Accuracy of unguided and ultrasound guided Coracohumeral ligament infiltrations - a feasibility cadaveric case series.

BACKGROUND: Coracohumeral ligament (CHL) thickening, contracture, and fibroplasia have been identified in glenohumeral idiopathic adhesive capsulitis (GHIAC). The CHL is the main structure responsible for the range of motion limitations. Favorable outcomes have been reported with CHL surgical release. Intra-articular glenohumeral joint corticosteroid infiltrations are utilized to disrupt the inflammatory process and reduce pain in GHIAC. The aim of this study was to investigate whether the CHL could be accurately targeted with a periligamentous infiltration. METHODS: A convenience sample of 1

Fengshi Gutong capsule alleviates osteoarthritis progression by targeting IGF2BP3 to regulate ANGPTL2 mRNA m6A methylation.

BACKGROUND: The Fengshi Gutong capsule (FSGTC), a commonly used traditional Chinese medicine formula for treating joint pain caused by osteoarthritis (OA), has demonstrated significant efficacy in both clinical and experimental studies. However, the specific mechanism through which FSGTC alleviates the pathological features of OA remains unclear. PURPOSE: To elucidate the key pathological features of OA and determine the therapeutic effects of FSGTC and its underlying molecular mechanism, thereby providing new targets and strategies for clinical intervention. STUDY DESIGN: Methods: An OA rat

Shoulder pain: are there predictive factors of response to treatment and of ultrasound findings?

INTRODUCTION: Shoulder pain is a common cause of consultation in Primary Health Care, and may correspond to up to 30% of the reasons for consultation. Pathology of the rotator cuff is the most common cause of pain. Ultrasound is a valuable diagnostic tool in assessing shoulder disorders; it can be as effective as magnetic resonance imaging. OBJECTIVE: To determine the predictive factors of response to treatment and ultrasound findings in shoulder pain. METHODS: We performed an analysis of the patients' cases sent to the rheumatology consultation with shoulder pain, every patient had an echog

Paediatric Sternoclavicular Joint Pseudodislocation Fixation Technique.

BACKGROUND: Posterior sternoclavicular joint (SCJ) injuries in children are rare and frequently misdiagnosed. Due to the anatomy of the immature medial clavicle, many of these injuries are Salter-Harris type I/II physeal fractures rather than true joint dislocations-termed pseudodislocations. These injuries require careful diagnosis and tailored management to ensure safe and functional outcomes. INDICATIONS: Surgical intervention is indicated in paediatric SCJ pseudodislocations with significant posterior displacement of the medial clavicular fragment, particularly when the posterior metaphys

Scleral structure, organisation and disease. A review.

Although disease of the sclera is unusual, when it occurs it can rapidly destroy both the eye and vision. However, normally the sclera provides an opaque protective coat for the intraocular tissues and a stable support during variations in internal pressure and eye movements, which would otherwise perturb the visual process through distortion of the retina and the lens/iris diaphragm. This stability, which is vital for clear vision is made possible by the organisation and viscoelastic properties of scleral connective tissue. Microscopically, the sclera displays distinct concentric layers inclu

[Analysis of anterolateral approach and lateral approach for the treatment of coronal shear fracture of the distal humeral].

OBJECTIVE: To treat the coronal shear fracture of the distal humeral during open reduction and internal fixation by anterolateral approach and lateral approach, and to analyze the advantage and disadvantage of each approach. METHODS: From September 2006 to July 2014, 10 patients with coronal fracture of the distal humeral were analyzed, who were all treated with Open Reduction and Internal Fixation (ORIF), 5 with anterolateral approach (group A) and 5 with lateral approach (group B). For the anterior-lateral approach, the radial nerve and brachioradialis were retracted laterally and the brach

Does the intervention for adhesive capsulitis in patients with diabetes differ from that for patients without diabetes?: A systematic review.

BACKGROUND: Adhesive capsulitis (AC) is often accompanied by pain and restriction in range of movement, which long lasting symptoms often have negative impact on patients' work and lives. Since diabetes mellitus (DM) is an independent risk factor for developing AC, and AC with DM have more limitations of joint motion than general people. AIMS: The aim of the present study is to evaluate the difference of clinical efficacy between diabetes and non-diabetes, and to explore whether AC with diabetes need special intervention compared to non-diabetes. METHODS: Pubmed NCBI and Embase were searched

The direct anterior approach: initial experience of a minimally invasive technique for total hip arthroplasty.

