Hemimasticatory spasm in hemifacial atrophy: diagnostic and therapeutic aspects in two patients.

We report two cases of hemimasticatory spasm in association with progressive hemifacial atrophy. On the basis of neurophysiological and magnetic resonance imaging assessments, a peripheral irritation of the trigeminal nerve--probably due to entrapment of the motor branches in the infratemporal fossa--is suggested as the cause of the involuntary movement. Local injections of botulinum toxin type A into the masticatory muscles proved to be a successful treatment in both patients.

Successful endoscopic endonasal surgery for very huge trigeminal schwannomas in nasopharynx.

We present a 60-year-old female diagnosed with a giant trigeminal tumor (5.2*6.4*8.2 cm) situated in the middle cranial fossa and nasopharyngeal area. The patient was operated on by endoscopic endonasal transmaxillary, transpterygoid and infratemporal approaches. Postoperatively she was stable, with no neurologic complication and no cerebrospinal fluid leakage. We review the literature on extremely large trigeminal schwannomas.

The transzygomatic approach: a long-term clinical review.

The transzygomatic approach has been utilised to improve access to the skull base, infratemporal fossa and orbit for a number of years. It provides a low anterolateral approach to the skull base, along the floor of the middle fossa. It allows both a transsylvian and subtemporal approach with a reduction in brain retraction and better exposure of adjacent neurovasculature structures. A long term review of 53 patients is presented highlighting outcome at two years post surgery and morbidity of the approach. It is concluded that the technique is versatile and can be used to improve exposure of a …

An accessory sphenoidal foramen of the middle cranial fossa detected on computed tomography.

BACKGROUND: The skull base depicts significant morphological variability, which is frequently described due to its neurosurgical significanc … otundum (FR) and 5.5 mm anterior to the foramen ovale (FO). Extracranially, the ASF opened into the infratemporal fossa and coexisted with another sphenoidal emissary foramen (SEF), anteromedially to the FO. On the right side, two SEF were located anteromedially to the FO. CONCLUSIONS: Similar to the current case, ASF of the middle cranial fossa were reported in a p …

Mandibular neuralgia due to anatomical variations.

In our large collection of macerated human adult skulls and disarticulated skulls of young individuals we found cases with an extremely large lateral lamina of the pterygoid process. The medial wall of the infratemporal fossa was defined as its formation by the lateral lamina of the pterygoid process and the medial pterygoid muscle. The muscular part formed two-thirds and the lateral lamina one-third of this wall. In cases of a very large lateral lamina in our specimens nearly the whole medial wall was osseous. The third portion of the trigeminal nerve gives off the lingual nerve and alveolar …

Osteosarcoma of the jaw. A brief review and a case report.

Osteosarcoma of the jaws accounts for 6-13% of all osteosarcomata. If not diagnosed early, it spreads extensively through the jaw and contiguous soft tissues into the nasal cavity, the maxillary sinus, the orbit and the infratemporal fossa, and may invade the oral soft tissues. A clinical finding of tooth displacement and tooth mobility associated with radiolucency/radiopacity should alert the practitioner to the possibility of osteosarcoma or some other malignancy.

Glomus Vagale Tumor Resection: 2-Dimensional Operative Video.

A 37-yr-old female with prior transient left facial paralysis presented with hearing loss, headaches, and resolved transient right facial paralysis. The neurological examination demonstrated normal facial movement, left hearing loss, and left vocal cord weakness. Magnetic resonance imaging demonstrated a >3 cm left paraganglioma traversing the jugular foramen. After obtaining informed consent from the patient, the tumor was embolized and then resected via a combined left postauricular infratemporal fossa and transcervical approach with cranial nerve monitoring. The ossicles were removed and th…

Clear cell meningioma in a child: A case report and review of literature.

An 11-year-old boy presented with holocranial headache associated with vomiting and heaviness in right paranasal region. On examination, he had left sided upper motor neuron type facial palsy along with a subtle ipsilateral hemiparesis (grade 4/5 Medical Research Council). Magnetic resonance imaging brain revealed a contrast enhancing large well-defined extra-axial mass lesion in right middle cranial fossa with extension into the infratemporal fossa. The mass was excised in two stages. Histopathology was suggestive of clear cell meningioma (CCM). Supratentorial pediatric CCM is a rare entity. …

Diagnostic importance of MRI and CT scans for synovial osteochondromatosis of the temporomandibular joint.

