Lack of discreet colocalization of epithelial apoptosis to the atretic precursor in the colon of the Fibroblast growth factor receptor 2IIIb mouse and staining consistent with cellular movement suggest a revised model of atresia formation.

BACKGROUND: Colonic atresias in the Fibroblast growth factor receptor 2IIIb (Fgfr2IIIb) mouse model have been attributed to increased epithelial apoptosis and decreased epithelial proliferation at embryonic day (E) 10.5. We therefore hypothesized that these processes would colocalize to the distal colon where atresias occur (atretic precursor) and would be excluded or minimized from the proximal colon and small intestine. RESULTS: We observed a global increase in intestinal epithelial apoptosis in Fgfr2IIIb -/- intestines from E9.5 to E10.5 that did not colocalize to the atretic precursor. Ad

Polysplenia complex and duodenal atresia. A case report.

The association between splenic abnormalities, congenital heart defects, and abnormalities in the location and shape of other viscera is well established in the literature. Intestinal atresia is rarely found with either the polysplenia or asplenia syndromes. We studied a case of polysplenia syndrome accompanied by duodenal atresia; we believe that duodenal atresia is a rare but predictable complication of the polysplenia complex.

[Problems of anesthesia and recovery in neonatal surgery. Personal experience during ther period 1970-1975].

On the basis of personal experience during the past five years, the anaesthesiological and resuscitatory problems of neonatal surgery are considered, with special regard to the following pathological conditions: oesophageal atresia, diaphragmatic hernia, omphalocele and gastroschisis, neonatal peritonitis, anal atresia, intestinal atresia and stenosis. It is stated that the new post-operative care tecniques have not only improved survival percentages, but, in the case of oesophageal atresia, have made it possible to operate (radically) even in seriously immature babies with ongoing complicatio

Outcomes of refeeding through mucous fistula based on indications in neonatal surgery.

OBJECTIVE: Mucous fistula refeeding (MFR) preserves intestinal function in ostomized neonates by reinfusing proximal output into the distal limb. We aimed to define its indications, effectiveness and complications. MATERIAL AND METHODS: A retrospective review of neonates ostomized after neonatal surgery who received MFR for bowel stimulation/decompression and/or to reduce parenteral nutrition (PN) dependence and complications. Patients were categorized into four groups based on the underlying pathology: intestinal atresia, meconium ileus, intestinal ischemia and necrotizing enterocolitis (NEC

Delayed primary surgery and outcomes in children with gastrointestinal anomalies in 264 hospitals and 74 countries.

The impact of delayed presentation to primary surgery on mortality and complication outcomes in children with gastrointestinal congenital anomalies is poorly understood. Using a cohort of 3767 children with gastrointestinal anomalies in 74 countries (2018-2019), we assessed predictors of delay and compare 30-day risk of all-cause mortality (ACM) and complication among children with delayed ($>$24 hours of life) versus non-delayed ($\le$24 of life) presentation to primary surgery. Children in low-and middle-income countries (LMICs) had greater probability of delay (PR: 1.79; 95% CI: 1.59, 2.02)

MRI of fetal GI tract abnormalities.

We describe the magnetic resonance (MR) patterns of a variety of fetal gastrointestinal (GI) abnormalities. Thirty-two fetuses between 23 and 38 weeks' gestation with abnormal appearance of the GI tract by ultrasound underwent MR imaging with T1- and T2-weighted sequences. The MR aspect of intestinal atresia (duodenal atresia, one case; small bowel atresia, nine cases) included dilatation of the bowel loops, accurate assessment of the normal bowel distal to the atresia (except in the patient with multiple atresia and apple-peel syndrome), and micro-rectum with decreased T1 signal (except in th

Colonic atresia: a clinicopathological insight into its etiology.

BACKGROUND: The pathogenesis of colonic atresia (CA) has not been discussed in detail, although the theory of a prenatal vascular insult, hypothesized from patients with small bowel atresia, has been accepted in the past. This review questions this etiology by reviewing all patients with CA treated in two institutions. MATERIAL AND METHODS: A retrospective analysis was done of the medical notes of 30 patients with CA treated in two tertiary centers of neonatal surgery in Glasgow, UK, and in Wroclaw, Poland, over a 30-year period with special emphasis on the intraoperative findings and morphol

[Preventive operations in intestinal abnormalities].

In intestinal anomalies, preventive surgical procedures include gastrostomies and enterostomies as well as some special operations such as ligature of the cardia and associated appendectomies. General indications for enterostomies are seen in life-threatening circumstances, i.e. prematurity, surfactant deficiency, major associated malformations and complications. Preventive surgical procedures are necessary in certain cases of esophageal atresia, complicated small bowel atresia, meconium ileus, anorectal malformations, Hirschsprung disease, neuronal intestinal dysplasia and necrotizing enteroc

Atresia of the ascending colon: a rarity.

Atresia of the colon is among the rare types of all gastrointestinal atresias. Ascending colon is the rarest site of all the colonic atresias. The authors report a case of a 4-day-old male baby who presented with the features of distal intestinal obstruction. At laparotomy type I atresia of the ascending colon, just distal to cecum, was found. Primary ceco-colic anastomosis along with a covering ileostomy was performed. Ileostomy was reversed 3 weeks later.