BACKGROUND: Less invasive approaches for hip arthroplasty have been developed in order to decrease traumatisation of soft tissue and shorten hospital stay. However, the benefits with a new technique can be at the expense of a new panorama of problems. This manuscript describes, with emphasis on postoperative complications, our experience from the first 200 cases of unilateral hip replacement using the direct anterior minimally invasive (MIS) approach. METHODS: A straight incision in front of the greater trochanter was used and the tensor muscle was approached subfascially and retracted latera

Flexor Pronator Slide Under Local Anesthesia without a Tourniquet for Non-Ischemic Contractures of the Forearm.

BACKGROUND: The flexor pronator slide is an effective treatment option for ischemic contracture and contracture related to spastic cerebral palsy, but little is known about the use of the flexor pronator slide in other non-ischemic contractures. I propose a flexor pronator slide to simultaneously correct wrist and finger flexor contractures and preserve the muscle resting length. To avoid overcorrection of the deformity, I propose the use of a wide-awake local anesthesia with no tourniquet (WALANT) procedure, in which the patient is able to continually assist the surgeon in assessing the contr

Treatment of Congenital Vertical Talus: Comparison of Minimally Invasive and Extensive Soft-Tissue Release Procedures at Minimum Five-Year Follow-up.

BACKGROUND: The most common historical treatment method for congenital vertical talus is extensive soft-tissue release surgery. A minimally invasive treatment approach that relies primarily on serial cast correction was introduced almost ten years ago, with promising early results. The purpose of this study was to assess the long-term outcome of patients with congenital vertical talus managed with the minimally invasive technique and compare them with a cohort treated with extensive soft-tissue release surgery. METHODS: The records of twenty-seven consecutive patients with vertical talus (for

Latarjet Procedure for Recurrent Anterior Shoulder Instability.

BACKGROUND: The Latarjet procedure is widely used to treat recurrent anterior shoulder instability, especially in cases with significant glenoid bone loss. The procedure involves transferring the coracoid process to the anterior inferior glenoid. Typically performed as an open surgery, arthroscopic Laterjet techniques have evolved, each offering unique benefits and challenges. INDICATIONS: The Latarjet procedure is indicated for patients with recurrent anterior shoulder instability, particularly those with substantial glenoid bone loss (typically >20%), or failed soft tissue (Bankart) repairs

Genetic subtraction reveals divergent pathways and targets in anxiety-related and anxiety-independent TMD.

BACKGROUND: Temporomandibular disorders (TMD) show substantial clinical and genetic overlap with anxiety, yet it remains unclear whether TMD risk reflects shared anxiety-related liability or distinct anxiety-independent genetic mechanisms. Disentangling these components is essential for understanding TMD heterogeneity beyond symptom-based classifications. METHODS: We applied GWAS-by-subtraction using genome-wide summary statistics for TMD (20,799 cases and 479,549 controls; FinnGen Release 12) and anxiety disorders (74,973 cases and 400,243 controls), partitioning TMD heritability into two ort

Arthroscopic Treatment of Tenosynovial Giant Cell Tumor of the Hip With Concomitant Femoracetabular Impingement Syndrome.

BACKGROUND: Tenosynovial giant cell tumor (TGCT) is a benign but locally aggressive proliferative disorder of the synovium. TGCT of the hip is destructive to the joint and is a risk factor for early total hip arthroplasty (THA). Open synovectomy has previously been the gold-standard treatment. However, hip arthroscopy has emerged as a less invasive approach for the management of TGCT of the hip. INDICATIONS: Arthroscopic management of TGCT of the hip is indicated when there is either confirmed TCGT on biopsy or strong suspicion for TCGT based on clinical and radiologic findings, minimal-to-no

Open Bankart Repair with Subscapularis Split.

BACKGROUND: Anterior shoulder dislocations are a common injury, especially in the young, active, male population1. Soft-tissue treatment options for shoulder instability include arthroscopic or open Bankart repair, with open Bankart repair historically having lower rates of recurrence and reoperation, faster return to activity2-4, and a similar quality of life compared with arthroscopic repair5. More recent literature has suggested similar recurrence rates between arthroscopic and open procedures6. However, open Bankart repair may be indicated in cases of recurrent instability, especially if t
