Simultaneous papillary muscle avulsion and free wall rupture during acute myocardial infarction. Intra-aortic balloon pump: a bridge to survival.

Mechanical complications of acute myocardial infarction (AMI) are rare, but often fatal. Medical therapy does not provide adequate risk reduction, and surgical correction is recommended when feasible. Supplemental hemodynamic support utilizing intra-aortic counterpulsation with a balloon pump provides an improvement in morbidity and mortality when combined with a corrective surgical approach. We report a case of an elderly male with a progressive 2-week history of ischemic symptoms presenting with acute pulmonary edema, hypotension and an inferior wall ST-elevation MI. His hospital course was

Enhanced intra-aortic balloon pump: markedly improved systemic hemodynamics and cardiac function in canines with severe, acute left ventricular failure.

Intra-aortic balloon pumps (IABPs) cannot sustain hemodynamics if the left heart is severely injured. An enhanced IABP was evaluated in 6 anesthetized dogs with acute stenosis of the left anterior descending coronary artery, regional left ventricular (LV) stunning, and global LV dysfunction. An IABP balloon was inserted into the descending aorta and an external chamber containing another IABP balloon was connected to the aorta through a catheter inserted into the left subclavian artery. This emulated the enhanced IABP with a conduit from its external chamber passing axially through an internal

Cardiopulmonary bypass and intra-aortic balloon pump use is associated with higher short and long term mortality after transcatheter aortic valve replacement: a PARTNER trial substudy.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) with the balloon-expandable Sapien transcatheter heart valve improves survival compared to standard therapy in patients with severe aortic stenosis (AS) and is noninferior to surgical aortic valve replacement (AVR) in patients at high operative risk. Nonetheless, a significant proportion of patients may require pre-emptive or emergent support with cardiopulmonary bypass (CPB) and/or intra-aortic balloon pump (IABP) during TAVR due to pre-existing comorbid conditions or as a result of procedural complications. OBJECTIVES: We hypothesize

Supported percutaneous coronary intervention using a novel 6-Fr intra-aortic balloon pump catheter via the brachial artery in a nonagenarian patient with an abdominal aortic aneurysm.

Intra-aortic balloon pump (IABP) counterpulsation is a useful hemodynamic assist device during complex percutaneous coronary intervention (PCI) in patients with poor left ventricular function; however, the presence of an abdominal aortic aneurysm poses a problem, because insertion via the femoral artery may cause distal embolism and aneurysm rupture. A 92-year-old man with unstable angina was admitted to our hospital. Coronary angiography revealed chronic total occlusion of the proximal left anterior descending artery and severe stenosis of the left circumflex artery (LCX). The left ventricula

Effects of Concomitant Intra-Aortic Balloon Pump Usage and Different Cannulation Techniques on Venoarterial Extracorporeal Membrane Oxygenation Support in Terms of Organ Perfusion.

INTRODUCTION: Various cannulation strategies for venoarterial extracorporeal membrane oxygenation (V-A ECMO) support are currently in use according to the clinical urgency and experience of the rescuing team. Although central V-A ECMO is considered more effective than a peripheral approach, the superiority of one cannulation configuration instead of another remains a controversial subject. This study mainly aims to compare the contribution of V-A ECMO circulatory support modalities to patients' improvement according to various cannulation site strategies and additional usage of intra-aortic ba

Intra-aortic balloon pump after VA-ECMO reduces mortality in patients with cardiogenic shock: an analysis of the Chinese extracorporeal life support registry.

BACKGROUND: The role of intra-aortic balloon pump (IABP) combined with venoarterial extracorporeal membrane oxygenation (VA-ECMO) in patients with cardiogenic shock (CS) remains unclear. This study investigated the effect of applying IABP for left ventricle (LV) unloading after VA-ECMO on reducing mortality in patients with CS. METHODS: Data from 5,492 consecutive patients with CS treated with VA-ECMO between January 2017 and July 2023 were collected from the CSECLS registry. The primary outcome was in-hospital mortality. The secondary outcomes included 30-day mortality, survival on VA-ECMO,

Intra-aortic balloon pump impacts the regional haemodynamics of patients with cardiogenic shock treated with femoro-femoral veno-arterial extracorporeal membrane oxygenation.

AIMS: To investigate the impact of intra-aortic balloon pump (IABP) on the regional haemodynamics of patients with severe cardiogenic shock undergoing femoro-femoral veno-arterial extracorporeal membrane oxygenation (VA-ECMO). METHODS AND RESULTS: From July 2017 to April 2018, a total of 39 adult patients with cardiogenic shock receiving both IABP and ECMO for circulatory support were enrolled consecutively in a university-affiliated cardiac surgery intensive care unit. The blood flow rates (BFRs) of the bilateral femoral artery (IABP side: iFA, ECMO side: eFA) and carotid artery (left: LCA,

Dynamic left ventricular outflow tract gradient resulting from Takotsubo cardiomyopathy ameliorated by intra-aortic balloon pump counterpulsation: a case report.

