Approach to the neoaortic valve for replacement after the arterial switch procedure in patients with complete transposition.

We report our experience of aortic valvar replacement subsequent to the arterial switch procedure, incorporating the Lecompte maneuver, in patients with complete transposition. The anterior wall of the neoaorta was incised in continuity with the posterior wall of the pulmonary trunk. The aortic root was reconstructed with or without augmentation using a patch after the operative procedures to the aortic valve. With this approach, the neoaortic root could readily be exposed without extensive dissection, and valvar replacement was safely achieved. These patients experienced no postoperative comp

[The integrated management of childhood illness: Haiti's example].

Child health in developing countries is a public health priority both at the national and international level. The World Health Organization, UNICEF and other technical partners have developed The Integrated Management of Childhood Illness (IMCI) strategy to reduce child mortality and improve child health and development through a holistic approach. By the end of 2002, 109 countries among which 17 in the region of the Americas and Caribbean had adopted and implemented this strategy,. In this region, Haiti presents the highest mortality rate for under-fives. Every year, more than 138,000 childr

Transcatheter treatment of muscular ventricular septal defect and pulmonary valvar stenosis in an infant.

An 11-month-old, 8 kg girl with muscular ventricular septal defect and pulmonary valvar stenosis had successful transcatheter treatment of both anomalies. The stenotic pulmonary valve was dilated first by balloon catheter, then the VSD was approached and closed using a 6 mm Amplatzer VSD occluder device. Due to technological improvements, transcatheter techniques are more and more the procedure of choice in a larger number of congenital heart diseases in younger and younger patients.

Recognition and management of childhood cardiac arrhythmias.

Cardiac arrhythmias are common in children, but most of them are benign and do not signify underlying heart disease. However, the main tachyarrhythmias (SVT and VT) can cause cardiovascular collapse, and should therefore be treated promptly. Drug treatment is generally effective and is appropriate in stable patients. DC shock is the treatment of choice in patients with compromised circulation. There is a wide array of antiarrhythmic drugs and though they are generally safe, most of them have the potential for serious adverse effects. Hence, there is a need to exercise caution and balanced judg

Preventive health behaviours among parents of infants aged four months.

Six preventive health behaviours have been frequently identified as having the potential to reduce mortality and morbidity during infancy: breast-feeding until the age of six months; no solid food until after four months of age; immunisation against whooping cough, diphtheria, poliomyelitis and tetanus; the use of a baby capsule to restrain the infant when travelling in a motor vehicle; regular attendance at a health care provider for preventive health checks; and no maternal smoking. This study surveyed 191 primiparous women four months after the birth of their babies to explore the proportio

Incarceration, maternal hardship, and perinatal health behaviors.

Parental incarceration is associated with mental and physical health problems in children, yet little research directly tests mechanisms through which parental incarceration could imperil child health. We hypothesized that the incarceration of a woman or her romantic partner in the year before birth constituted an additional hardship for already-disadvantaged women, and that these additionally vulnerable women were less likely to engage in positive perinatal health behaviors important to infant and early childhood development. We analyzed 2006-2010 data from the Pregnancy Risk Assessment and M

The clinical and genotypic-phenotypic findings of mucopolysaccharidosis VI patients: an Iraqi single-study descriptive study.

BACKGROUND: Maroteaux-Lamy syndrome (mucopolysaccharidosis type VI; MPS VI) is a chronic illness that causes progressive enlargement, inflammation, or scarring of several tissues and organs until their collapse. In most cases, an infant with MPS VI shows no symptoms. The early signs and symptoms of MPS VI in children often develop in the first few months of life. MPS VI affects various systems in the body, including the skeletal, cardiac, and respiratory systems. The authors aim in our study to describe the clinical and genotypic-phenotypic findings of MPS VI patients in 'children Welfare Teac

Socio-demographic risk factors for home-type injuries in Swedish infants and toddlers.

UNLABELLED: In this paper we use data from Swedish national registers to study socio-demographic patterns of hospital admissions as a result of injuries sustained at home (poisoning, falls, scalding and ingestion/intrusion of foreign objects) in children 0-3 y. The study population comprised 546 336 children born in Sweden during the period 1987-91. The different injury mechanisms peaked at different ages: ingestion of foreign objects at 10-12 mo, scald injuries at 13-15 mo, non-drug poisoning at 16-18 mo and drug poisoning at 24-30 mo. In a multivariate analysis it was demonstrated that child

Severe neonatal hyperbilirubinemia and adverse short-term consequences in Baghdad, Iraq.