Since synovial chondromatosis (SC) clinically mimics symptoms of internal derangements of the TMJ, the diagnostic value of MRI and CT, overlooked for years, is discussed in the presented case. Multiple amorphous calcifications in the left infratemporal fossa and upper synovial compartment of the TMJ were detected on the CT and MRI scans. The patient underwent open TMJ arthrotomy and removal of 15 calcified loose bodies. SC may be diagnosed radiographically when sclerosis of the glenoid fossa, soft tissue edema, and intraarticular radio-opaque loose bodies are detected. Advanced imaging of the …

Primary orbital Ewing sarcoma.

The authors report the clinical manifestations, pathologic character, management, and prognosis of primary orbital Ewing sarcoma, and review pertinent literature. A 6-year-old boy presented with the complaint of abruptly decreasing visual acuity and right proptosis. Computed tomography scan revealed a mass in the region of the right infratemporal orbit with extension into the infratemporal fossa and masseteric space, and there was erosion of the right lateral orbital wall. An anterior orbitotomy eyelid crease was performed to remove the tumor. The tumor cells were positive in membranous patter…

Modified partial maxillary swing approach for myxofibrosarcoma in pterygopalatine fossa.

BACKGROUND: Extirpation of tumors arising in the pterygopalatine fossa is challenging because of its anatomic complexity. METHODS AND RESULTS: A 67-year-old man was referred to our department with a diagnosis of a tumor in his left pterygoid fossa. An incisional biopsy through the canine fossa was diagnosed as myxofibrosarcoma. The upper part of the maxilla was swung laterally to remove the tumor while the hard plate was preserved. The defect was reconstructed using rectus abdominis musculocutaneous free and ipsilateral temporal. The postoperative course was uneventful, without facial palsy o

Osteoplastic maxillotomy approach for infraorbital nerve schwannoma, a case report.

BACKGROUND: Extracranial schwannomas can readily occur in the head and neck region and rarely involve the trigeminal nerve. As a rule, their treatment is surgical and dictated by the location of the tumor and nerve of origin. METHODS: We describe a case of a 14-year-old boy with a mass invading right nasal fossa, maxillary sinus, orbital floor, pterygopalatine fossa, and infratemporal fossa. The diagnosis of a nerve sheath tumor was evoked after angiography showed no vascular blush. RESULTS: The tumor was removed through a Weber-Fergusson incision with subciliary extension followed by maxill…

Immunoglobulin G4-Related Disease Presenting as Temporal Bone Lesion with Facial Nerve Palsy.

Immunoglobulin G4-related disease (IgG4-RD) is a systemic condition which could present as local infiltration of skull base structures. We r … emporal lobe stroke, and brain abscess. Magnetic resonance imaging (MRI) showed infiltration in the infratemporal fossa, nasopharynx, spreading along the Eustachian tube and perineurally along the branches of CN V and CN VII intracranially, forming a dural based mass in the middle cranial fossa. Intracranial mass compressed the temporal lobe of the brain, causing per …

The Endoscopic Endonasal Transmaxillary Approach to Meckel's Cave Through the Inferior Orbital Fissure.

BACKGROUND: Surgical access to Meckel's Cave (MC) is challenging due to its deep location and surrounding important neurovascular structures … o MC. It may be safer than the existing approaches as it does not require transposition of the ICA, infratemporal fossa, and pterygopalatine fossa, and allows access to tumors located anteriorly on the floor of the middle cranial fossa.

Endoscopic Endonasal Transpterygoid Approach: Technical Lessons.

INDICATIONS CORRIDOR AND LIMITS OF EXPOSURE: The endoscopic endonasal transpterygoid approach (EETPA) provides direct access to the petrous apex, lateral clivus, inferior cavernous sinus compartment, jugular foramen, and infratemporal fossa. 1,2 In the coronal plane, it provides exposure far beyond a traditional sphenoidotomy. ANATOMIC ESSENTIALS NEED FOR PREOPERATIVE PLANNING AND ASSESSMENT: The pterygoid process of the sphenoid bone forms the junction between the body and greater sphenoid wing before bifurcating because it descends into medial and lateral plates. The key to this exposure li…