BACKGROUND: Takotsubo cardiomyopathy is a variant of acute coronary syndrome with characteristic acute left ventricular apical ballooning. Uncommonly, there can be associated left ventricular outflow tract (LVOT) obstruction causing cardiogenic shock refractory to inotropic support. The use of afterload-reducing mechanical support such as intra-aortic balloon pump (IABP) counterpulsation is not routinely employed in instances of this kind. CASE SUMMARY: In our case report, we describe a 66-year-old female with acute Takotsubo cardiomyopathy and associated LVOT obstruction which failed to resp

Early intra-aortic balloon pump in acute decompensated heart failure complicated by cardiogenic shock: Rationale and design of the randomized Altshock-2 trial.

BACKGROUND: Cardiogenic shock (CS) is a systemic disorder associated with dismal short-term prognosis. Given its time-dependent nature, mechanical circulatory support may improve survival. Intra-aortic balloon pump (IABP) had gained widespread use because of the easiness to implant and the low rate of complications; however, a randomized trial failed to demonstrate benefit on mortality in the setting of acute myocardial infarction. Acute decompensated heart failure with cardiogenic shock (ADHF-CS) represents a growing resource-intensive scenario with scant data and indications on the best mana

Additional Use of a 6-Fr Intra-Aortic Balloon Pump on Extracorporeal Membrane Oxygenation Was Effective in a Patient with Cardiogenic Shock with Low Pulse Pressure.

We report the case of a 79-year-old man with acute myocardial infarction caused by left main trunk lesion, who experienced cardiogenic shock during percutaneous coronary intervention (PCI). To reverse the cardiogenic shock, we initiated veno-arterial extra corporeal membrane oxygenation (VA-ECMO) without an intra-aortic balloon pump (IABP) due to the severe tortuosity of the left external iliac artery. Although PCI was successful, arterial pressure monitoring revealed that the pulse pressure was too low to recover from the cardiogenic shock of decreased cardiac contraction function (the left v

Risk factors of septicemia and perioperative myocardial infarction in a cohort of patients supported with intra-aortic balloon pump (IABP) in the course of open heart surgery.

In 1325 open heart operated (OHO) patients with a perioperative mortality of 5.8% the incidence of septicemia and perioperative myocardial infarction (PMI) were much higher in a cohort of 110 patients given intra-aortic balloon pump (IABP) support during the operative course. Analysis of this cohort showed that peri/postoperative insertion of the pump, the presence of disease in the descending branch of left coronary artery (LAD) and the need of more than one saphenous vein graft were risk factors for PMI. The presence of LAD disease was the only independent risk factor for PMI with an odds ra

Treatment of acute myocardial infarction complicated by ventricular septal rupture with delayed surgery following intra-aortic balloon pump support: A case report.

RATIONALE: Inferior wall myocardial infarction (MI) complicated by ventricular septal rupture (VSR) is a rare but fatal complication of acute MI. Early surgical intervention is associated with a high mortality rate, and the optimal timing for surgery remains uncertain. This paper presents a case of surgical management of acute inferior wall MI complicated by VSR, with the aim of exploring diagnostic and therapeutic strategies, as well as the timing of surgical intervention. PATIENT CONCERNS: A 74-year-old man was admitted to our hospital with chest tightness and tachypnea lasting 3 days. He h

UNOS policy change is associated with attenuated risk of treated rejection in intra-aortic balloon pump-supported patients and no change with other MCS devices.

Objective. Whether the increased use of temporary mechanical circulatory support (tMCS) as a bridge to heart transplantation following the 2018 United Network for Organ Sharing (UNOS) policy change has translated to a different risk of treated rejection episodes is unknown.Approach. We analyzed adult heart transplant recipients listed and transplanted between November 1, 2010 and October 31, 2020 from the UNOS registry. Multinomial logistic regression was used to estimate relative risk ratios (RRRs) for post-transplant treated rejection, using patients who were alive without treated rejection

Comparative outcomes of intra-aortic balloon pump versus impella in non-myocardial infarction cardiogenic shock: A National Inpatient Sample Database analysis.

BACKGROUND: Intra-aortic balloon pumps (IABPs) and percutaneous left ventricular assist devices (pLVADs; Impella) are used to stabilize patients with cardiogenic shock (CS). While recent evidence supports their usefulness in myocardial infarction (MI)-related CS, data on non-MI CS, particularly regarding newer Impella models, remain limited. We compared outcomes associated with IABP versus various Impella devices in non-MI CS. METHODS: We retrospectively analyzed National Inpatient Sample (NIS) data from 2016 to 2022 to identify patients with non-MI CS who received IABP, Impella 2.5/CP, or Im

The intra-aortic balloon pump as a rescue device: Do we need to shift our strategy for cardiogenic shock rescue after cardiac surgery?

OBJECTIVE: The intra-aortic balloon pump (IABP) is widely used to rescue patients from complications following cardiac surgery. Given improvements in rescue strategies over the past decade, the appropriateness of IABP must be reexamined. This study assessed the risk factors, outcomes, and predictors of survival of rescue IABP placement. METHODS: Patients receiving an isolated rescue IABP during or after cardiac surgery from 2012 to 2020 were studied. All adult patients undergoing cardiac surgery except transplantation and primary mechanical circulatory support (MCS) procedures were included.