BACKGROUND: Severe neonatal hyperbilirubinemia, when unmonitored or untreated, can progress to acute bilirubin encephalopathy (ABE). Initiatives to prevent and eliminate post-icteric sequelae (kernicterus) are being implemented to allow for timely interventions for bilirubin reduction. OBJECTIVES: We report an observational study to determine the clinical risk factors and short-term outcomes of infants admitted for severe neonatal jaundice. METHODS: A post-discharge medical chart review was performed for a cohort of infants admitted for management of newborn jaundice to the Children Welfare

Resurrection of vitamin D deficiency and rickets.

The epidemic scourge of rickets in the 19th century was caused by vitamin D deficiency due to inadequate sun exposure and resulted in growth retardation, muscle weakness, skeletal deformities, hypocalcemia, tetany, and seizures. The encouragement of sensible sun exposure and the fortification of milk with vitamin D resulted in almost complete eradication of the disease. Vitamin D (where D represents D2 or D3) is biologically inert and metabolized in the liver to 25-hydroxyvitamin D [25(OH)D], the major circulating form of vitamin D that is used to determine vitamin D status. 25(OH)D is activat

Ethical dilemmas in fetal echocardiography.

Recent technology permits detection of congenital heart diseases as early as the 17th or 18th week of gestation. These detectable defects include hypoplastic left heart syndrome, Ebstein anomaly, and atrioventricular canal malformation. Surgical treatments for these problems raise ethical as well as medical questions regarding patients whose prenatal diagnosis reveals congenital heart disease. We report six cases of in utero diagnosis of hypoplastic left heart syndrome and other complex congenital heart anomalies. Empirical factors relevant to our management were gestational age, severity of t

Comprehensive survey on the use of plastic additives in toy products used in Japan.

BACKGROUND: Plastic additives have adverse effects on human health. Children frequently use toys that contain various substances found in paints, plasticizers, and other materials, which heighten the risk of specific chemical exposure. Infants are particularly prone to chemical exposure through the "mouthing" behavior because of the possibility of placing toys in their mouths. Thus, this vulnerability should be considered during risk assessments of chemical exposure. METHODS: This study performed a comprehensive analysis of the chemical components in various 84 plastic toys including "designa

[Evidence of the impact of vitamin A supplementation on maternal and child health].

The aim of this article was to collect the results of systematic reviews and meta-analyses that evaluated the effect of vitamin A supplementation on child growth and maternal, fetal, and child morbidity and mortality. A detailed search was performed in PubMed, Cochrane Library, LILACS, PAHO, CAPES, USP Digital Thesis Library, and UNIFESP Collection Database. A total of 14 studies published from 1993 to 2006 were included in the review. There is evidence that vitamin A supplementation in children is associated with a reduction of 23% to 30% in mortality risk and attenuation in the severity of m

Antenatal events causing neonatal brain injury in premature infants.

Understanding possible causes of neonatal brain injury is important for perinatal nurses because neonatal brain injury predicts subsequent infant mortality and morbidity in the premature infant. The pathogenesis of the brain injury, germinal matrix/intraventricular hemorrhage and white matter damage, is usually related to a hypoxic event. The hypoxic event may occur in utero, resulting from various conditions, such as maternal infection, maternal alcohol consumption, maternal smoking, placental bleeding disorders, maternal hypercoagulability conditions, metabolic disorders (diabetes and hypert

Perinatal death due to abruptio placentae in an African city.

Abruptio placentae was a common cause of perinatal death in Addis Ababa, Ethiopia in 1974-1975 with a frequency of 5.5/1000 births. This disorder had its peak frequency at term. No abnormalities were found in the placentas to explain the placental abruptions but there were other clues to their genesis. There was a strong association of the fatal abruptions with severe poverty in the mothers. These poor mothers were both undernourished and malnourished during pregnancy. Their fetuses and neonates who died had multiple evidences of undernutrition including a relative undergrowth of adrenals, spl

Delivering services and influencing policy: health care professionals join forces to improve maternal, newborn, and child health.

This article reviews the major activities of health care professional organizations (HCPAs), and emphasizes the role they can play in advocating for women and children and influencing maternal, newborn, and child health (MNCH) programs and policies. The ICM/FIGO joint effort to prevent postpartum hemorrhage and the 40-year partnership between the American Academy of Pediatrics (AAP) and the Indian Health Service (IHS) are highlighted as examples of how and why HCPAs should assume a leadership role in advocacy work. The action-oriented multicountry HCPA workshops organized by the Partnership fo

Cardiac complications in children following infection with varicella zoster virus.

Infection with varicella zoster virus, leading to chicken pox in susceptible hosts, is usually a benign self-limiting disease conferring immunity in those affected. Cardiac complications are rare, but when present may lead to severe morbidity or mortality. We have recently encountered three children, all of whom developed significant cardiac complications secondary to infection with varicella. Myocarditis has long been associated with such infection. The pathological mechanism is presumed similar to other cardiotropic viruses, where both direct cytopathic and secondary auto-immune effects cont