Ileal atresia associated with Hirschsprung disease (total colonic aganglionosis).

One of the most common causes of small bowel obstruction in newborns is ileal atresia, and one of the most common causes of colonic obstruction in neonates is aganglionic megacolon (Hirschsprung disease). However, atresias of the small intestinal tract associated with Hirschsprung disease are extremely rare. We describe an infant born with both ileal atresia and Hirschsprung disease. This is the 19th known report of the case of an infant who had ileal atresia associated with Hirschsprung disease.

Colonic atresia: a rare entity in the newborn. A six-case report and a bibliographic review.

INTRODUCTION: Colonic atresia is a rare malformation accounting for 1.8-15% of all intestinal atresias. We present a 6-case series along with a bibliographic review. CLINICAL CASE: This 6-case series consists of three female cases and three male cases diagnosed 24-84 hours following birth. They were all located in the right hemicolon. The most relevant clinical signs included abdominal distension, absence of defecation, and bilious to fecal vomit. Repairs included primary or step-by-step anastomoses for bowel transit reconstruction. One patient diagnosed at > 72 hours of life died. CONCLUSIO

Alterations of Cajal cells in patients with small bowel atresia.

PURPOSE: Interstitial cells of Cajal (ICC) are regarded as the pacemaker cells of the gastrointestinal tract. There are some well-designed studies investigating the structure and function of ICC subsequent to experimentally induced intestinal obstructions. However, it remains unclear whether reduction of number of ICC primarily leads to mechanical obstruction of the bowel such as seen in intestinal atresia. We aimed to investigate the number of ICC in proximal and distal parts of the atresias of patients with small bowel atresia. PATIENTS AND METHODS: Twenty-one patients (13 male and 8 female

Staged Repair Using Modified Bishop-Koop Procedure in Complicated Congenital Colonic Atresia in a Neonate.

Colonic atresia (CA) is the rarest type of intestinal atresia and is defined as an obstruction in the large intestinal lumen. This is a rare case presentation of a four-day-old full-term female patient presented with signs and symptoms of intestinal obstruction. Investigation revealed that she had complicated CA located in the splenic flexure. Laparotomy and colostomy were done on the patient. About two months later, she was admitted for stoma closure, which was converted to modified Bishop-Koop stoma. Lastly, the patient underwent a successful stoma closure. Upon one month of follow-up, the p

The congenital pouch colon can be used for vaginal reconstruction by longitudinal splitting.

Congenital pouch colon (CPC) is a rare malformation in which the distal part of a shortened colon forms a dilated pouch. It is associated with an anorectal malformation. We report 2 patients with CPC, one with a cloaca and one with vestibular fistula and vaginal atresia. It is the first description of CPC, vestibular fistula, and vaginal atresia. The purpose of this report was to demonstrate that the pouch can be split longitudinally--in analogy with Bianchi's intestinal lengthening procedure [Bianchi A. Perinatal survival of a fetus with intestinal volvulus and intussusception: a case report and review of the literature.

Fetal intestinal volvulus is a rare life-threatening condition. Late diagnosis of volvulus contributes to high rate of morbidity and mortality. It has variable degrees of presentation and survival. Intrauterine volvulus may be complicated by intestinal atresia due to ischemic necrosis. To our knowledge, there are three reported cases of term fetal demise. We report a case of fetal intestinal volvulus with perinatal survival of the largest term infant described with this complication to date. The volvulus was associated with type 3A jejunal atresia and intestinal pathology was noted on prenatal

The Combination of Gastroschisis, Jejunal Atresia, and Colonic Atresia in a Newborn.

We encountered a rare case of gastroschisis associated with jejunal atresia and colonic atresia. In our case, the jejunal atresia was not discovered for 27 days after the initial abdominal wall closure. The colonic atresia was not discovered for 48 days after initial repair of the gastroschisis secondary to the rarity of the disorder. Both types of atresia were repaired with primary hand-sewn anastomoses. Other than the prolonged parenteral nutrition and hyperbilirubinemia, our patient did very well throughout his hospital course. Based on our case presentation, small bowel atresia and colonic

[Biliary atresia and polysplenia syndrome].

Polysplenia syndrome is a rare malformation characterized by the association of multiple rates and other congenital anomalies dominated by cardiac, vascular, intestinal and bile malformations. We report the observation of a patient operated in the neonatal period (3 days) for an upper intestinal obstruction with situs inversus. Surgical exploration noted the presence of multiple rates, a preduodenal vein, a biliary atresia and a duodenal atresia. The surgical procedures performed were a latero-lateral duodeno-duodenostomy and hepatoportoenterostomy of KASAI with simple immediate and delayed ou

Unusual presentations of gastroschisis.

Gastroschisis is a defect of the ventral abdominal wall nearly always located to the right of the umbilicus. A relatively low incidence of associated anomalies exists with the most common being nonduodenal intestinal atresia occurring in 10 per cent of patients. Although multiple hypotheses have been proposed the pathogenesis of gastroschisis is unproven and controversial. Herein we describe four infants with rare presentations of gastroschisis: one infant with true left-sided gastroschisis and three infants with unusual patterns of intestinal atresia. At present there is a female preponderanc
