Compared to systemic chemotherapy, intraarterial administration of tumoricidal agents with or without systemic chemotherapy in the treatment of nonresectable metastatic colorectal cancer is associated with superior response rate. A 79-year-old patient with post-polycythaemic myelofibrosis presented with severe hypersplenism. After splenic artery catheterization, cytosine arabinoside was given intrasplenically from November 1999 to March 2000 for 5 d/month at 10 mg/m2 and increased each month by 10 mg/m2. It was then administered by continuous infusion until June 2000, starting at 20 mg/m2/d and tapering by 5 mg/m2 every 2 weeks to a final daily dose of 5 mg/m2/d. The drug was then stopped. The spleen had decreased to one third of the initial volume. Clinical conditions and haematological indices improved substantially.

The authors developed a dog model for the biliary sclerosis that occurs as a severe complication of protracted hepatic arterial floxuridine (FUDR) infusions (using implanted drug delivery systems) in patients with hepatic cancers. Hepatic regional treatment represents an attempt to improve tumor response by increasing drug concentration with low systemic toxicities. Medicinal lysis with an intra-arterial streptokinase infusion was used in a patient who had suffered multiple embolic occlusions 2 weeks after a myocardial infarction. The good outcome, as verified by arteriography, and the experience of other groups, makes it clear that there is a specific place for this therapy in such patients. We report a case in which a renal scan and angiography revealed a renal artery thrombosis. The only presenting symptom was left flank pain and no predisposing factors could be found on clinical or laboratory examination. Local infusion of streptokinase through an intra-arterial catheter led to complete dissolution of the thrombus despite the fact that it had been present for 13 days. A therapeutic regimen is described for sedative, analgesic, and anti-emetic effect in patients receiving intra-arterial carmustine (BCNU) for malignant gliomas. This regimen consists of nalbuphine, 30 mg, i.v., and droperidol, 2.5 mg, i.v., given immediately prior to intra-carotid BCNU infusion. Droperidol, 2.5 mg, i.v., is then administered on four hour intervals for sixteen hours post-procedure.
