any given gestational age, infants with low birth weight have relatively high morbidity and mortality. It is not known, however, whether there is a threshold weight below which morbidity and mortality are significantly greater, or whether that threshold varies with gestational age. METHODS: We analyzed the neonatal outcomes of death, five-minute Apgar score, umbilical-artery blood pH, and morbidity due to prematurity for all singleton infants delivered at Parkland Hospital, Dallas, between January 1, 1988, and August 31, 1996. A distribution of birth weights according to week o

Mortality of low birthweight infants in England and Wales 1953 to 1979.

National trends in the survival of low birthweight infants for the years 1953 to 1979 were examined. While stillbirth and day 1-28 mortality rates improved steadily throughout this period, day 0 mortality did not start to fall until the mid-1960s and then it improved in all birthweight groups. This suggests that special and intensive baby care units introduced progressively since about 1961 are exerting an appreciable effect on mortality. There is a serious lack of representative data on rates of handicap in the survivors.

Respiratory distress and neonatal mortality in infants of diabetic and prediabetic mothers.

The 149 live births of 19 insulin-dependent diabetic mothers and 62 prediabetic mothers between 1940 and 1970 were reviewed. The frequency of respiratory distress was 31% in infants of diabetic mothers, 2.6% in infants of prediabetic mothers, and 0.46% in all infants born at the same hospital during the same period. Neonatal mortality during the first month of life was 14% in the infants of diabetic mothers and 3.5% in the infants of prediabetic mothers. Both these rates for neonatal mortality are higher than those reported for live births in the United States and Minnesota between 1950 and 19

Mortality of preoperative peritonitis in newborn infants without intestinal obstruction.

Within a period of 15 years 649 neonates were subjected to laparotomy; 60 (9%) of these patients died. Eighty-seven of the patients had a peritonitis already preoperatively without intestinal obstruction. Many of these were cases of ruptured omphaloceles or gastroschisis. In 17 infants a spontaneous intestinal perforation was the cause of the peritonitis. In 13 there was a preexisting meconium peritonitis. Seven children suffered from gangrenous intestine. Further causes for preoperative peritonitis were a complicated enteritis in 7 and a perforated appendix in 2 cases. Twenty-five or 29% of t

Current patterns of neonatal morbidity and mortality in infants of diabetic mothers.

Recent advances in antepartum fetal evaluation have contributed to a marked reduction in fetal deaths in pregnancies complicated by overt diabetes mellitus. To determine the effect of these changes on neonatal morbidity and mortality, a retrospective analysis of complications in 322 infants of diabetic mothers (IDM) in White classes B--R was undertaken. The majority (89 per cent) of the IDM were delivered at term with a mean gestational age of 38 weeks. Neonatal morbidity correlated significantly with gestational age, occurring in 80 per cent of the preterm and 40 per cent of the term infants.

Housing-Based Socioeconomic Status at Birth and Infant Mortality in a Population-Based Cohort.

OBJECTIVE: Socioeconomic status (SES) impacts infant mortality risk, yet measuring individual-level SES in large health datasets remains challenging. We evaluated the association between individual-level SES, measured via the HOUsing-based SocioEconomic Status (HOUSES) index, and infant mortality within a population-based cohort. METHODS: This retrospective cohort study used the Rochester Epidemiology Project (REP) medical records-linkage system to evaluate live births from 2010 to 2021. The primary exposure was individual-level SES at birth, quantified into quartiles (Q1=lowest SES, Q4=highe…

Social Inequalities in Infant Mortality Related to Congenital Anomalies: A Population-Based Register Study.

BACKGROUND: Infant mortality has recently increased in France, especially in deprived areas. Congenital anomalies (CA) are a major cause of infant mortality and could contribute to these trends. OBJECTIVES: This study aimed to investigate socio-spatial inequalities in infant mortality associated with CA. It also examined socio-spatial differences in antenatal detection. METHODS: We included all live births ≥ 22 weeks with CA from the Paris Registry of CA (remaPAR). Births are linked to infant deaths < 1 year using vital statistics records. A case-population design was applied to assess the a…

Black:white inequities in infant mortality across the 69 most populous US cities, 2018-2021.

The United States has poor birth outcomes, including high rates of infant mortality and substantial racial inequities, compared to other developed nations. However, both overall mortality rates and racial inequities in rates vary across locations, emphasizing the structural forces that shape population health. We used mortality and natality data from the National Vital Statistics System to assess racial inequities in infant mortality rates within the most populous US cities for 2018-2021. Specifically, we: (1) calculate overall and race-specific infant mortality rates for 69 cities and racial …

Pre-pregnancy and gestational cardiometabolic disorders and risk of preterm birth and infant mortality.

BACKGROUND: Cardiometabolic disorders (CMDs) are common in pregnancy and can harm the offspring's health. While prior studies have explored clustered cardiometabolic risks in pregnancy, most have focused on a limited number of conditions or a single period. We aimed to examine the associations of individual, multiple, and separate and combined patterns of six pre-pregnancy and gestational CMDs with preterm birth and infant mortality. METHODS: Using data from US National Vital Statistics System (2014-2020), we analysed pre-pregnancy CMDs (body mass index, diabetes, hypertension) and gestationa…

Neonatal and infant mortality after maternal influenza and pertussis vaccination: Probabilistically linked cohort study.

Maternal influenza and pertussis vaccination is an important strategy to reduce morbidity and mortality in infants. Previous vaccine safety studies have mostly focused on the association between maternal vaccination and fetal death. We conducted a probabilistically linked cohort study of mother-infant pairs in three Australian jurisdictions: Queensland, Northern Territory, and Western Australia, to evaluate the risk of all-cause neonatal and infant mortality associated with seasonal influenza and pertussis maternal vaccination. Vaccination records were obtained from regional immunization datab…
